PERSONAL IDENTITY PROTECTION Definition:

Personal Identity Protection offers reimbursement for covered expenses you incur to restore your identity, up to the maximum amount allowed for the product enrolled, as a result of a Covered Stolen Identity Event.

A Stolen Identity Event means the theft or unauthorized or illegal use of your name, demand deposit account or account number, Social Security number, or any other method of identifying you.

Eligibility:

To be eligible for Personal Identity Protection, you must be an eligible accountholder, in good standing, of a participating financial institution and you must reside in the U.S. or Canada. Coverage is split among joint account holders.

Overview:

Covered costs under the Personal Identity Theft benefit are:

 Costs you incur for re-filing applications for loans, grants, or other credit or debt instruments that are rejected solely because the lender received incorrect information as a result of a Covered Stolen Identity Event.

 Costs for notarizing affidavits or other similar documents, long distance telephone calls, and postage reasonably incurred as a result of your efforts to report a Covered Stolen Identity Event or to amend or rectify records as to your true name or identity as a result of a Covered Stolen Identity Event.

 Costs incurred by you for a maximum of four (4) credit reports, requested as a result of a Covered Stolen Identity Event, from any entity approved by the Benefit Administrator.

 Actual lost wages for time taken away from your work premises solely as a result of your efforts to amend or rectify records as to your true name or identity as a result of a Covered Stolen Identity Event.

 Costs for reasonable fees for an attorney appointed by the Benefit Administrator and related court fees you incur with the consent of the Benefit Administrator for suits brought against you by a creditor or collection agency or similar entity acting on behalf of a creditor for nonpayment of goods or services or default on a loan as a result of a Covered Stolen Identity Event.

Costs not covered are:

 Any dishonest, criminal, malicious, or fraudulent acts by you.

 Any damages, loss, or indemnification.

 Costs associated with any legal action or suit other than those set forth under Covered costs.

 Sick days and any time taken from self-employment.

 Any costs as a result of theft or unauthorized use of an account by a person to whom the account has been entrusted.

Payment for Covered costs will be limited to costs incurred in the United States, its territories and possessions, Puerto Rico, or Canada for a loss occurring during the benefit period.

Operating Procedures: To file a claim:

 Call our Benefit Administrator at 855-822-9464 immediately when you reasonably believe a Covered Stolen Identity Event has occurred and provide information including, but not limited to, how, when, and where the Covered Stolen Identity Event occurred.

 A signed, sworn proof of loss or affidavit containing the information requested by the Benefit Administrator must be submitted within sixty (60) days.

 If you reasonably believe that a law may have been broken, you must promptly file a report with the police.

 The Benefit Administrator may also require other reasonable information or documents regarding the loss.

 Once your claim has been verified, under normal circumstances, reimbursement will be initiated within five (5) business days of receipt and approval of all

required documents. Other Information:

PAYMENT CARD PROTECTION Definition:

Payment Card Protection gives you access to a fraud specialist if your credit or debit cards are lost or stolen.

Eligibility:

To be eligible for Payment Card Protection, you must be an eligible accountholder, in good standing, of a participating financial institution.

Overview:

You are entitled to access to a fraud specialist to assist with cancelling the stolen or lost cards and reissuing new cards. There is no need to register your payment cards in advance to use Payment Card Protection.

Operating Procedures:

 Call BaZing Customer Service at 855-822-9464.

 BaZing will connect you with a fraud specialist.

 The fraud specialist will assist with pulling your online credit report and researching related information.

 The fraud specialist will initiate a call with you and each affected creditor and/or bank.

 With all parties on the phone, the request will be made to cancel existing credit/bank accounts and to have new cards issued.

 If you prefer, you may choose to speak directly to the card issuers without assistance from the fraud specialist (we will provide you with card issuer contact information from your credit report).

 At your request, a fraud alert with the Credit Reporting Agencies may also be established.

IDENTITY THEFT AID Definition:

Identity theft affects more than 12 million families and costs over $50 billion annually. Identity Theft Aid extends well beyond identity theft to address contemporary and emerging fraud issues. It provides high-touch, personalized resolution addressing every type of identity fraud, as well as services designed to preserve and protect personal and financial information. Eligibility:

To be eligible for Identity Theft Aid, you must be an eligible accountholder, in good standing, of a participating financial institution.

Overview:

As a member, you have unlimited access to personal fraud specialists who will provide guidance and identity recovery and restoration assistance.

Fraud Resolution Services

 Fraud Resolution is the core of a personal information management program. These services address known fraud and suspicions of fraud: identity theft, account takeover, and credit card fraud. Resolution includes financial, synthetic, criminal, and medical identity fraud. Correcting these issues without professional assistance can be arduous.

 You will speak immediately with a fraud specialist, often referred to as a personal advocate, who will work on your behalf throughout an exhaustive and efficient process. You will not be required to execute a Power of Attorney as your

advocate will work with you hand-in-hand. The resolution process concludes with a comprehensive Identity Wellness Checkup.

 Your specialist will prepare necessary correspondence and place file alerts and freezes, if you deem such action appropriate. As part of the resolution process, you will be given a variety of reports and monitoring products. These products are helpful to measure the progress of resolution and to serve as early warning indicators of subsequent fraud.

