Network Providers

This directory provides a list of your plan’s network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage (EOC).

Cigna is committed to building a trusted network of health care providers. The doctors, hospitals, facilities, and other providers we contract with must meet certain credentialing requirements. They must also agree to our billing rates. This helps make sure you have access to quality, cost-effective care. To build our network, we look at how many primary care and specialty doctors are in a specific area. We also look at what hospitals, labs, and other facilities are in that area. Our goal is to build a network that gives you choice and convenient access to treatment and services. Provider networks vary by state and plan, and include doctors, hospitals, and health care facilities in your local area.

For Cigna Medicare Advantage PPO plan customers, we encourage you to select a Primary Care Provider (PCP) from our network, but you’re not required to do so. Generally a PCP is a physician who is trained to provide general medical care. Your PCP can also coordinate the rest of the covered services you need. You have the option to select doctors and benefits both in and out of network; no referrals required.

The network providers listed in this directory have agreed to provide you with your health care, vision and/or dental services, as applicable. Please note: not all Cigna Medicare Advantage plans have vision and dental benefits. Check your Evidence of

Coverage to see if your plan has a vision and/or dental benefit or allowance. For plans with preventive dental care, you may go to any General Dentist listed in this directory; however, services provided by Dental Specialists will not be covered. For plans with comprehensive dental care, you may go to any of our network dental providers listed in this directory. In some cases, you may get covered services from non-network dental providers. Customers in plans with a dental allowance can use any licensed dental provider who is eligible under Medicare.

For health care, you may go to any of our network providers listed in this directory. In some cases, you may get covered services from non-network providers.

Out-of-Network Providers

While you are a member of our plan, you may use providers outside of the network. Your out-of-pocket costs may be higher if you use out-of-network providers. In non-emergent situations, please tell out-of-network providers that you are a Cigna Medicare Advantage customer before you see them to ensure that they will see Cigna Medicare Advantage customers. Our payment for out-of-network services is based on the Medicare Allowable Amount. If the out-of-network provider charges more than the

Medicare Allowable Amount, you may be liable for the difference between the Medicare Allowable Amount and the provider’s total charge. Please refer to your Evidence of Coverage for details.

Out-of-network/non-contracted providers are under no obligation to treat our customers, except in emergency situations. For a decision about whether we will cover an out-of-network service, we encourage you or your provider to ask us for a pre-service organization determination before you receive the service. Please call our customer service number or see your Evidence of Coverage for more information, including the cost sharing that applies to out-of-network services.

Relationships within our Provider Network

Cigna Medicare Advantage has different types of Primary Care Provider relationships. We may have contracts with independent physicians, Independent Physician Associations, Physician Organized Delivery system and clinic-based providers in your area.

The following paragraphs describe the relationship these providers have with us.

specialists and hospitals affiliated with the same IPA and with whom they work closely to coordinate your care. You will receive all of your care, including gynecological and specialty care from the physicians within the same IPA.

Gynecological care can be accessed by any Gynecologist in the network.

Your IPA is designed to provide a team of primary care and specialty physicians who can manage your care needs.

However, if your medical condition should ever require you to seek care outside the IPA, your primary care physician will continue to assist you in the coordination of any needed prior authorizations or referrals for services outside the IPA.

• A Physician Organized Delivery (POD) system represents a network of physicians with similar referral patterns,

organized to administer health care services to the plan’s customers. If you select a Primary Care Physician who belongs to a POD, you should be aware that your PCP will refer you to specialists affiliated with the same POD and with whom they work closely to coordinate your care. However, your primary care physician can refer you to any other in-network physician outside of the POD, and your primary care physician will continue to assist you in the coordination of any needed services outside the POD.

• Independent physicians

Independent physicians are Primary Care and Specialist physicians located in and around your area that are not part of an Independent Physician Association (IPA) or Physician Organized Delivery system (POD). If you select an independent Primary Care Physician you should be aware that your PCP will refer you to specialists and hospitals with whom he/she is affiliated and with whom they work closely to coordinate your care. Please confirm whether the referred physician or hospital is a network provider with your plan. You will receive all of your care, including gynecological and specialty care from the physicians affiliated with your PCP. Gynecological care can be accessed through any independent Gynecologist.

If you should receive a full-cost bill from a provider

There may also be times when you get a bill from a provider for the full cost of medical care you have received. In many cases, you should contact Customer Service or send this bill to us instead of paying it. We will look at the bill and decide whether the services should be covered. If we decide they should be covered, we will pay the provider directly. (Please note that we cannot pay a provider who has opted out of the Medicare program. Check with your provider before receiving services to confirm that they have not opted out of Medicare.) If we determine that the services are not covered or were not medically necessary, we may deny coverage and you will be responsible for the entire cost.

