17 I currently ha e no in-person visitation with my 24-months old son, since we started visits at 18 Terra Firma on Sept mber 3,2020, (after 6 months of no in-person visitation and only two Zoom 19 sessions a month) an after Thygesen' unilateral schedule change the child cries inconsolability to 20 the point he is taken way with in 15 minutes, and he is given back to the father. Now, I travel from' 21 Utah to California fo these visits it's absolutely an emergency that visits are canceled every time I 22 arrive from out of s te, I request immediate permission to use any other comparable visitation

23 facility (i.e.; Parent C oice or private supervisor) immediately, and then schedule for mediation and 24 subsequently trial for the other requests I have below. This is my first formal request for order, we

25 are still exercising 0 of the March 6, 2019 ex parte custody orders, no full hearing on the merits of

26 the current custody a d visitation has ever been held.

27 1. I have been going to Supervised Visitation for close to two years. There has been 43-recorded

2. I participated in 1 hr. weekly Rally visits at first. I have also attended bi-monthly; in-person

visits with the child at Rally, each time driving thousands of miles to California from Utah, where I have been living with my parents. Since 2019, I have cumulatively driven close to a hundred thousand miles to attend my weekly then bi-monthly visits with the child. The 312 pages of Supervised Visitation Case Reports have showed the interactions between myself and maternal grandparents with the child are successful, appropriate, and loving, during my visits, playing, and laughing with child and otherwise remaining actively engaged.

3. “In examining the “Best Interests” factor, I will direct the court to the 312 pages and 43

Supervised Visits, and to my continuous consistency in visitation, having attended 100 percent of

scheduled visits. I show up, each and every time, that alone shows tremendous efforts myself and

my family has taken to build a relationship with a child who was so heartbreakingly taken away

from us. The Case Reports from Rally and ACAFS, show the child was thriving and bonding with

his mother; his grandmother, and grandfather, who raised him with me in their home in Utah. 4. On March 5, 2020, Rally was closed due to COVID-19. Thygesen refused to comply with using another facility and in-person visits stopped for 6 months. Zoom visits were substituted instead at Utah Family Academy (ACAFS).

5. On July 15, 2020, I filed an Order to Show Cause for in-person visitation; we started Supervised Visitations at Terra Firma in Hayward, California on September 3, 2020.

6. On September 15, 2020, Thygesen unilaterally changed our visitation schedule. I found out

when I called in to confirm my regular Thursday visits, and that’s when the Supervisor told me that Thygesen changed all future visits. Since then, Kayson has become grief-stricken; I can hear him screaming while he is still with his father, slowly approaching Terra Firma for visits.

7. Since Thygesen’s unilateral schedule change, there has been a string of unsuccessful visits @ Terra Firma, where I drive 32 hours and 2,000 from Utah to California for visits, just to have my visits terminated within 10-15 minutes, due to the child being inconsolable at Terra Firma.

8. Kayson’s birthday was November 26, 2018; he will be two years old, I have not been able to see him in person since October 28, 2020.

solution to the problem, Thygesen has refused using another facility, he has refused a schedule change even though he unilaterally changed our original, Rally schedule from Thursdays to Wednesdays without notice, he refused to go through mediation, instead communicating in private with Cecelia Wang telling her there is no need for mediation, and falsely stating we reached an agreement. I respectfully request the Court grant my requests for unmonitored visitation, monitored exchanges, and/or ability to use any supervisor with our 24 months old son, Kayson Wang.

10. It’s been two years of supervised visitations; it’s time for a transition. It’s in the best

interests of Kayson Wang to have more parent-time with his mother, that he is able to identify with his culture. Supervised visitations are not in his best interests, visits at Terra Firma are not in his best interests, and if supervised visitations are continued then I respectfully request that Christoffer brings the child to Utah. At Utah ACAFS there is an outdoor area, which would be highly appropriate for a child his age.

Relief Requested:

1. Unsupervised; and/or Monitored Exchanges, or Supervised Visits with by Maternal Grandparents;

2. I’m requesting 35% time-share, or at minimum visits for 8 hours on alternate weekends. 3. Bi-weekly Video Conference Calls either in my home or through ACAFS for at least 30

minutes twice a week or once a week for 1- Hour. (* Note: we’ve had 6+ months of successful alternate weekday 3-Hour Zoom Visits at ACAFS twice a month, w/ no in-person visitation at all)

4. To have Thygesen bring the child for visits at Utah Family Academy (ACAFS) part of the time and compensate me for travel expenses to see the child.

5. A holiday, schedule request is also attached with this RFO, and I also request an order addressing "Makeup" Parent Time. And two weeks of vacation for the child to take place in Utah.

6. Holidays: Mothers day, Thanksgiving, and two weeks of parent-time in Utah.

7. If Supervised Visitation is continued after two years w/ no review date, I request that private supervisor and ability to use any supervising facility immediately as a interim

order where the child can have visits outdoors or other child friendly locations instead of

confined to a room. It’s an extreme hassle to order a out of state Pro Per litigant to have to file and wait 6 months, and argue against 3-4 lawyers just to change a Supervised Visitation facility.

8. Per Utah Judge Low’s Court Order after hearing, I request the Court to honor that order under the UIDDA, and allow me to continue receiving Kayson Wang’s medical reports.

The last visitation modification was on September 12, 2019, the Sua Sponte Ordered

visitation change from 1 hour once a week at Rally to twice a month for up to 3 hours. A change from Rally to Terra Firma was implemented with a no change in parent-time on August 27, 2020. I was instructed by the court to file an FL-300 for additional parent-time requests. [See filed on 7/15/20 a signed Notice of OSC, paragraph 7]

Exercising monitored visitation proved problematic. Rally could not accommodate monitored visits for more than two hours per day so were therefore not able to accommodate the three hours as provided for in the Judgment. (See 9/12/19 filed Modified Rally Visitation Order)

Since Rally closed on March 5, 2020, another monitoring services agency, Terra Firma, was used. The visits at Terra Firma proceeded for a short period of time until Thygesen unilaterally changed the visitation schedule, the child does not do well at Terra Firma. According to Shelley La

Botte Supervisor and Coordinator for Center for Families, Children & the Courts of the Judicial

Council of California, she would not recommend Terra Firma. (See emails attached dated June 4, 2020)

I’m not exactly sure how sole supervisor can supervise multiple visits with one person monitoring multiple different people in multiple rooms at once. When Christoffer brings the child for visits, he is unable to console Kayson to stop crying, he waits with the child at the entrance of Terra Firma, which cuts at least 10 to 15 minutes of visitation time, which have recently ended in 15 minutes due to the child being inconsolable at that location.