Social Media Services

 As the use of social media increases, so will the threat to an individual’s information. Younger users may be particularly vulnerable. Without proper controls in place, obtaining an individual’s personal information is simple. A fraud specialist will assist with notifications of privacy changes, virus alerts, policy changes, etc. on social media sites; provide information on what not to share on social media and what should only be shared with close friends; work to support needs related to protecting the information held in smart phones, including help with deactivating a lost cell phone; work with victims of stalking and cyber-bullying to place protection around them and, where the laws allow, to interact with law enforcement to stop the cyber-stalking and cyber-bullying.

Beneficiary and Surviving Spouse

 The Beneficiary and Surviving Spouse Program was created to ease the stress of securing one’s identity and financial information during periods of transition. This program addresses the challenges faced when considering assisted living or hospice care, as well as the complexities faced by the caregiver or spouse

when death occurs. This program is designed to assist the surviving spouse and/or estate executor. A report of the deceased’s credit file is reviewed for fraudulent activity and to schedule open credit accounts. Issues of fraud are resolved, including those impacting the surviving spouse. Once the credit file has been corrected, it is permanently suppressed to avoid subsequent fraud. Fraud specialists also search for imbedded life insurance benefits, often overlooked. These highly experienced specialists will assist with Notification of Death to appropriate creditors, and the procurement and completion of documents and forms necessary to settle the estate.

Document Replacement

 The experience of a catastrophic loss is devastating. Rebuilding a home and reclaiming an identity is an arduous process requiring a myriad of sensitive documents, including: driver’s licenses, Social Security cards, insurance policies, birth certificates, marriage licenses, divorce instruments, passports, work permits, deeds, tax records, immigration and naturalization papers, credit cards, account information, and bank records.  Replacement to assist replacing any lost or stolen documents — a catastrophic event is not required.

Travel Identity Assist

 The loss of a wallet or airline ticket is an unsettling dilemma when travelling domestically, but a stranded international traveler without a payment mechanism or appropriate travel documents may become overwhelmed. This service

provides help to address such predicaments and assist with your safe return home. A fraud specialist may initiate a variety of measures on your behalf. These services include coordinating communications with your family and business associates. Reactive measures include replacing sensitive documents, maintaining access to financial accounts, and facilitating your return home.

 You are encouraged to use this prior to your departure. A fraud specialist will provide travel precautions to consider during your trip and offer suggestions for securing information, both at home and while travelling. You will also be provided with emergency contact information for your personal fraud specialist should you experience a problem while travelling.

Transitions and Address Change

 Forty million Americans change their residence each year. Some move their permanent residence while others transition to college or enjoy seasonal or second homes. In every case, a move elevates the likelihood of identity-related crimes.

 Prior to a move, you may wish to contact your fraud specialist to help mitigate the forgoing risks. Your personal advocate will provide proactive measures and assist with change-of-address letters to facilitate mail forwarding from utility companies, magazine publishers, etc. If sensitive documents are misplaced during the move, your advocate will assist with their replacement.

Home and Auto Break-in Protection

 A home or auto break-in can cause significant vulnerability to fraud as a

you are a victim of theft, your fraud specialist will assist with a review of your credit file and explain various proactive measures to protect your identity. If fraud is discovered, you are entitled to comprehensive resolution of those issues. Marriage and Divorce

 If you are planning a wedding or contemplating divorce, you may wish to consider unwanted credit exposure or risk of identity fraud. Although marriage and divorce are emotional events, both can cause unsuspected financial entanglements and fraud.

 Prior to marriage, an advocate will guide you and your fiancée through a systematic Identity Checkup by reviewing respective credit files and other personal and financial information. If fraud has occurred, resolution will be initiated. The fraud specialist can also provide precautions to limit future

exposure. You may also seek guidance to facilitate bridal name change with the Social Security Administration, credit bureaus, financial accounts, lenders, medical providers, DMV, and IRS.

 During a time of separation or divorce, fraud specialist can help analyze your credit file to determine if fraud has been perpetrated. Your fraud specialist can assist in placing protective measures around your personal and financial information. Assistance can be provided to reinstate your family name with governmental organizations such as the Social Security Administration, DMV, and IRS.

Deployed Military Personnel

 Families of deployed military personnel are soft targets for identity thieves. These crimes may take longer for detection, especially if the soldier is deployed abroad. Deployed military personnel are entitled to unique, proactive measures to protect their identity.

 If you are in the military and deployed in active duty, a fraud specialist will place Active Duty Military Fraud Alerts on your credit files. The three alerts remain for twelve months unless you request otherwise. The alerts also remove your name from pre-approved credit card offers and other personalized marketing for two years.

 The service also provides personalized assistance for veterans obtaining documentation, completing forms and protecting personal information. Protection for Minors

 Theft of an infant’s identity and similar crimes perpetrated against children are variants of identity theft. Minors are particularly vulnerable because identity fraud against children can take years to detect. Fraudsters normally use a child’s personal information to apply for credit — creating a credit file with few restrictions. If your child has been victimized, a fraud specialist will work to remove fraudulent entries. Once the credit file has been purged of fraudulent entries, the file will be suppressed until your child reaches adulthood. Restoration reduces the likelihood of erroneous information impacting your children’s future ability to secure legitimate credit.
