Logisticare Transportation (Medicaid Reimbursed Transportation):

 Logisticare partners with non-profit agencies to ensure access to essential health care.

 Logisticare acts as a broker for the Medicaid Clients. The Medicaid Client calls the number on the back of their card to access Logisticare. The client needs to give them all the information about the

appointment such as their Name, Address, Date and time of appointment, approximately how long their appointment will be and where the appointment is exactly. Logisticare takes this information and finds a partnering transportation provider to provide the trip. RRCS is one of the partners to Logisticare. Our agency receives reimbursement for these Medicaid trips. This is why it is so important to ask if the client is a Medicaid Client.

 Eligibility Requirements: Must be a Medicaid recipient and have contacted Logisticare five business days in advance of appointment.

Availability & Make a Reservation

 Transportation is available 24 hours a day, 7 days a week, holidays included.

 Have your Medicaid number ready as well as your pick-up address and the address of your destination. The customer service representative (CSR) will verify that you are eligible for Medicaid transportation. That means that you are currently enrolled in Medicaid, that you cannot transport yourself and the trip is to a Medicaid-covered service.

 Some services require a Prior Authorization (PA) before transportation can be provided. Ask your Medicaid service provider to request a PA for you if one is necessary for your Medicaid service.

 Please request your trips at least 5 days in advance unless it is an urgent trip.

 If you become ill and your doctor can see you in less than five days, call Logisticare and request an Urgent Trip.

 If you will have at least three trips per week at the same time and to and from the same destination, you can request a Standing Order and avoid booking each trip individually.

 The CSR taking your reservation will give you a pick-up time and a unique ID number for the trip. Be ready to go 15 minutes before the pick-up time.

 Saving the trip number will help the CSR retrieve your trip if you need to make a change or have a complaint.

 The trip is sent electronically to your LogistiCare regional office for assignment to a transportation provider.

 The same transportation provider will pick up and deliver you to the appointment and return you home afterward.

 Regional Transit Vans & Trolleys are available in Culpeper County. The route does not change daily or cross county lines. Senior Citizens ride free on Mondays.

 Saturdays: 9:30 a.m. – 2:30 p.m.

On Demand ADA Accessible Van:

 The ADA Van/Bus travels daily in and around the Town of Culpeper and will get the person where they need to go. This van/bus will NOT cross county lines. Call between 7:00 a.m. and 2:00 p.m. to pre-register and to schedule the trip. Person must call at least 24 business hours in advance or earlier to schedule a trip. This is a first come - first serve program. Cancel trips at least 2 hours before the scheduled time.

 Eligibility Requirements: Anyone ADA certified of any age (9 and under accompanied by an adult) can ride on the ADA Accessible Van. No guarantee that the van will be available for person’s trip if the van is already reserved.

Demand Response Van (County Express):

 The Demand Response Van will pick up person at residence in county or town and bring client to appointments, errands, etc. Reservations are required and it is a first come - first serve program. No guarantee that the van will be available for person’s trip if the van is already reserved.

 LifeCare Medical Transports is a health care transportation provider. LifeCare provides wheelchair van services for physically challenged clients who require transportation to medical appointments, dialysis treatments, and other outpatient medical care.

 LifeCare also provides ambulatory services for bedridden patients to and from hospitals, nursing care facilities, doctor’s offices, dialysis, radiation, physical therapy treatments and other outpatient services.

 LifeCare covers appointed Logisticare transports (Medicaid Transportation Only) and also certain Medicare transports. Billing to Medicare is provided by LifeCare but many stipulations are in place and every situation varies as to whether there is coverage for the client.

 For Transport by ambulance the client must be bed-ridden to be covered by Medicare.

 Eligibility Requirements: Each situation varies as to coverage. LifeCare will bill Medicare if applicable. Private pay is also an option and they do accept credit cards.

 Availability: LifeCare is available 24 hours a day, seven days a week for both local and long distance, emergency and non-emergency transports. Call LifeCare Communications Center at 540-752-5883.

American Cancer Society

 American Cancer Society – This program is volunteer-based and assists anyone with transportation for cancer treatments.

 Eligibility Requirements: Must be scheduled for Healthcare/Cancer treatments or appointments. Must be ambulatory. Escorts are allowed to accompany patient.

 Availability: First come/first served basis. At least 5 business days notice before transport. Monday thru Friday. Transportation depends on availability of volunteers. No fee.

