Lane County Health & Human Services Strategic Plan Development

REQUEST FOR QUOTES

Introduction

Lane County Health & Human Services is interested in contracting with a consultant to help develop an updated strategic plan. The department provides a wide range of health and human services to people in Lane County through a combination of direct and contracted services. This project will include working with the department leadership to design the planning process, as well as directly facilitating some planning efforts, and producing final materials that describe the 3-5 year strategic plan.

The contract from this solicitation will be written for the period September 1, 2015 through June 30, 2016 with the ability to renew the contract through June 30, 2018 based on contractor performance, available funding and Lane County need. The anticipated annual funding for the project is not to exceed $30,000 per year. Actual costs will be dependent upon Lane County’s use of the services.

Submission Requirements/Information

1. Applicants must provide information requested in the section titled “Required Information”.

2. Quotes must include all documentation to be considered.

3. Lane County considers this a quote process under LM21.118. Lane County reserves the right to add interviews to this process or to make a decision based on the information received. Additional phases of this project may be awarded to the successful recipient of this award without further competitive process.

4. All applicants will be notified, in writing, of the decision within three working days of the decision.

Protests of Award

All written protests of award must be filed within seven (7) days of the notice of intent to award. Protests of award shall be considered by the Board of it would approve and award the contract. Protests of award shall be handled by the County Administrator, or designee, for those contracts to be awarded by the County Administrator.

(a) The protest for a non-public improvement contract shall specify the applicable grounds for the protest as set forth in ORS 279B.410(1). The protest for a public improvement contract shall specify the applicable grounds for protest set forth in OAR 137-049- 0450(4)(c) which is adopted.

(b) For contracts to be awarded by the Board, if the public officer determines there is sufficient merit to reject quotes under LM 21.105(10), he or she may do so. If any portion of the protest remains, the Board shall be provided and may consider a complete copy of the written record, and any other evidence provided at a public meeting and shall issue its decision by Board Order. The Board may affirm, reverse, send back to the Department or revise the award.

Contract Requirements

A. The contractor must operate the program independently and not as an agent of Lane County.

B. The contractor must comply with all applicable federal, state and local statutes and rules governing the operations of the program.

C. The contractor must be able to provide insurance as required in Attachment 1.

Required Information

1. Describe your organization’s experience assisting organizations with strategic planning efforts, particularly with public sector and/or health related organizations. 2. Indicate an estimated cost of the project based on your organization’s approach. Include the hourly rates and anticipated hours for the various types of staff who you anticipate assigning to this project. Also include your organization’s administrative costs. This estimate is non-binding and may be revised based on information obtained during the interview and/or contract negotiations.

3. Please provide a list of at least three clients who can serve as references.

INSURANCE COVERAGES REQUIRED

Contractor shall not commence any work until Contractor obtains, at Contractor's own expense, all required insurance as specified below. Such insurance must have the approval of Lane County as to limits, form and amount. The types of insurance Contractor is required to obtain or maintain for the full period of the contract will be:

X Commercial General Liability Insurance shall include Damage to Rented Property ($50,000), Medical Expenses ($5,000), Personal & Advertising (same per occurrence) and Products/Completed Operations (same per occurrence.)

X Additional Insured Clause The general and auto liability insurance coverage's required for performance of this contract shall be endorsed to name Lane County and its divisions, its commissioners, officers, agents and employees as additional insureds on any insurance policies required herein with respect to Provider's activities being performed under the Contract. The additional insureds must be named as an additional insured by endorsement, and the policy must be endorsed to show cancellation notices to the Lane County department who originated the contract. Coverage shall be primary and non-contributory with any other insurance and self-insurance.

By execution of this Form, the undersigned Proposer acknowledges that its entire quote is subject to Oregon Public Records Law (ORS 192.410–192.505), and may be disclosed in its entirety to any person or organization making a records request, except for such information as may be exempt from disclosure under the law. Proposer agrees that all information included in this quote that is claimed to be exempt from disclosure has been clearly identified either in the Proposer's Statement, or in an itemization attached hereto. Proposer further acknowledges its responsibility to defend and indemnify the County for any costs associated with establishing a claimed exemption.