Change of Circumstances:

While for a change in visitation not amounting to a change in custody does not require a

showing of change circumstances, nevertheless there has been sufficient change. 1

4. Since 6 months of no in-person visitation, the child is no longer doing well in a closed room for even 15 minutes, the frequency of the visits need to increase. I’m proposing alternate weekends for 8 hours back to back with Zoom visits twice every week. This is urgent; I’ve seen my child for only 6 hours in person all year! He is 24 months old I’ve missed out of almost his entire life, due to the extreme parental alienation by Thygesen.

For the period of time covered by this case report, the supervised visits have not occurred at Rally due to the “Shelter in Place” order. Rally offered the custodial father the virtual

visits, but he declined (see “Additional Correspondance”). The complete observation notes,

Christoffer Stanford Thygesen had me arrested on October 17, 2019 before our October 18, Utah hearing, there were 10+ SFPD officers at the Rally Front Door Lobby.

I always go through the emergency entrance as part of Rally Policy, but Mr. Thygesen's attorney, ask me to meet him there @ 4:15 p.m. to pick up Trial Exhibits as I am Pro Per. unfortunately, I received no exhibits but was instead met by 10+ SFPD Officers.

I was with my parents who were forcibly alienated from Kayson for over 7-Months, whom they helped raise from birth. I believe my mom went upstairs to inform Rally. I, unfortunately, had no knowledge that was going to happen, we were shuttered in fear, and the Thygesen family had my bail set @ $750,000, we had to put our house on the lien. Under those circumstances, is there a way to put a description next to that No-shows?

I'm in dire circumstances as Mr. Thygesen will also not allow Skype or any sort of video conferencing, in these terrifying times, I may not see my infant son possibly the rest of the year of this Pandemic continues.

Hello Ms. Wang,

Hope you and your family are well. At the moment, Rally is still closed until the “Shelter in Place” order is lifted. This applies to all Rally sites. We will let you know once we have an update.

On April 15, 2020 father sent the following e-mail to Rally:

Dear Ms. Osorno,

I received a call from a Rally staff member today informing me that Rally is starting to offering video visitation. While I’m glad to hear that Rally is providing this service, it is unnecessary in my case. Though my attorney, I have worked out an informal and

temporary solution to get Ms. Wang video visitation. I found a professional visitation supervisor in Utah who is able to physically supervise Ms. Wang during her video visitation. Ms. Wang and I are, as I understand it, going to do our intake with the Utah group tomorrow. This professional supervisor will be conducting a supervised video visitation of Kayson at our home here in California from their facility in Utah. This Utah supervisor is able to enforce Rally’s no photos/video policy that applies in this case. I am voluntarily hiring this Utah supervisor while Rally has suspended its normal operations. Please let me know when you expect to resume normal operations that can provide for in-person supervision.

The period of Supervised visits scheduled from: October 17, 2019 to March 5, 2020. Supervised visits were scheduled: Every other Thursday for two (2) hours.

This report is based upon observations of exchanges between parents and children occurring in a protected, fair, and structured setting. It is a compilation of objective, fact-based observational field notes prepared by professionals, paraprofessionals, and/or volunteers at varying levels of training. Person, identifying information may be omitted from this report if the safety of program participants is considered at risk.

Rally Family Visitation Services does not provide evaluations of the family members nor make recommendations about future parent-child custody/visitation arrangements. No conclusions or

October 17, 2019

The supervised visit didn’t occur as scheduled since the mother didn’t show up.

Mother’s mother came to inform Rally that mother was arrested; therefore, the visit for today was cancelled. Grandmother asked if she could see Kayson indicating she misses him very much, but staff told her that was not possible as the mother needed to be present too. Staff then called father and let him know that the visit was canceled, and he said, “Fantastic; I’ll turn around right now.”

October 31, 2019

Mother arrived at the scheduled time with both her parents. Father arrived at the

designated time and handed Kayson to staff and said, “We can talk after the visit.” Kayson cried and staff took him to mother and grandparents. Grandmother took Kayson from staff.

Grandmother held Kayson, but he still cried. She then gave him to his mother. Mother set him down and showed him a toy duck and sat with him. Kayson stopped crying. Grandmother sat on the floor too. They showed him other toys. Grandfather sat in a chair away from the area where Kayson was sitting. Grandfather asked Kayson if he remembered Grandmother giving him a bath every night. Grandmother said something in Chinese, and staff reminded them that

they needed to speak in English.

After, grandmother gave Kayson a stuffed animal. Grandfather went to a counter and got a truck. He smiled and handed it to Kayson. Grandfather made comments about how cute

Kayson is now. Mother then read him an electronic book. Grandfather made a comment that Kayson was shy, and Grandmother said that it was very normal behavior.

Grandmother said she wondered if he could walk. She then picked him up (apparently to see if he could walk) and he made a face as if he was going to cry. Mother made a comment that he may walk later. Grandmother put him back where the toys were, and he started playing with them quietly.

Mother noticed he had four (4) teeth as opposed to three teeth he had last time. Mother made a comment about a possible mosquito bite. Grandmother went to turn on some music. Grandfather handed Grandmother her phone. Mother asked her to put on children’s songs and Grandmother did but left the phone near where Grandfather had moved to sit by the window. Mother made a comment about how “Chubby wubby” Kayson was.

Next, Grandmother and mother sat with him on the blanket with Kayson and offered him toys. Grandmother asked him if he wanted to stand, but he sat and then crawled to the pacifier mother had out on the blanket for him. He then went towards a book, and Grandmother handed it to him (it was by Grandmother’s foot). Grandfather made a comment that Kayson was quiet and Grandmother added that Kayson used to “Talk all the time.”

