2. Submission of CD-ROM or DVD containing geographic data in accordance with Standards for Digital Plan Submittals to Municipalities [provision added 02/24/2021]

1. Prepared by a registered land surveyor and has the professional seal and signature.

3. Identification of plan by name & signature of owner of record & the location of land in question, including Deed, Book/Page, Assessor Map & lot number, area &

frontage in feet, the scale, North point & date.

Title of plan shall be revised to read “363 & 373 Highland Street…”

not 363-373 4. The statement “Approval Under Subdivision Control

Law Not Required” and sufficient space in the lower right-hand corner for the date and signatures of all five members of the Planning Board.

5. The statement “Endorsement of this plan does not imply the plan complies with the Zoning Bylaws of the Town of Northbridge.”

Notation on plan shall be revised accordingly (see also comment #7) 6. The Zoning Classification and any zoning district

boundaries that may lie within the locus of the plan.

Residential-Two (R2) Zoning District 7. In the case of the creation of a new lot, the remaining

land area and frontage of the land in ownership of the applicant shall be shown.

Note: proposed ANR/conveyance may create zoning non-conformity issues; potential violation of min. frontage requirement for “corner lot” -see Lot Line, Front definition (§173-2 Northbridge Zoning Bylaw)

8. Notice of any decisions by ZBA including but not limited to variances and special permits.

If necessary, written verification by the Town clerk that there is sufficient frontage on an established public way, or a way maintained and used as a public way.

10. Names of abutters from the most recent tax list. If applicant has knowledge of any changes subsequent to the latest available Assessor’s records, this information shall be indicated on the plan.

11. The bearings and distance of all lines of the lot or lots and the distance to the nearest permanent monument.

12. The location of existing buildings on the land or within 50 feet of its boundaries, including setback and side and rear yard designations.

13. Existing contours at five (5) foot intervals and the location of any topographic features, which interfere with the use of the frontage for access.

Notation shall be added to plan noting proposed frontage (Lot 3) is not impaired by steep slope ore excessive grades

14. The location of all bounds, fences, walls and bodies of water, including but not limited to streams, brooks, watercourses, ponds, lakes, floodplains and other standing bodies of water. The plan shall reflect the average annual high-water level with respect to any such bodies of water.

15. A locus map at a scale of one inch to 1,000 feet.

as a building lot, the statement “Not a building lot; no further building may occur without further approval by the Planning Board pursuant to the Subdivision Control Law.”

18. Justification/description of basis for claim to ANR endorsement, clearly noted both on the application form and on the plan (i.e., required frontage on a town accepted road, separation of lots with buildings which preexist subdivision control – including evidence – etc.)

Justification/description shall be (amended) added to plan, clearly noting intent: (e.g.) “Purpose of plan is to convey Parcel(s) A, ….. to create Lot(s) …., having XX frontage along…..”
