1.

PURPOSE

To set forth additional site requirements at the BASF Corporation (“BASF”) Wyandotte,

Michigan (“Wyandotte”) site necessary to comply with

·

BASF policy BC032.017, Confined Space Entry, and

·

State of Michigan’s General Industry Safety Standard Rules R408.19001

-R408.19002, Confined Space Entry and General Industry Health Standard Rules

325.63001- 325.63049, Permit Required Confined Spaces.

2.

DEFINITIONS

2.1.

See Section 2.0 of BC032.017, Confined Space Entry for a complete list of

BASF Definitions related to confined space.

2.2.

Confined Spaces

Spaces (such as tanks, vessels, bins, silos, boilers, vaults, pits, ditches, or

sewers) which can be bodily entered, have limited or restricted means of egress,

are not designed for continuous employee occupancy, and have one or more of

the following characteristics:

·

contain or have the potential to contain a Hazardous Atmosphere (such as

having held flammable, explosive, acidic, caustic, toxic materials) or where

oxygen enriched or deficient atmospheres or noxious odors may exist

·

contain a material that has the potential for engulfing an entrant

·

have an internal configuration such that an entrant could be trapped or

asphyxiated by inwardly converging walls or by a floor which slopes downward

and tapers to a smaller cross-section, or

·

contain any other recognized serious safety or health hazard, such as, ionizing

radiation, electric shock, temperature extremes, or moving parts.

Note: Ladders and temporary, movable, spiral, or articulated stairs are considered a limited or restricted means of egress. Fixed industrial stairs will be considered a limited or restricted means of egress when the conditions or physical characteristics of the space, in light of the hazards present in it, would interfere with the entrant’s ability to exit or be rescued in a hazardous situation.

2.3.

Confined Space Rescue Team

A site-wide group of volunteers from maintenance, production, engineering and

EHS who are properly trained, equipped and authorized to perform entry rescue in

Confined Spaces.

2.4.

Entry

The action by which a person passes through an opening into a Confined Space

and is considered to have occurred as soon as any part of the entrant's body

breaks the plane of an opening into the space.

2.5.

Entry Supervisor (Permit Issuer)

The operations/owning department person (such as operations supervisor,

process owner or team leader) responsible for

·

determining if acceptable Entry conditions are present at a Confined Space,

·

issuing Confined Space Entry Permit,

·

overseeing Entry operations, and

·

terminating Entry activities, as required.

2.6.

Limited/Restricted Egress

·

Areas that are difficult to exit quickly, or

·

A space deeper than 4 feet which does not have a permanent stairway or

ladder.

2.7.

Site Safety “Center of Excellence” (COE)

Multi-disciplinary team established to facilitate site safety-related activities.

·

Cost Center oversight of Confined Space Rescue Team.

2.8.

Permit Acceptor

The person in charge of the Entry Team that:

·

attests that their personnel entering the Confined Space are fully trained and the

required equipment has been inspected and is functional.

·

walks down the job and reviews safety requirements with permit issuer and

·

signs the Permit Acceptor part of the Entry Permit.

2.9.

Operations/Owning

The Department that owns or controls the Confined Space and requests that a

Confined Space Entry be performed.

3.

SCOPE

Applies to all Personnel at the Wyandotte site.

4.

PROCEDURE

4.1.

Compliance With Corporate Policy/Procedure

·

Persons entering a Confined Space must comply with the Authorization

requirements specified in Section 4.1.1 of BASF procedure

BC032.017,Confined Space Entry.

·

This procedure is in compliance with BC032.017,Confined Space Entry.

·

Employee participation at the Wyandotte site shall comply with the

requirements specified in Section 4.1.3. of BC032.017,Confined Space

Entry.

4.2.