Kayson looked at Grandfather and he said, “hello” and spoke to him. Kayson played with more toys and then Grandmother gave him a spinach and vegetable wonton. Mother told staff that the new court order said that feeding and pictures were to be “Addressed by Rally.” Staff asked mother to send that to Rally for a review.

Kayson smiled and Grandmother made a comment about it and smiled also. Mother asked Kayson about smiling, and he gave mother a large smile. Mother cheered and Grandfather clapped his hands. Kayson turned around and crawled towards a wall. Grandmother was there and offered him some cut grapes and he ate one.

Kayson then went and pulled up on a table. Grandfather told Grandmother to get behind Kayson and Grandmother moved behind Kayson. Mother put on “Baby Shark” song and

Grandfather came and helped Kayson saying they could dance. Grandfather also held him a few moments and mother and Grandmother stood beside them. Kayson then leaned to Grandmother, who took him and held him. She walked around the room then put him down. Grandmother then took his hands and held him while he walked a few steps.

Kayson went to a table where the phone was lying. Grandmother put the grapes there and fed him as he played with a ball on the table and listened to the songs playing. He smiled as he sat with Grandmother and listened to the music and eating the grapes. He started leaning side to side and slapping the table. Grandmother and mother both slapped the table when Kayson did. Mother got him a stuffed animal and then got out some squash father sent. He spit that out. Mother then got his bottle and Grandmother held Kayson and feed it to him.

Grandfather went and watched as Kayson drank. Kayson drank all but one third (1/3) of the bottle. Grandmother then fed Kayson won-tons and then a cooked carrot. Grandmother next suggested feeding him the cereal father sent but then picked him up and walked around the room. Grandmother then sat in a chair and held him. She gave him the rest of the bottle.

Grandfather then held him a moment and mother sat beside them. Kayson leaned towards mother and made a whining sound. Mother took him and put him in front of a toy and he stopped whining. Grandmother then sat with him by that toy. He crawled away and went to the bag of cereal. Grandmother took the bag and took out a honeycomb looking cake. He put it in his mouth and crawled to the blanket. Grandmother sat there and he spit out half of the honeycomb.

Grandmother gave him the pacifier.

He then went back to a table in the corner and pulled up on it. He played with some toys there, and then mother decided to give Kayson a diaper change. Mother held a toy in front of Kayson while grandmother changed his diaper. After that, he got a toothbrush shaped like a banana and put it in his mouth. He crawled around the room near Grandfather. Grandfather lifted Kayson to his lap. He stayed a few moments and then tried to get out of Grandfather’s lap.

Grandmother also picked him up and took him to the bookcase and mother picked out the book “Goodnight Gorilla.” Grandmother sat in the rocking chair and mother sat in a chair beside them and read the book. He looked at the book periodically but then twisted on Grandmother’s lap and looked at Grandfather, who stood behind Grandmother’s chair.

Grandfather took him and smiled. Kayson made some sounds and smiled. Mother then stood and finished reading the book as Grandfather held Kayson. Grandmother wiped Kayson’s face periodically (he still had the banana toothbrush). Grandfather walked around the room until mother came to stand near them, and Kayson leaned to mother. Mother took him and sat on the blanket.

Grandmother joined them and they watched as he explored different toys. He crawled to a rocking horse, and Grandmother and mother put him on it and rocked it. Mother got the

electronic duck toy and showed it to him as he sat on the horse. He then tried to get off the horse and Grandmother helped him. Mother put the duck on the blanket in front of him.

Staff announced the end of the visit. Mother held him and grandmother kissed his cheeks. All three told Kayson they loved him. Mother handed staff the backpack and staff took Kayson. Staff escorted Kayson with his backpack to father.

Father gave staff payment and asked if the last visit mother cancelled was going to be paid by mother. Staff told him that if it was a late cancellation by mother, then he would not be charged for that and staff was only putting today’s normal visit amount on the card. Father signed the receipt and then left with Kayson.

November 14, 2019

Mother arrived as scheduled with both her parents. Father arrived at the designated time and handed Kayson to staff. Father told staff that Kayson had a stomach bug and he was red on his bottom from this. He added that Kayson had a sippie cup already full of formula/milk. Kayson cried, and staff took him to mother and grandparents. Mother took Kayson from staff.

Mother sat with Kayson on the blanket she had set out. A phone was on a nearby table playing children’s music. She had the toy electronic duck as per the usual routine for the visit, and showed it to him. Grandparents sat with him on the blanket also. Kayson stopped crying and looked at the toy.

Grandfather told Kayson,”Happy Birthday” and then mother and grandmother told Kayson the same. Mother found a pacifier and gave it to Kayson who only used it a moment before taking it out of his mouth. They all told him he was a handsome baby. Staff informed them of the sippie cup and the diaper rash.

Grandmother then told staff that the sippie cup wasn’t sealed well and it spilled in Kayson’s bag. Grandmother gave him a toothbrush shaped like a banana (it was brought in previous visits also) and brushed his teeth a bit then stopped. Grandfather made a comment that Kayson wasn’t smiling much this visit.

After, Grandmother got some Chinese noodles and sat to feed them to Kayson’s hand and he ate them. He pulled up on a chair and Grandfather held him standing, but Kayson sat down. Grandmother got a toy with wooden handles joined by a ribbon. She moved them to the tune of Old McDonald and he looked at it. Grandmother offered it to him, but he didn’t take it.

Grandmother fed him some broccoli and he ate it.

Mother also got a book and put it beside him. She sat behind him and got a board book she brought and read it to him. Grandmother fed him noodles periodically. He then found a toy cell phone mother brought to the visit. He then looked at the toy duck. Mother put a toy teething key ring. He next pulled up on the chair. Mother leaned to him and smiled and Kayson smiled

back. Both grandparents exclaimed that Kayson was smiling in happy sounding voices.

Grandmother then tried to feed him custard, but he didn’t eat it. He sat back down. Grandmother took him and held him to stand up. Grandfather asked Kayson to give him a “hi-5” and when he did, both grandparents exclaimed and smiled. Grandmother got the rocking horse and

Grandfather held him on it and rocked him gently. He got off after a minute.

