This document is to be used in conjunction with the Netsmart Sandbox
environment as a guide. Following the steps included in this guide will allow
you to get a sense for the general workflow for your role and how you will
interact with the Netsmart solutions, once your own system build is
completed. It will provide you with a foundation for understanding the
required data collection and making any design decisions associated with the
project.

Purpose
This exercise will help you become familiar with creating outside providers records in MSO that are providing services to part of your client population.
You will walk through the following steps:
Applying Contract Provider
You will create a provider entity.
Performing Provider Registration
You will create a performing provider record in MSO.
Contracting Provider Registration
You will capture detailed information about the provider as well as the performing providers that provide services for the provider.
Contracting Provider Service Authorization
Create an authorization for a contract provider that provides them with a bucket of codes to be used across their population and does not require a client-level authorization to be completed.
Contracting Provider Termination
You will terminate a contracting provider’s association with a funding source.
Terminology
Guarantor/Payors/Funding Source – the financial entity that will reimburse your organization for the cost of the service rendered to the client.
Outside Providers – facilities that your organization may contract with to provide services to clients outside of your

1. Launch Avatar from the provided URL address: https://sandbox.asp.ntst.com/plexus/pm 2. Click the Start button to launch the application.
3. Log in by selecting the Server you are going to log in. 4. Type “SAMPLE” in the System Code field.
5. Type your username in the Username field. You will use INTAKE1, INTAKE2, INTAKE3, INTAKE4, or INTAKE5 as the
Username.
6. Type your password in the Password field. The password for all the Intake users is INTAKE99. 7. Click the Sign In button.

Applying Contract Provider
Applying Contract Provider is one of the first three steps in creating a contract provider record in Avatar. It’s most utilized to track contract providers that are in contract negotiations with your organization.
1. On the Home View, locate the Forms and Data widget.
2. Type “Applying” in the Search Forms field. Select Applying Contracting Provider.
3. A Select Provider window will appear.
4. Type “Sunflower House” in the search field. Click New Provider. 5. Click Yes in the Auto Assign Next ID Number message screen.
7. The Applying Contract Provider form will open.
8. Type “02/01/2000” in the Application Date field.
9. Click the Facility radio button from the Type of Contracting Provider field. 10. Type “Sunflower House” in the Name field.
11. Click the Type of Contact drop-down list. Select Phone. 12. Type a phone number in the Office Telephone field.
13. Type “244 Sun Street” in the Office Address – Street 1 field. 14. Type a zip code in the Office Address Zip Code field.
Note: The City and State fields will automatically populate. If note, please manually enter the City and State in the required fields.
15. Click Submit to complete the form.

Performing Provider Registration
Performing Provider Registration is utilized to define a performing provider
1. On the Home View, locate the Forms and Data widget.
2. In the Forms and Data widget, type “Performing Provider Registration” in the Search Forms field. Select Performing Provider Registration.
3. The Select Performing Provider window will open.
4. Type “Gibson” in the search field. 5. Click New Performing Provider.
7. Type “GIBSON,JANET” in the Name field.
8. Type “01/01/2000” in the Registration Start Date field.
9. Click Performing Provider’s License Data section form the form tree on the left side of the screen..
10. Click Add New Item.
11. Click the License Type drop-down list. Select LCSW. 12. Click the License State field. Select a state.
13. Type “123456” in the License # field.
14. Click the Yes radio button in the License Data Verified field. 15. Click Submit to complete the form.

Contracting Provider Registration
The Contracting Provider Registration form is used to capture detailed information about the provider as well as the
performing providers that provide services for the provider.
1. Locate the Forms and Data widget on the Home View.
2. Type “Contracting Provider Registration” in the Search Forms field. Select Contracting Provider Registration.
3. The Select Provider window will open.
4. Type “Sunflower” in the search field. 5. Click Sunflower House to highlight it. 6. Click the Select button.
7. The Contracting Provider Registration form will open.
Note: Some fields will already be populated based on the information completed in the Applying Contracting Provider form.
8. Type “02/01/2000” in the Registration Date field.
9. Click the In Network radio button from the In Network/Out Of Network field.
11. Scroll down the form to view the Performing Provider required field. 12. Type “Gibson” in the Performing Provider field.
13. Click Add New Item. 14. Select JANET GIBSON. 15. Scroll to the top of the page.
Note: Fields have become populated.
16. Click Performing Provider’s License Data from the Form Tree on the left side of the screen.

Contracting Provider Service Authorization
Contracting Provider Service Authorization authorizes services to be performed by a contracting provider, for any member. 1. On the Home View, locate the Forms and Data widget.
2. Type “Contracting Provider Service” in the Search Forms field. Double click Contracting Provider Service Authorization.
3. The Select Provider window will open.
4. Type “Sunflower” in the search field. 5. Click Sunflower House.
6. Click Select.
8. Click the Approved radio button from the Current Authorization Status field. 9. Click the Initial radio button from the Initial or Continuing Authorization field. 10. Type “02/01/2005” in the Begin Date Of Authorization field.
11. Type “02/01/2020” in the End Date Of Authorization field.
12. Click the Individual radio button from the Authorization Grouping or Individual Authorizations field. 13. Click the CPT Code radio button from the Procedure Code Type (1) field.
14. Click the Code Authorized (1) drop-down list. Select Assessment Interactive (90802). 15. Type “1000” in the Units Authorized (1) form.
16. Click Submit to complete the form.

Contracting Provider Termination terminates a contracting provider’s association with a funding source.
1. Locate the Forms and Data widget on the Home View.
2. Type “Contracting Provider Termination” in the Search Forms field. Select Contracting Provider Termination.
3. The Select Provider window will open.
4. Type “Sunflower” in the search field. 5. Click Sunflower House to highlight it. 6. Click the Select button.
8. Double click the highlighted row to open the Contracting Provider Termination form.
9. Click T in the Termination Date field.
10.
Click Submit to complete the form.11.
Once a Contracting Provider’s record has been terminated in Avatar, a new record can be opened via the Contracting Provider Registration form if needed.Section Recap:
