Contemporary Forums and the Faculty would like to welcome you to this educational offering. Contemporary Forums' staff members will be available online throughout the meeting to assist you with any questions or technical issues. Contemporary Forums' mission is to provide continuing education to enhance the skills and knowledge of health care professionals and improve patient care. We trust that this conference will meet your expectations and that your educational practice will be positively influenced by what you learn from this program. To assist you in understanding program details, PLEASE READ THE FOLLOWING INFORMATION BEFORE THE WEBCAST BEGINS.

I. CERTIFICATE OF ATTENDANCE

You must first complete the online speaker evaluations for each session you participated in and then the overall webcast evaluation before you can print your certificate. Certificate of Attendance will be available to view/print via your CE Transcript link in the Contemporary Forums Online at any time by logging in to your account.

II. SPEAKER HANDOUTS

Slide/handout materials, when available, will be posted by the speakers prior to the conference in the Contemporary Forums Online at ContemporaryForumsOnline.com. You may download or print these materials from The 2014 National Conference of Neonatal Nursing page to preview and they will also be available to you online during the webcast.

III. RECORDED WEBCAST SESSIONS

Approximately 2 weeks after the end of the conference the recorded webcast sessions will be posted in the Contemporary Forums Online. You will have access to these materials for two years. If you missed any of the live webcast sessions you will be able to earn CE credit at no cost for the online enduring materials by taking a post-test and short evaluation. You will print a separate CE Certificate for any missed live webcast sessions. Main Conference participants will also have complimentary access to concurrent sessions not part of the live webcast and may earn additional CE credit online at no cost.

IV. SPEAKER QUESTIONS

A. Speaker/Webcast Evaluations - You may access the speaker evaluations online after each webcast session concludes and

the overall webcast evaluation at the end of your participation. You will not be able to view/print your Certificate of Attendance until evaluations for sessions you participated in and the overall evaluation are completed. Evaluations should be completed by June 2, 2014. Once evaluations are completed, you can access your certificate at any time by logging into your

Contemporary Forums Online account then clicking the CE Transcript link.

B. Post-Conference Survey - You will receive a brief post-conference email survey from Contemporary Forums approximately

2-4 weeks after the conference and again 4-6 weeks later. The purpose of these surveys is to measure the effect this conference has on the care you provide to your patients. Please take the time to complete and return the survey(s) to us as they will be very helpful in planning future conferences.

VII. CONTINUING EDUCATION CREDIT

A. Contemporary Forums’ Continuing Education Policy - Contemporary Forums is designated as a provider of continuing

education by the ANCC as well as several other accrediting agencies. We are required by these organizations to adhere to their guidelines for awarding CE credit. Attendance at this educational program is documented on the honor system, believing that it is reasonable for adult learners to verify their attendance by completing the online evaluation following the conference.

B. Accreditation - Participants are responsible for registering their credit with the appropriate boards, organizations or

agencies.

Center's Commission on Accreditation.

Contemporary Forums is approved by the California Board of Registered Nursing, Provider #04516. (CA BRN contact hour = 50 minutes)

NCC Certified Nurses and Nurse Practitioners

Contemporary Forums is a continuing education organization recognized by the National Certification Corporation. This conference covers a variety of the Core Competency Areas and Content Topics required for the NCC's Certification Maintenance in your specialty.

Pharmacology Credit for Prescriptive Authority Preconference A: 1.5 hours

Main Conference: 1.75 hours available

Rx indicates pharmacology content. Because states' requirements vary, contact your Board of Nursing for more information. Case Managers

This program has been pre-approved by the Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers. The course is approved for 23 CE contact hours. The CCMC awarded a total

of 23 contact hours for the Preconference and Main Conference. Certified Case Managers must call CCMC customer service to adjust their credit since the CE hours shown on your Certificates of Attendance are different from the total hours approved by CCMC.