The minor crawled to another side table and looked at a truck that was there. Grandfather sat with him and mother sat to the side and watched him. Grandmother feed him some more noodles as he stood and played with various toys. Grandmother moved and sat across the room while mother and Grandfather were watching Kayson play with the toys; Kayson smiled as he played with the toys and grandparents made a comment that he was now smiling.

After a few minutes, he crawled away to the blanket and played with the toy cell phone. Grandmother made a comment about how he didn’t make any noise but used to when he was a baby. He crawled to the other table and sat by it and played with some toys. Mother followed him and sat with him. Mother said, “I love you, Kayson,” and smiled at him He soon crawled to various places near the blanket.

Grandfather got a toy car and rolled it to him. Grandmother gave him a bite of food and he smiled. They cheered when he ate. At this point, he smiled frequently when mother or

grandparents had interactions with him. Mother gave him some of the cereal father sent. Kayson crawled to mother, and she held out the cereal and he took some. They all made an exclamation when he was able to feed himself. Mother told staff that the CPS case was “reversed” and is not open anymore.

Kayson and Grandfather played with toy cars and then Kayson found the banana toothbrush and smiled when he put it in his mouth. Grandmother held him and he reached and got a spoon that was on the table he was sitting near. He put it in front of Grandmother’s mouth and she pretended to eat off it, making exaggerated noises. Kayson smiled and then put it back on the table. Kayson then went to the blanket and opened a book. Mother sat beside him and read it to him while Grandfather smiled and watched Kayson.

Afterwards, mother put him in her lap and gave him a stuffed dog she brought. Kayson said, “Ohh, Ohh,” and grandparents mimicked him and smiled. Mother told him she loved him. Grandmother and mother played on the floor with him while Grandfather sat in a nearby chair.

Grandmother helped Kayson stand near a baby seat with multiple toys on it. He played with it a moment and then crawled to the table near the blanket.

About twenty(20) minutes until the end of the visit, Grandmother got a large pad; she brought and laid it out to change Kayson’s diaper. When Grandmother took his diaper off, she noticed the diaper rash. They all made comments about how bad the rash was and how it was hot. Staff looked at rash and saw a red area around his anus about a 2 inch circle from it. There was no bleeding or skin chafing. There was no diaper ointment. They changed his pants to sweat pants grandparent brought. Grandmother said they were more comfortable and breathable and would help with the rash. The pants he was sent in were put in his bag.

Mother then put some toddler building blocks out for him and he pulled some out and set them aside. He then touched a book, and Grandmother started to read it to him. Grandfather said there were six (6) minutes left. Mother put him on her lap and kissed him and then Grandmother held him a few moments until mother asked Grandmother to let Grandfather hold Kayson. Grandmother gave Kayson to Grandfather. Mother told Kayson that “grandfather loves you.” Grandmother stood near them and Kayson leaned to her and Grandfather gave him to

Grandmother. Grandmother put him in a jacket that she brought. Mother sat with Kayson and kissed his cheeks and told him she loved him. Grandmother showed him the toy duck until end of the visit.

Staff announced the end of the visit. Mother held him and grandmother kissed his cheeks. All three told Kayson they loved him. Mother handed staff the backpack and staff took Kayson. Staff escorted Kayson with his backpack to father. Kayson cried until he saw father and then he smiled.

Father gave staff payment. Father also signed the receipt and then left with Kayson. Mother and grandparents cleaned room and left at designated time.

November 28, 2019

The Supervised visit didn’t occur as scheduled since Rally canceled it due to the Thanksgiving holiday. The make-up was scheduled on November 26, 2019.

8 November 26, 2019

Mother arrived at the scheduled time with her mother, blankets, a gift, food, and a cake. Father arrived as schedule and handed Kayson to staff. Father told staff that Kayson had eaten solid food, had a bottle, and had also been changed. He added that Kayson had a sippie cup already full of formula/milk and toys in his backpack. Kayson cried, and staff took him to mother and grandmother.

Once in the room, Grandmother took Kayson from staff; she said, “Hi” and kissed him on the cheek and then gave him to his mother. Staff told mother and grandmother, “father said Kayson ate solid food, had a bottle, and had been changed, and he has, a sippie cup, snacks, and some diaper in his backpack.” Kayson stopped crying once his mother took him and started talking to him.

Mother also gave Kayson a kiss and said, “Today is your birthday, your one (1) today my little nugget.” They all sat on the floor, and mother and grandmother started singing “Happy birthday” to Kayson. Mother then said, “Hi nugget; I missed you.” Kayson crawled to a car on the other side of the blanket, picked it up, and started playing with it. Mother and grandmother watched him as he played with the car. Mother said, “Kayson grandma ‘Lowl’ sent you this book for your birthday, and she said ‘Happy birthday.’” Mother started reading the book to Kayson, and he smiled when she made different animal noises.

Afterwards, mother said, “he talks more; he definitely talks more than the last time.” Kayson smiled at his mother as he ate cheerios and listened to a toy that played nursery rhymes. Mother counted the teeth in his mouth and said, “You have six (6) teeth now.” She added, “A good little boy brushed his teeth; you’re a good little boy aren’t you?”

Grandmother rubbed Kayson’s head and said, “Your happy today.” Mother started reading a book and Kayson said, “Mama.” Kayson pointed to the apples on the table and mother opened the bag, wiped his hands, and gave him a slice on apple. Kayson smiled, danced, and waved “Hi” as he sat on the blanket and ate his apple.

Mother next read a book to Kayson, and Kayson grabbed the book and started playing with it. Mother looked at Kayson and asked him if he wanted to read the book; he smiled and gave the book back to his mother. Kayson picked up rings and put them back on the base, mother and grandmother cheered and said, “Good job.” Kayson tried to put the ring in his mouth and

mother said, “No, no, no, that’s dirty, let me wipe that off.” Mother wiped off all the rings and gave them back to Kayson.

Mother also kissed Kayson on the cheek and said, “You smell good.” Kayson pointed to the wall, and mother said, “That’s a monkey.” Kayson smiled, and mother said, “You’re a happy baby.” Grandmother said, “You want your shoes off.” Grandmother then took his shoes off, and Kayson tried to put his foot in his mouth. Mother and grandmother laughed as he continued to put his foot in his mouth.