General Requirements

4.2.1. Operations/Owning Department

The Operations/Owning Department issues the Confined Space Entry

Permit. Prior to issuing the permit the Permit Issuer and Permit Acceptor

shall:

· personally inspect the confined space preparation, · review precautions taken,

· review personal protective equipment to be used,

· review procedures and instructions for all personnel involved in the assignment,

· ensure the permit is located at the work site,

· provide an opportunity for the confined space entrant or authorized representative to observe atmospheric monitoring.

4.2.2. Issuance and Authorization

No person shall enter a Confined Space without the prior issuance and

posting of the entry permit at the Confined Space entrance.

4.2.3. Confined Space Exemptions

Areas determined not to meet the definition of a Confined Space by joint

opinion of the Process Owner, Maintenance Team Leader and EHS.

4.2.4. Confined Space Entry Exceptions

·

Exceptions to required equipment or conditions may be granted

through joint signatures by the Process Owner (or properly trained

staff-level designee) and the Maintenance Team Leader (or

Maintenance Facilitator) on the Entry Permit.

·

Only those equipment and condition exceptions as specified on the

back side of the Entry Permit are allowed.

4.2.5. Immediately Dangerous to Life & Health (“IDLH”) Policy

While it is a policy not to routinely enter atmospheres immediately

dangerous to life and health (IDLH) it is recognized that certain emergency

circumstances (e.g. Confined Space rescue, etc) may require IDLH entry

·

IDLH entry will not be performed where a flammable or explosive

atmosphere exists.

4.3.

Permits

·

Permits shall be issued and cancelled according to the requirements specified

in Section 4.2 of BC032.017,Confined Space Entry.

The Wyandotte site has the following additional requirements with the

following title and order:

The Wyandotte site entry permit (see Attachment) contains all components required by 4.2.2 of BC032.017. 2 Special Procedures, Instructions, Exceptions Section 4.2.3.

Equipment or condition exceptions are noted on the Entry Permit and signed off by the Process Owner (or staff-level designee) and Maintenance Team Leader (or Maintenance Facilitator).

3 Duration Section

4.2.4.

· The permit shall not be valid for longer than one shift of personnel performing the Entry activities (maximum 16 hours). · New (different shift) Entry Supervisors must re-validate or

re-issue the entry permit and inform the Permit Acceptor of the supervision change.

4 Authorization Section

4.2.1

The permit must be signed by designated representatives from both the Operations/Owning Department (Permit Issuer) and Maintenance (Permit Acceptor).

5 Entry Permit

Termination

Section 4.2.4.

The permit can be terminated by anyone participating in the Confined Space Entry based upon:

· Completed entry operations or 16 hours has elapsed since permit issuance.

· A condition that arises that is not allowed under this procedure.

· A condition that arises that any participant considers unsafe. · Permit Acceptor or crew changes.

Section 4.2.5

· Upon completion or discontinuation of the Entry, the

completed Confined Space Entry permit shall be returned to the Operating/Owning department supervision.

· Any problems encountered during an Entry shall be noted on the permit so that appropriate revisions to the site program can be made.

· Completed permits shall be retained at an interval no greater than one year to facilitate the program review stated in Section 4.8

Contractor Employees shall be debriefed concerning the program and any hazards that were created or confronted during Entry at the end of any Entry operation.

· If product or other material remains in the vessel after cleaning, careful review of the situation by Operations/Owning and

maintenance personnel is required to identify special provisions (e.g. re-cleaning, increased/ improved monitoring, ventilation or ppe).

· At least two Rescue Team members be notified and on site to respond (per Corporate Std. Section 4.6.2).

· Air testing can only be conducted by Operations/Owning Personnel who have received documented training in

· span gas checking, · calibration,

· testing equipment use, and · data interpretation.

2. Preparation and Isolation.

All adjoining piping and hazardous energy sources shall be de-energized and isolated per Lock, Tag,

and Try procedures contained in BC032.014 and WYN032.010. NOTE: If the Confined Space

contains internal power driven mechanisms, such as agitators, all switch gear and other power sources shall be " locked, tagged and tried". Look for stored inertial energy of rotating equipment, as well as, all hydraulic and pneumatic energy sources.

