Doctor of Philosophy (PhD) Programme in Statistics by Research

Applicants’ Checklist

1. Submit online application through https://inetapps.nus.edu.sg/GDA2/Home.aspx

and click on “Print Online Application Form” button on the summary page after you have submitted the online application. Please only submit that printed online application form for your application submission to us.

2. The following documents and supporting documents (as indicated at No. 3) need to

be submitted to the department for the processing of the application by the deadline, 15 November 2015 :

a) A hardcopy of the application form (printed from online application system)

b) A recent passport-size photograph

c) The following non-refundable application fee is payable for each programme you apply for:

Mode of Application Application Fee

Paper-based application S$40 per application (inclusive of prevailing GST)

Online application S$20 per application (inclusive of prevailing GST)

Please note that an application without the application fee payment or with insufficient application fee amount will not be processed.

Local Students:

a. Online Payment by any of the methods (For application submitted for Semester 1 AY2012/2013 onwards)

 Credit Card (AMEX, VISA or MasterCard)

 Debit Card (applicants must have an online banking account with either of the following banks - POSB/DBS, UOB or Citibank).

OR

b. Complete the relevant Application Fee Form and send it together with a cheque (should be crossed) made payable to "National University of Singapore". Please write your name, mailing address and programme applied for at the reverse side of the cheque. Separate cheques are to be written for each programme applied.

OR

c. Complete the relevant Application Fee Form and make payment by NETS or cash at the Student Service Centre (located at Level 1, Yusof Ishak House) during operating hours. Please attach your payment receipt to the application package to show

evidence that you have made your application fee payment. International Students:

a. Online Payment by any of the methods (For application submitted for Semester 1 AY2012/2013 onwards)

i. Credit Card (AMEX, VISA or MasterCard)

ii. Debit Card (applicants must have a online banking account with either of the following banks - POSB/DBS, UOB or Citibank).

b. Complete the relevant Application Fee Form and send it together with a bank draft (drawn on a bank in Singapore in Singapore Dollar) made payable to "National University of Singapore". Please write your name, mailing address and programme(s) applied for at the reverse side of the bank draft.

3. Supporting documents to be submitted in hard copy are:

a) An official/copy of Bachelor Degree Transcript (with English translation if applicable)*

b) A certified copy of Bachelor Degree Certificate/Scroll (with English translation if applicable)*

c) An official/ copy of Masters Degree Transcript (if applicable) (with English translation if applicable)*

d) A certified copy of Masters Degree Certificate/Scroll (if applicable) (with English translation if applicable)*

Note: * These documents should be sent to us in SEALED envelopes with its flap bearing the security seal of the university and signature of the Registrar or his representative.

e) Two Referee Reports (either online submission or hardcopy)

For hardcopy application, these reports should be sent to us in sealed envelopes whose openings bear the referee’s signature across it.

If you are submitting online application, the referee reports will directly be sending to your referees by e-mail to complete the online referee reports.

f) Official TOEFL/IELTS and GRE score sheets for applicants whose native tongue

or medium of undergraduate instruction is not English.

(Note: Please submit copies of TOEFL/GRE/IELTS score sheets together with other supporting documents and please request the testing organization to send your score sheet(s) directly to NUS for verification).

Our institution/department code is 9087/59 for TOEFL and 0677/0705 for GRE.

The minimum score ranges requirement for TOEFL score is: Paper-Based Test: 580-600,

Computer-Based Test: 237-250, Internet-Based Test: 85

The minimum score requirement for IELTS is 6 and above.

Please note that the TOEFL/IELTS scores are valid for 2 years from the test date. If it has been more than two years since you last took the test, you must take it again to have the scores reported.

Please note that the GRE scores are valid for 5 years from the test date.

g) Professional Certificates, if any;

h) A copy of citizenship certificate/national identification card (Pink) for Singapore

Citizens OR

A copy of identification card (Blue)/Re-Entry Permit for Singapore Permanent

Residents (PRs) OR

A copy of passport page/Identification Card showing nationality and personal details/ Work Pass (For example: Employment Pass, S-Pass) (if applicable) for

International Applicants

j) Documentary evidence of financial support (e.g. bank statement, or letter of confirmation from a sponsor, or documentary evidence of scholarship or other awards obtained or salary statement for last three months) (Only applicable for

the self-finance applicants and those who want to consider their application if their scholarship application is unsuccessful).

4. For the applicants who are not submitting original supporting documents need to bring along the original documents to our department for verification by 15 November 2015.

(Only applicable to those applicants who are residing in Singapore)

5. Any omission of information requested in the application form or supporting documents will cause delay and may render the application void.

6. Applicants must produce originals of all documents when requested if admitted to the programme