Kayson said, “Ohh, Ohh,” when he picked up a red car and started playing with it.

Kayson crawled over to a table with toys on it, and his mother went over to the table and cleaned the cars and let him select the car he wanted to play with. Mother said, “Be careful nugget; we don’t want the car to fall on your foot, okay.” Mother said, “When you were a baby you were cubby wobble, with a double chin”

Kayson crawled to the table, pulled himself up, and started messing with his birthday cake. Grandmother said, “Do you want some cake?” Kayson said, “Mama” and his mother picked him up and kissed him on his cheek and said, “Hi nugget.” Mother sat Kayson down and read him another book.

Grandmother clapped her hands and said, “Clap your hand.” Kayson imitated his

grandmother; he smiled and started clapping his hands as he danced to the nursery rhyme playing in the background. Kayson crawled back to the table and pulled himself up and started messing with the cake again. Grandmother said, “You want some cake?” Grandmother then put some cake on a spoon and tried to give it to Kayson. He then turned his head and grandmother said, Okay you don’t want any cake, that’s alright.” Grandmother picked Kayson up and gave him a hug and a kiss.

Grandmother put the container of Legos on the blanket; Kayson smiled and crawled to the box and started pulling them out. Kayson tried to put the Legos in his mouth and

grandmother said, “No, no, no, let me wipe them off because they are dirty.” Grandmother wiped the Legos off and gave it back to him.

Grandmother also said, “Kayson you want to get your dirty diaper changed?”

Grandmother put a pad on the blanket, laid Kayson down, and tried to change his diaper. Kayson cried and crawled away. Mother said, “It’s okay, it will be quite.” Mother gave Kayson a music toy and a stuffed animal and laid him down and again tried to change his diaper; Kayson began

to cry again and mother and grandmother changed his diaper. Mother rubbed him on the back and then picked him up and said, “It’s okay, sorry baby. It’s okay, are you alright? I sorry

nugget, I had to change your diaper because it was blue. It’s alright, I love you.” Kayson stopped crying and he started smiling and mother said, “I love you little nugget.” Mother gave Kayson an apple and said, “Hi little nugget.”

The minor stared laughing and smiling, when his mother started playing “Pick- A- Boo” with him. Kayson crawled to a toy and started talking to his mother. Mother said, “grandpa John and grandma Lowl said, “Happy birthday.” Kayson stood on his grandmothers legs and used it to play in the container of grapes. Grandmother then said, “He’s very smart; he used my legs to make him taller, so he can be tall enough to play with the bowl of grapes.” Kayson started crying and mother picked up and talked to him and he stopped crying. Mother said, “it’s time to go little nugget; I love you and I’m going to miss you. I’m going to bring you Christmas gifts the next time I see you.”

Staff announced the end of the visit. Mother held him and grandmother kissed his cheeks. Mother and grandmother told Kayson they loved him. Mother handed staff the backpack and staff took Kayson. Staff escorted Kayson with his backpack to father. Kayson cried until he saw father and then he smiled. Father asked staff it he ate anything. He added, “I’m just asking because he can’t tell me.” Staff said, “Yes, he ate some apples and Cheerios.” Father said, “Okay, thank you” and and left at his designated time. Both parents departed at the scheduled time.

December 12, 2019

Mother arrived at the designated time with her mother. They brought apples, cheerios, and a new electric toy dog, a stuffed moose, and a toy electronic piano. Father arrived as

scheduled. He handed Kayson to staff and said that Kayson had a cold and had been to the doctor who verified that it was only a cold and not flu. He added that Kayson may be fussy and to call him if Kayson wouldn’t stop crying. Staff took the minor, who was crying, to mother and grandmother.

Grandmother took Kayson and mother showed him an electronic toy dog and Kayson stopped crying and looked at it. Grandmother cut a piece of apple and gave it to Kayson, but he didn’t eat it. Then, mother and grandmother sat on the floor with him and offered him different

toys. After about ten (10) minutes, Kayson started crying. Mother held him, but he didn’t stop and then grandmother held him. He didn’t stop, and mother got a toy car. He stopped crying a moment, but then started again.

Grandmother said that he was sweating and changed his clothing (the outfit was a thick, fuzzy material) into some clothing she brought with her. He still cried and coughed once.

Grandmother bounced while holding him, but he kept crying. Mother handed him a small stuffed animal and he leaned to the side, and then mother put him down.

He crawled to a blanket and another stuffed animal. Mother then put on the song “Baby Shark” and he stopped crying. Mother also showed him the electronic dog, which he looked at as mother pressed different buttons on it and rolled it around. Mother then found the banana toothbrush he has used in past visits and he held it. She also gave him a pacifier which he held but didn’t use. Mother told Kayson, “You’re so cute” and smiled at him as he played.

Kayson coughed several times while he was playing. He then yawned a few times. Mother read him the book “12 Days of Christmas” and Kayson looked at the book as she read. Grandmother commented that Kayson looked at mother instead of the book. She pointed at mother and then told Kayson that she was his mother.

He played with other toys a few minutes and then mother read him an, “I Spy” book. He held the stuffed moose. Grandmother said she thought his diaper needed changing. Mother smelled him and said he did need changing. Grandmother got out a changing pad and laid it on the floor. Mother got a stuffed animal to show him, and he reached for it but then tried to turn over when grandmother took his diaper off and cried. Grandmother put him in her lap to wipe him and mother helped wiping him.

They used one of the two diapers father sent but there was no change of clothing for Kayson, so they put him in clothing they brought for him. After he was changed, he stopped crying and mother sat with him on the floor. She showed him the toy dog and then read him the “12 Days of Christmas” book. He looked at book as mother showed him the pages.

Afterwards, mother offered him the cheerios that they brought, but he wouldn’t eat them. Grandmother offered him a slice of apple, and he held it but didn’t eat any of it. He then reached for a duck that he has played with at each visit. Next, she read him a felt book canned “Noah’s Ark.” Kayson listened and at times he looked at the animals.

Grandmother sat next to him and wiped his nose and commented that it was “running a lot.” Kayson moved his hands around (in the hand movements for the song “Wheels on the Bus”). Mother asked grandmother to put that song on. Grandmother played it on her phone and in the meantime, mother found a ball and bounced a ball in front of Kayson and he reached for it and bounced it also.