3. Contaminated Equipment

· The Confined Space and inter-connecting equipment shall be decontaminated in compliance with the Contaminated

Equipment procedure WYN032.046.

7 Preparation and Isolation Requirements Section 4.3.4. 4. Radioactive Devices

5. Postings & Barricades

· Cautions signs and barricades must be posted as specified site procedure WYN032.002.

· “Caution – Confined Space Work Underway” will be posted in the immediate work area during entry. · Yellow caution tape may be used to additionally

highlight the area.

· Numerous Confined Spaces exist at the Wyandotte site and vary in size, configuration (layout) and hazard type.

· The types include tanks, tank trucks, reactor vessels, railcars, pits, pipes, ditches, active excavations, etc.

Note:

· The Site does not distinguish between Permit and non-permit Confined Spaces as defined in the OSHA

Standard, Permit-required Confined Spaces (1910.146).

· Entry into a Confined Space always requires a permit.

· Posting of signs is not typically performed for those Confined Spaces that fall into general categories of known or apparent Confined Space status.

· The Business Unit, however, may choose to post any and all Confined Spaces that fit into these general categories.

· Employees are informed of the existence and location of the Unit’s Confined Spaces through appropriate training.

· The Unit is responsible for evaluating the workplace to determine if confined spaces exist and to ensure that personnel are adequately trained to recognize a confined space and the potential hazards associated with such space.

· Confined Spaces that do not fall into general categories of known or apparent Confined Space status that could inadvertently have someone enter the space, are identified through posting.

· Postings will read, " DANGER, CONFINED SPACE, ENTER BY PERMIT ONLY".

· Barriers are used to prevent unauthorized entry into open manholes or excavations.

· Spaces appearing to be Confined Spaces but determined not to be are exempted from the requirements.

· Confined Spaces which do not contain a hazard or potential hazard capable of causing physical harm or for which adherence to a requirement would increase the hazard (e.g., blowing air in a dusty environment) may be also be exempted from one or more of the Program requirements.

· Exemptions must be documented in writing.

· Exemptions must be reviewed and signed off by the · Process Owner (or Staff-level designee), · Maintenance Team Leader (or designee), and · EHS Safety Coordinator (or designee).

8.

Atmospheric Conditions

· An Entry Permit will not be issued for Confined Spaces containing less than 19.5% or greater than 21% oxygen. · The oxygen analyzer must be in close proximity to the

entrants at all times.

· An Entry Permit will not be issued for Confined Spaces with any detections on the combustible gas meter. · The combustible gas meter must be in close proximity to

the entrants at all times.

· Monitoring for toxic level gases will be conducted if a Confined Space contains or has contained a toxic material which can be detected by direct-read methods. The entry supervisor will consult, as necessary, with EHS to

determine applicable direct read methods.

· In situations where the concentration cannot be maintained below the permissible limit, EHS is contacted to evaluate acceptability of issuing a permit and assist in determining the appropriate ppe and other equipment necessary for safe entry.

· Where a potential hazardous atmosphere is being generated during the course of the entry operations, such as during welding or burning, supplied air respiratory protection is required unless testing has been conducted to verify the absence of a hazardous atmosphere and the space is being ventilated.

· When conducting excavations, the project-specific IH protocol outlined in the Project Excavation Guide (PEG) obtained from the M.E.S.S. Team for soil excavations must be followed.

· Periodic toxic level re-testing or continuous monitoring may be required due to changing conditions or creation of toxic conditions (e.g. welding, etc.).

· All testing will be documented on the Permit with the test date and time, acceptable level (including toxics), test result and the date of the last span gas check or calibration and tester’s initials.

· Entrants must be notified of their right to witness (or have a designated representative witness) any atmospheric testing performed within the Confined Space as required in the 1999 OSHA Standard clarification.

