COMPETENCY BASED ORIENTATION

Supply Chain Management

The (job title) will carry out his or her duties in a safe, skilled manner (or provide safe, skilled, age-appropriate and population-specific care) according to University Hospitals and Health System administrative policies, departmental policies and procedures, and equipment manuals

(may include any legal guidelines).

I. PROCESS

During the orientation period, the (job title) will be assigned to work with a preceptor (supervisor, or designee) who will be responsible for guiding the orientee through the competency based orientation process. The (job title) must assume accountability and responsibility for identifying his or her learning needs during orientation. The preceptor and the (job title) will collaborate to identify learning opportunities to meet the identified needs.

The preceptor will observe the (job title) in the performance of skills and the use of equipment in the (state specific setting). Once competency has been validated by the preceptor, the preceptor will date and sign the Basic Orientation Competencies accordingly. Each competency statement and line item should be individually dated and signed.At the completion of orientation the preceptor, employee and supervisor will sign the Verification of Orientation Competency.