After that, he played with different toys while mother sat with him and engaged with him while he played. Kayson made some hand movements and grandmother and mother mimicked him. Grandmother blew him a kiss and he reached out to her. She did it several times and one time, he smiled and reached for her too. He then played with some trucks. Grandmother put a jacket on him that she brought. She commented that it fit him well and that it was an eighteen month size, but father sent him in a twelve month old outfit that was too tight.

He stood by the table and mother put some toys on it. Mother offered him some cheerios, but he didn’t take them. Grandmother offered him the bottle of milk father sent, but he didn’t take that either. After a few minutes, he reached for some noodles that were out. Mother got them and sat with Kayson on the floor and opened them. Grandmother gave him a plastic fork. He got some noodles with it but gave them to the stuffed moose.

He then stood up and played with some other toys and then went over to the blanket mother had set up where he got the pacifier. Mother read the “12 Days of Christmas” book a moment but then he pressed the button on the electronic duck and swayed back and forth and smiled. He then played with some blocks mother had set on the floor until the end of the visit.

Staff announced the end of the visit. Mother got Kayson’s items together. Staff took Kayson to father and they left. Mother and grandmother stayed their designated time then left.

December 26, 2019

Mother arrived as scheduled with her parents. Father arrived at the designated time with a diaper bag. Staff escorted Kayson to mother. Kayson cried until grandmother took him and held him.

Kayson leaned over to mother, and she took him from grandmother. She placed him on a blanket that was set out with various toys, books, and a pacifier. Mother and Grandparents sat on the floor around the blanket. Mother turned on the song “Baby Shark.” Kayson found the pacifier and put it in his mouth. He then held a stuffed moose mother had for him on the blanket.

Grandmother commented that Kayson was till sick from two weeks ago and wiped his nose. He played with various toys as mother and Grandparents talked to him about the toys he picked up. He coughed and Grandfather commented that Kayson was still coughing. About fifteen (15) minutes into visit, Kayson started crying. Mother picked him up and walked around with him and then showed him a book. He stopped crying and mother sat and read the rest of the book to him. Grandmother found the raison and cereal mix that father sent. Grandmother fed him some, and he ate one and smiled. All three adults cheered that he was happy.

Kayson put his hand to Grandmother’s face as she fed him more raisins. Grandmother kissed his hand. He coughed a “wet” cough while he ate. Mother put one the song “The Wheels on the Bus” and mother and Grandparents did the hand motions to the song. At times, Kayson also made hand motions. Mother read him another book. He picked up a small stuffed bear with a Santa hat on it. Mother told him that he had that bear from his first Christmas. Mother checked him to see if he needed to be changed and said he didn’t need to be.

Kayson got a book and tried to open the book. Grandfather made a comment that Kayson understood to turn the book around to open it. Kayson opened the book and mother told him he was a “smart little baby.” Grandfather got the raisins and fed Kayson some and smiled. Kayson picked up a ball that was on the blanket and threw it. Mother and Grandfather exclaimed that he remembered how to throw the ball (last visit he did not throw it until encouraged and modeled to do so).

Grandmother got him some moccasins and put them on him. She commented about how large his feet were. Mother said that he was also already in eighteen (18) month clothing. Kayson moved to a table with plastic toys on it. Grandmother and mother commented on how tall he was getting. He stood beside it then walked to Grandmother, who hugged him. They all exclaimed about him walking so many steps.

Grandmother offered Kayson a bite of apple, but he did not eat it. He sneezed and Grandmother wiped his nose. As Kayson sat he said, “Momma” and later said, “Dada.” Grandparents enthusiastically commented on his saying words. Mother read him “Sleeping Beauty” pop up baby book for a few minutes until he moved back to the table with the plastic toys on it. Grandmother rearranged the toys, so he could see them better.

Grandmother took a Ferris wheel toy and sat about four (4) feet from the table and turned it. Kayson watched her and then walked over to the toy and they all commented about him

walking well. Grandmother then pulled out some other toys from a bag she brought. Mother got a sandwich she brought and ate some while sitting on the floor with Kayson. Grandmother offered him a bite of apple, but he didn’t eat it. Grandfather said that the last time he loved the apples but didn’t want them this time.

The minor also played with various toys and then crawled over to a baby chair that had toys attached and started playing with the toys there. Mother sat beside it. He stayed there one minute and then walked toward Grandfather. Grandmother moved toys out of his way, and he walked and picked up a plastic stool and held it while standing. All exclaimed about his being able to do this.

Grandfather suggested putting him on the rocking horse, but when Grandmother tried he frowned, so she immediately took him off and put him on the ground. He then crawled back to the blanket and Grandfather and mother followed him there and sat. He coughed again and mother said he may need some water. Grandmother offered him bottled water with a straw, but he didn’t take it. She then got the sippie cup of milk father sent and he drank a sip of that. He then started playing with a toy truck.

He next got a toy cell phone and mother turned it on. He put it down and went back to playing with the truck. He found the cheerios and raisins again and gave some to Grandfather. Mother rubbed his back as he sat there eating cheerios and giving them to Grandfather. Kayson finished the cheerios. Grandmother then brought him some pumpkin flavored cheerios they brought but he spit it out.

Mother got some toddler sized Legos and he played with some of them. Grandfather showed him how to get two apart and Kayson mimicked Grandfather. He parted one set but then couldn’t get another set and made a “humph” noise and threw them back in the box. Mother and Grandparents laughed. He then played with the toy piano and said, “Oh yeah!” Mother checked him to see if he needed changing, but then said he didn’t.

Afterwards, Kayson went back to playing with various toys. Mother put him in her lap and he frowned and mother put him back on the floor. He sat a moment and then started crying. Grandmother picked him up and mother showed him a video of Kayson as a baby and he stopped crying. Mother then put on the baby shark video, and Grandmother bounced him up and down as she stood. Grandmother put him down and he stood while mother gave him a sip of milk.

Grandmother got a book, but he walked away to go back to the baby shark video that was playing again on mothers phone. He stood and ate some cheerios that were on the table.