Transportation to your healthcare and legal appointments. Citizens may also call the senior center in their county for assistance in setting up volunteer transportation.

 Care-A-Van is a program of RRCS and is available Monday thru Friday. It is not available holidays and during inclement weather. If inclement weather, listen to your local radio station to see if there is an announcement that states, “The Rappahannock-Rapidan Community Services Transit Service will not be running today”. If you hear this, the program will not be transporting and you will need to reschedule your transportation through the Care-A-Van Staff.

 Eligibility Requirements: Must give 48 business hours notice, Monday thru Friday. Focus is on senior citizens and persons with disabilities.

 Availability: Monday thru Friday. First Come/First Served. Transportation depends on availability of volunteers.

 The Rappahannock-Rapidan Regional Commission Commuter Services program (RRCommute) operates a rideshare database, matching commuters with carpools and vanpools. Commuters contact RRCommute and are paired based on their destinations. In support of this program, there are Park and Ride lots across the region. RRCommute can also provide information on other commuting options or alternatives, such as Virginia Railway Express (VRE), Metro and Academy Bus, a commuter bus leaving Culpeper and Warrenton each weekday with stops in Northern Virginia and Washington DC. Assistance is available for new and current vanpools.

 RRCS Transit Department functions as a primary source of transportation for the majority of clients of the Agency’s varying day support and treatment programs. These clients are transported to the various programs that the Agency operates so that they may participate in a developed and managed plan of care that meets their specific needs.

 Eligibility Requirements: Transports are available for clients of the RRCS unless it is a Demand Response transport through Medicaid reimbursement.

 Availability: Rides can be scheduled for Monday thru Friday (no holidays). First Come/First Serve. Transportation depends on availability of volunteers. Donations accepted.

 A Transportation Coordinator matches volunteer drivers with Fauquier County older adults age sixty or older, and/or wheelchair bound or legally blind of any age in need of transportation, and are unable to drive due to disabilities or health reasons.

 Reservation Requirements: Must give 48 hours notice to set up transport and this does not include the day of the call. Focus is on senior’s age sixty or better. Call and leave a message to schedule a ride anytime, any day. We will return your calls Tuesday thru Friday between 9:30 am and 2 pm.  Logisticare partners with non-profit agencies to ensure access to essential health care.

 Logisticare acts as a broker for the Medicaid Clients. The Medicaid Client calls the number on the back of their card to access Logisticare. The client needs to give them all the information about the

appointment such as their Name, Address, Date and time of appointment, approximately how long their appointment will be and where the appointment is exactly. Logisticare takes this information and finds a partnering transportation provider to provide the trip. RRCS is one of the partners to Logisticare. Our agency receives reimbursement for these Medicaid trips. This is why it is so important to ask if the client is a Medicaid Client.

 Eligibility Requirements: Must be a Medicaid recipient and have contacted Logisticare five business days in advance of appointment.

Availability & Make a Reservation

 Transportation is available 24 hours a day, 7 days a week, holidays included.

 Have your Medicaid number ready as well as your pick-up address and the address of your destination. The customer service representative (CSR) will verify that you are eligible for Medicaid transportation. That means that you are currently enrolled in Medicaid, that you cannot transport yourself and the trip is to a Medicaid-covered service.

 Some services require a Prior Authorization (PA) before transportation can be provided. Ask your Medicaid service provider to request a PA for you if one is necessary for your Medicaid service.

 If you become ill and your doctor can see you in less than five days, call Logisticare and request an Urgent Trip.

 If you will have at least three trips per week at the same time and to and from the same destination, you can request a Standing Order and avoid booking each trip individually.

 The CSR taking your reservation will give you a pick-up time and a unique ID number for the trip. Be ready to go 15 minutes before the pick-up time.

 Saving the trip number will help the CSR retrieve your trip if you need to make a change or have a complaint.

 The trip is sent electronically to your LogistiCare regional office for assignment to a transportation provider.

 The same transportation provider will pick up and deliver you to the appointment and return you home afterward.

 LifeCare Medical Transports is a health care transportation provider. LifeCare provides wheelchair van services for physically challenged clients who require transportation to medical appointments, dialysis treatments, and other outpatient medical care.

 LifeCare also provides ambulatory services for bedridden patients to and from hospitals, nursing care facilities, doctor’s offices, dialysis, radiation, physical therapy treatments and other outpatient services.

