The questions are simple and easy to follow. If you need assistance just ask your Assurant® representative for guidance in completing your Final Wishes Planning Guide.
The result will be a clear record of your final wishes, a source of important documents for legal and public records, and a permanent keepsake of your fondest memories to speak to future generations.
Please store the completed book in a safe but public place in your home. Everyone in your family should know exactly where to find it quickly and easily without a key or the distress of a search.

I wanted to spare you as much anxiety, grief and expense as possible at the time of my death, so in this booklet I’ve suggested some arrangements in advance. I hope they relieve some pressure at this difficult time.
This booklet is divided in two. The first section includes vital statistics, funeral service guidelines and cemetery requests, all important to the funeral director while planning the details of my service.
The second section is more personal and provides material for eulogies, obituaries and other remembrances.
If I needed more room I used the additional information pages at the end of the book.

We all accumulate important papers. To the survivors. A few thoughts on how to deal with bereavement.
Death is the most natural thing we do, but modern culture has made it hard to grieve naturally. We often tend to bottle our sadness inside. This only makes the process more difficult and less complete.
It’s important to share your feelings of loss with others. A sensitive funeral ceremony is an important first step, but the weeks and months following are crucial, too. We urge you to seek out family, friends, clergy or a professional therapist, even if you feel you’re coping well. This can be particularly important for children.
The pain of loss is a natural part of death, and it has a purpose.
Properly acknowledged and experienced, your hurt will eventually subside and leave in its wake the best and most tender memories of your loved one.