Emergency care/Urgent care

You may get emergency medical care whenever you need it, anywhere in the United States or its territories. Our plan covers ambulance services in situations where getting to the emergency room in any other way could endanger your health. Please refer to your Evidence of Coverage for details. You may also get covered emergency medical care whenever you need it, anywhere in the world. If you have an emergency, we will talk with the doctors who are giving you emergency care to help manage and follow up on your care. The doctors who are giving you emergency care will decide when your condition is stable and the medical emergency is over. After the emergency is over you are entitled to follow-up care to be sure your condition continues to be stable.

Your follow-up care will be covered by our plan. If your emergency care is provided by out-of-network providers, we will try to arrange for network providers to take over your care as soon as your medical condition and the circumstances allow.

If you obtain routine care from out-of-network providers, you will have to pay the out-of-network cost-sharing except for emergency care and out-of-network urgent care. Please refer to your Evidence of Coverage for more information.

Behavioral Health Services

Behavioral health services, including mental health and substance abuse services, for beneficiaries are covered by your plan.

Customers and providers may access behavioral health services by calling 1-866-780-8546 (TTY 711) for assistance in finding a behavioral health care professional or to arrange for inpatient admission authorization. Behavioral health services are available 24 hours per day, 7 days a week.

How do you find plan providers that serve your area?

Cigna Medicare Advantage PPO plan customers are encouraged to select a PCP from our network, but you’re not required to do so. Generally a PCP is a physician who is trained to provide general medical care.

To choose a PCP from the Provider Directory, turn to the Primary Care Provider section for a listing of primary care providers in your area. You can also call Customer Service at the number listed below for assistance. When you select a PCP, you will find a space on your Enrollment Application to enter the PCP’s name. If you choose a PCP after your Enrollment Application has been submitted, call Customer Service and inform them of your PCP selection. If you want to change to another PCP, you may do so at any time by calling Customer Service at the number listed below. The name and office telephone number of your PCP are printed on your ID card.

Cigna tiene el compromiso de construir una red confiable de proveedores de cuidado de la salud. Los médicos, hospitales, centros y otros proveedores que contratamos deben cumplir determinados requisitos de acreditación. También deben aceptar nuestras tarifas de facturación. Esto ayuda a que nos aseguremos de que usted tenga acceso a servicios de calidad y con una buena relación costo-beneficio para el cuidado de su salud. Para construir nuestra red, nos fijamos cuántos médicos de cuidado primario y especialistas hay en un área en particular. • Asociaciones de Médicos Independientes (IPA)

Las Asociaciones de Médicos Independientes (IPA, por sus siglas en inglés) comprenden médicos de cuidado primario y especialistas ubicados en su área y en los alrededores. Si usted elige un médico de cuidado primario que pertenece a una IPA, debe saber que su PCP le referirá a especialistas y hospitales afiliados a la misma IPA con quienes trabaja en estrecha colaboración para coordinar su cuidado. Recibirá toda su atención, incluida la atención ginecológica y

especializada, de médicos pertenecientes a la misma IPA. Se puede acceder a atención ginecológica a través de cualquier Ginecólogo de la red.

Su IPA está diseñada para brindarle un equipo de médicos de cuidado primario y especialistas que puedan manejar sus necesidades para el cuidado de su salud. Sin embargo, si su condición médica alguna vez exigiera solicitar atención fuera de la IPA, su médico de cuidado primario seguirá brindándole asistencia para la coordinación de las autorizaciones previas o referencias necesarias para los servicios fuera de la IPA.

Si usted elige un médico de cuidado primario que pertenece a una POD, debe saber que su PCP le referirá a especialistas afiliados a la misma POD con quienes trabaja en estrecha colaboración para coordinar su cuidado. • Médicos independientes

Confirme si el médico u hospital referido es un proveedor de la red de su plan. Recibirá toda su atención, incluida la atención ginecológica y especializada, de médicos afiliados a su PCP. Puede acceder a atención ginecológica a través de cualquier Ginecólogo independiente.

Si recibe una factura por el costo total de un proveedor

También puede haber ocasiones en que reciba una factura de un proveedor por el costo total de la atención médica que recibió.

En muchos casos, debe comunicarse con Servicio al Cliente o enviarnos esta factura en lugar de pagarla. Nosotros analizaremos la factura y decidiremos si los servicios se deben cubrir. Si decidimos que se deben cubrir, le pagaremos directamente al

proveedor. (Tenga en cuenta que no podemos pagarle a un proveedor que ha optado por quedar excluido del programa Medicare. Consulte a su proveedor antes de recibir servicios para confirmar que no haya optado por quedar excluido de Medicare). Si determinamos que los servicios no están cubiertos o que no eran médicamente necesarios, es posible que deneguemos la cobertura, y usted será responsable del costo total.