Grandmother then commented he needed to be changed. He started crying and Grandmother put him in her lap and asked Grandfather to get him a toy. Grandfather showed him a toy while he was being changed. When she was finished, Grandmother stood and held him. Grandfather came and got him from Grandmother. He still cried and then reached for Grandmother, who came and got him. Mother turned on more children’s songs.

Grandmother also bounced him while holding him and he stopped crying. Mother gave him a stuffed bear and showed him a child’s video. Grandfather smiled at Kayson and stood to the side. Kayson leaned to mother, and she tried to hold him while sitting but Kayson started crying again. Grandmother took him and stood. Mother then showed Kayson pictures of himself as a baby, and he stopped crying and looked at them. Grandfather then got the electric duck and showed it to him. He spit out the pacifier. Mother gave him the banana shaped toothbrush and he chewed on it.

Mother rubbed his back as Grandfather showed him the duck. Grandmother made a comment that Kayson’s shirt was wet from Kayson drooling. Mother said there was no change of clothing in the bag. Grandmother put him down on the blanket and he played with some Legos until end of visit.

Staff announced the end of the visit. Mother got Kayson’s items together. Staff took Kayson to father and they left. Mother and grandparents cleaned the room, stayed their designated time, and then left.

January 09, 2020

Mother arrived as scheduled with her mother. Father arrived at the designated time with a diaper bag. Staff escorted Kayson to mother. Kayson cried until mother took him and held him.

Mother placed him on a blanket that was set with various toys, books, and a pacifier. Mother and Grandparents sat on the floor around the blanket, and mother starting playing music from a little lamb. Mother then began reading Kayson a “Winnie the Pooh” book, and as mother read the book, Kayson waved and smiled at his grandmother.

Grandmother asked mother to grab the bag of apples and animal cookies off the table. Grandmother put an apple up to Kayson mouth and he turned his head. After, Grandmother gave

Kayson an animal cookie and he began eating it. Mother, grandmother, and Kayson all danced as they played different nursery rhymes. Kayson pointed to an apple grandmother was holding; grandmother gave him the apple slice and began eating the apple slice.

Grandmother commented that Kayson wasn’t as sick as he was during their last visit. Mother and grandmother noticed that Kayson wasn’t as action as he normally is indicating he usually grabs different toys that are laid on the blanket. Grandmother asked, “What’s going on Kayson; you’re just setting there?” Mother replied he was probably tired. Mother said, “I love you so much Kayson.” Grandmother replied, “Yes your mother love you so much and she talks about you every day.”

Grandmother gave Kayson a bottle and he began drinking it. Mother started talking about Kayson drinking several bottles during the night. Mother commented that Kayson was a good boy indicating he drunk almost the whole bottle. Kayson laid back on his mother and strength out his legs and stared at his grandmother. Grandmother asked Kayson to come to her. Mother asked him, “Are you going to walk to your grandmother?” Kayson got up and grab a stuff animal (dog) and began playing with it, and once he laid the stuffed animal down, he picked up a book. Mother began reading the book to Kayson. Kayson picked up a ball and grandmother asked him to throw the ball to him and Kayson did so. Mother clapped and said, “Good job; can you throw if to grandmother again!” Grandmother gave him the ball again, and he though it to her again. Grandmother and mother both said, “Good job!” Kayson then grabbed a duck and pressed a bottom and when the music came on he began to shake his head and dance. Mother and grandmother both laughed as he continued to dance to the music.

Mother asked Kayson if he could show her how much he walks. Mother said he used to crawl around a lot. Grandmother replied, “I know” and then asked, “Do you want to walk to me?” Kayson saw his mother with a slice of apple and he tried to grab it from her. Mother got another slice of apple from the bag and gave it to Grayson. Kayson began eating the apple and then he put the apple up to his mother mouth, and mother thanked Kayson and pretended to eat the apple slice. Kayson spotted a little chair and began hitting on it like it was a drum. Mother and grandmother commented that he picked the chair up and used it to walk with. Kayson began picking up the chair, and mother and grandmother cheered as he picked the chair up. Kayson smiled at his mother as she talked to him about how strong he was as he continued picking up the chair.

Kayson began playing with Legos and as he did, mother demonstrated how to put the blocks together and take them back apart. Mother and grandmother cheer Kayson on and he smiled as he successfully pulled the blocks apart and put them back together again.

Kayson crawled to the table and pulled a car down on the blanket and began playing with it. Kayson looked around the blanket and spotted a stuffed animal, put the car down grab the stuff animal, and began playing with it. The minor spotted a pouch of food on the blanket and picked it up and gave it to his mother. Mother tried to feed him the food, and Kayson turned his head and grabbed a toy and began playing with it.

Grandmother find nursery rhymes on her cellphone and began playing them, and when the song the bus on the wheels go round and round Kayson began smiling, clapping his hands and dancing. When the song stopped playing, he handed his grandmother the cellphone. As he crawled to the table, he started talking, pulled himself up on table, grabbed two toys and walked to his mother. Once Kayson got to his mother, he handed her one toy, walked to the doll house, and put the other toy in the dollhouse. Mother and grandmother cheered and laughed as Kayson continued to walk around the room talking and smiling as he played with a book.

Grandmother next pulled a toy out the bag and said, “Kayson, we have one more toy for you to play with, while you get your diaper changed.” As grandmother changed his diaper, mother kept him preoccupied with a toy. Grandmother successfully changed Kayson’s diaper and commented that that was the first time he got his diaper changed without him crying. After getting his diaper changed, Kayson climbed on the chair and began playing the food that was located on the table. Mother gave Kayson a piece of banana and got off the chair and walked over to the table and grabbed a toy.