 LifeCare covers appointed Logisticare transports (Medicaid Transportation Only) and also certain Medicare transports. Billing to Medicare is provided by LifeCare but many stipulations are in place and every situation varies as to whether there is coverage for the client.

The Rappahannock-Rapidan Regional Commission Commuter Services program (RRCommute) operates a rideshare database, matching commuters with carpools and vanpools. Commuters contact RRCommute and are paired based on their destinations. In support of this program, there are Park and Ride lots across the region. RRCommute can also provide information on other commuting options or alternatives, such as Virginia Railway Express (VRE), Metro and Academy Bus, a commuter bus leaving Culpeper and Warrenton each weekday with stops in Northern Virginia and D. C. Assistance is available for new and current vanpools.

 RRCS Transit Department functions as a primary source of transportation for the majority of clients of the Agency’s varying day support and treatment programs. These clients are transported to the various programs that the Agency operates so that they may participate in a developed and managed plan of care that meets their specific needs.

 Eligibility Requirements: Transports are available for clients of the RRCS unless it is a Demand Response transport through Medicaid reimbursement.

Logisticare Transportation (Medicaid Reimbursed Transportation):

 Logisticare partners with non-profit agencies to ensure access to essential health care.

 Logisticare acts as a broker for the Medicaid Clients. The Medicaid Client calls the number on the back of their card to access Logisticare. The client needs to give them all the information about the

appointment such as their Name, Address, Date and time of appointment, approximately how long their appointment will be and where the appointment is exactly. Logisticare takes this information and finds a partnering transportation provider to provide the trip. RRCS is one of the partners to Logisticare. Our agency receives reimbursement for these Medicaid trips. This is why it is so important to ask if the client is a Medicaid Client.

 Eligibility Requirements: Must be a Medicaid recipient and have contacted Logisticare five business days in advance of appointment.

Availability & Make a Reservation

 Transportation is available 24 hours a day, 7 days a week, holidays included.

 Have your Medicaid number ready as well as your pick-up address and the address of your destination. The customer service representative (CSR) will verify that you are eligible for Medicaid transportation. That means that you are currently enrolled in Medicaid, that you cannot transport yourself and the trip is to a Medicaid-covered service.

 Some services require a Prior Authorization (PA) before transportation can be provided. Ask your Medicaid service provider to request a PA for you if one is necessary for your Medicaid service.

 Please request your trips at least 5 days in advance unless it is an urgent trip.

 If you become ill and your doctor can see you in less than five days, call Logisticare and request an Urgent Trip.

 If you will have at least three trips per week at the same time and to and from the same destination, you can request a Standing Order and avoid booking each trip individually.

 The CSR taking your reservation will give you a pick-up time and a unique ID number for the trip. Be ready to go 15 minutes before the pick-up time.

 Saving the trip number will help the CSR retrieve your trip if you need to make a change or have a complaint.

 The trip is sent electronically to your LogistiCare regional office for assignment to a transportation provider.

 The same transportation provider will pick up and deliver you to the appointment and return you home afterward.

LifeCare Medical Transports, Inc.

 LifeCare Medical Transports is a health care transportation provider. LifeCare provides wheelchair van services for physically challenged clients who require transportation to medical appointments, dialysis treatments, and other outpatient medical care.

 LifeCare also provides ambulatory services for bedridden patients to and from hospitals, nursing care facilities, doctor’s offices, dialysis, radiation, physical therapy treatments and other outpatient services.

 LifeCare covers appointed Logisticare transports (Medicaid Transportation Only) and also certain Medicare transports. Billing to Medicare is provided by LifeCare but many stipulations are in place and every situation varies as to whether there is coverage for the client.

 For Transport by ambulance the client must be bed-ridden to be covered by Medicare. The cost is approximately $395.00 (as of 6/15/09) base pay and $12.00 per mile. Wheelchair transports are $70.00 each way (base pay as of 06/15/09) if the transport is less than 10 miles - one way. If the mileage is more than 10 miles one way than there is a $4.00 per mile charge starting with mile one (1) in addition to the $70.00 base pay.

 Eligibility Requirements: Each situation varies as to coverage. LifeCare will bill Medicare if applicable. Private pay is also an option and they do accept credit cards.

 Availability: LifeCare is available 24 hours a day, seven days a week for both local and long distance, emergency and non-emergency transports. Call LifeCare Communications Center at 1-540-752-5883.