Mother and Grandmother commented that Kayson was a good little boy, as playing with a duck that was playing music. Grandmother picked Kayson up and said, “I didn’t get a chance to hold you today.” Grandmother then picked Kayson up, gave him a hug and kiss, and said, “I love you” and handed him to his mother. Mother then gave Kayson several hugs and a kisses and said, “I love you so much.” Mother sat Kayson’s down; he picked up a toy and began playing with it. Mother said, “You are a sweetheart.” Kayson smiled and called his mother “mama” as she talked, kissed, and hugged him. He next handed his mother different toys and talked to her as

Staff announced the end of the visit. Mother got Kayson’s items together, hugged and kissed him and also said, “I love you.” Kayson said, “Mama” as she kissed him. Staff took Kayson to father, and he asked staffed if there was anything he needed to know such as if he pooped. Staff replied, “No he didn’t pooh; he was changed. He ate bananas and cheerios. There was a pouch of food in his, but he didn’t want to eat it.” Father replied he didn’t put a pouch of food in his bag. Staff responded, “There was a pouch of food in his backpack. I saw mother take it out of his backpack, and when she tried to give it to him, he didn’t want to eat it.” Father then replied, “okay my mother probably put in in his backpack.” Father then left. grandmother cleaned the room, stayed their designated time, then left.

January 23, 2020

Mother arrived as scheduled with her mother. Father arrived at the designated time with a diaper bag and informed staff that there were snacks as well as milk in the diaper bag. Staff escorted Kayson to mother and grandmother; Kayson cried until grandmother took and held him.

Grandmother gave Kayson to mother, and mother placed him on the blanket that was full of toys. Mother picked Kayson up, put him on her lap, and kissed his cheek. Grandmother then took off Kayson shoes and pointed out what looked like a bruise on his lower ankle.

Grandmother asked staff to come look. Staff looked and didn’t see the bruise. Mother and Kayson played with a toy that played music. Kayson bobbed his head, smiled, and rocked from side to side as he listened to the “Wheels on the Bus Go round and round.”

Mother got a book off the table and began reading it to him, and as she read him the book, he smiled and started making noises. Kayson noticed the music playing in the background, stood up, and began dancing to the music. Mother and grandmother smiled and laughed as he danced. Mother also gave Kayson a piece of bread; he got up and walked to the table and got his sippy cup and began drinking from it. Kayson walked back over to his mother and put the bread up to his mother mount; mother pretended she was eating the bread and said, “Thank you nugget for sharing. You’re so nice.” Kayson noticed a container on the table and picked it up. Mother opened the container full of small sliced up grapes and gave him one. Kayson ate a couple more grapes. He then walked away grabbed a book and handed it to his mother. Mother began reading the book to Kayson.

Grandmother and mother sat with him on the blanket; Kayson picked up a ball and began playing with it. Mother kissed Kayson and said, “I love you.” Kayson put the ball down and grabbed a train and started pushing it on the floor, and while rolling the train on the floor, he smiled at his mother. Grandmother started playing the song, “The Wheels on the Bus” and mother and grandmother laughed as he bobbed his head from side to side, rocked back, and forth and smiled as he listened to the music.

Mother next put Kayson in the activity chair and as he played with different thinks on it, he put both of his hands on the side of his mouth and smiled. Mother and grandmother laughed and expressed how cute he looked. Kayson held his hands out to his mother; mother got him out of the activity chair and placed him on the floor, and they began playing with a fire truck. Kayson walked over to the table and grabbed a grape; mother said, “Let me wipe your hand before you put grapes in your mouth.” Mother wiped his hand, and Kayson grabbed a grape and put it in his mouth. Mother read him a book, and he kicked up another book, so mother began reading the new book to him. Kayson smiled at his mother as she read the book to him.

Kayson pointed to a container of Legos, and mother got the Legos down and they started playing with the blocks. Mother called him her little “chubby wubby” and they cheered as he successfully put two blocks together and told him “Good job.” Mother and Kayson continued playing with the Legos, and when he took a block apart and mother said, “Good job.”

Grandmother and mother sat on the floor and mother asked Kayson to put the toys back, and they cheered as he put the blocks back in the container. Kayson walked over to the activity chair and pointed, so mother put him back in the chair and he started playing with different activities on the chair. Mother called him a sweet little angle boy, and he smiled at her. Kayson pointed to a stuffed animal and mother gave it to him. Kayson picked up the stuff animal and talked as he handed it to his mother. Kayson waved and smiled at his mother and grandmother as he played and ate cheerios in the activity chair.

Grandmother asked Kayson to show her how many teeth he had, and he smiled and pointed to the cheerios she was holding; he began dancing ,smiling, and talking when she gave him a cheerio. Grandmother changed Kayson and commented that it was a good diaper change because he did not cry like he normally do. Mother said Kayson, “Good diaper change Kayson. Good boy son.” Mother and son played with a ball, and Kayson talked and smiled as they rolled the ball around the room.

Mother and Kayson also walked around the room playing with various toys. Kayson picked up a toy, and when the song “Tinkle Tinkle Little Star” started playing, he rocked back and forth and smiled. Mother and grandmother laughed and talked about how cute he looked while dancing.

Staff announced the end of the visit. Mother and grandmother confirmed. Mother held him, and mother and grandmother also kissed his cheeks, and they both told Kayson they loved him. Mother handed staff the backpack and staff escorted Kayson to his father. Father asked, “Did Kayson ate anything and have he been changed?” Staff replied, “Yes he ate cheerios and grapes cut up very small pieces, and his diaper was changed about ten minutes ago.” Father then asked, “Did he drink any milk?” Staff replied, “Yes, he drunk some milk.” Mother and Father left at their designated times.

February 06, 2020

Mother arrived as scheduled with her mother. They had toys and food for Kayson. Father arrived at the designated time with a diaper bag and informed staff that there were snacks and only water in the diaper bag. Father then told staff that Kayson had eaten and been changed before arriving. He added that the minor was getting over a virus per the doctor and may have a runny nose still. Kayson coughed as staff held him but did not cry. Staff escorted Kayson to mother and grandmother.

Grandmother took Kayson from staff ,and mother kissed his cheek. Kayson coughed some more before mother took him and put him on the floor where she had several toys placed around. He also played with the electric duck that mother brings each visit and then played with a toy tractor.

He walked around and got other toys. Mother moved near to him and he gave mother several toys and she put it in a pile. Mother commented that he “likes to have things in one pile” in recent visits. He sat down near mother and played with a truck. Mother put the song “Baby Shark” on to play with her phone. She also wiped his nose since it was running.

He got up after a moment and went to the table where mother had set out some eggs and blueberries. He coughed and then ate a bite of a Valentines cookie. He then walked to the doll house and moved some furniture there. He sat near his mother, who had moved near him, and