· The entrant can request re-testing of the atmosphere anytime there is a reasonable belief that atmospheric conditions within the Confined Space have significantly changed.

§ Heat, humidity, and cold stress hazards during Confined Space Entry shall be controlled to prevent adverse health effects to Authorized Entrants.

10. Required Entry Equipment

· Each entry must have the equipment specified on the Entry Permit unless appropriately excepted with Process Owner (or staff-level designee) and Maintenance Team Leader (or Maintenance Facilitator) signature.

4.4.

Monitoring

·

Monitoring must be consistent with that specified in Sections 4.5.,

BC032.017, Confined Space Entry, and

·

§ The atmosphere in the Confined Space shall be monitored continuously during the Entry activities for oxygen.

§ The atmosphere in the Confined Space shall be monitored continuously during the Entry activities for flammable gases/vapors when work activities to be performed in a Confined Space are expected to generate flammable gases/vapors or when the confined space has contained flammable gases/vapors. In situations where continuous monitoring is technically infeasible, pre-determined periodic monitoring shall be conducted as

appropriate.

4.5.

Rescue

·

Rescue Services must be consistent with that specified in Sections 4.6.,

BC032.017, Confined Space Entry, and

·

· Full body harnesses with retrieval lines attached to a mechanical device or fixed point are required for all entries unless the design of the space prohibits use or creates greater hazard.

· Exceptions to use of harnesses, lines and/or mechanical or fixed point retrieval systems must be written on the Entry Permit. Rescue

Section 4.6.2.

2. Rescue Services

· Non-entry methods of rescue will be used first.

· Attendants are trained to perform non-entry rescue; however, prior to initiating non-entry rescue, the attendant must notify at least two members of the rescue team.

· In no case will a person attempt non-entry or entry rescue without training according to the requirements of this program.

3. Rescue Team Organization

· The site rescue team may be comprised of maintenance, production and site services personnel.

· Rescue Team activities are overseen by the Site Safety COE. · The Rescue Team Roster is posted on the Intranet.

4. Rescue Team Notification

· Members of the rescue team must be notified that a Confined Space entry is taking place before the entry permit is authorized.

· A minimum of two rescue team members must be on site and available for timely response for all Confined Space entries.

· Any member of the rescue team may be contacted.

· Solicitation of rescue team members for planned off-shift entries must be performed during the day-shift prior to the off-shift entry.

· The contacted rescue team member is responsible for advising the Entry Supervisor (Permit Issuer) if more than two members are needed for that entry.

· Requests for additional rescue team members may be made by Operations/Owning or maintenance performing the entry.

· The Entry Supervisor (Permit Issuer) is responsible for contacting all necessary rescue team members to establish a rescue team for that entry.

5. Rescue Team Communication

· A method of summoning rescue (i.e., radios, pagers, etc.) must be established prior to commencement of entry operations and noted in the "Instructions to Summon Rescue" section of the permit.

· The Confined Space entry attendant will contact the rescue team via the established method in the event that a rescue may be necessary. · Drills to practice summoning rescue and evaluate response time shall

be conducted periodically.

6. Rescue Team Equipment

· Rescue equipment is contained in the Rescue Team trailer. · The equipment is maintained by members of the CS

Rescue Team.

· An equipment list is maintained by the Site Safety COE Confined Space Optimization Team.

· Inventory and inspection of rescue equipment is the responsibility of the Rescue Team.

· Annual inspection is to be conducted on all Rescue Equipment.

4.6.

Training

·

Training for Permit Issuers, Entrants, Atmosphere Testers, and Rescue

Services must be consistent with that specified in Sections 4.7.1 and 4.7.2

BC032.017, Confined Space Entry, and the check list contained in

Attachment 1 specifying the Confined Space Training requirements.

·

Frequency Section 4.7.4. · More frequent requirements are specified in other procedures (such as First Aid/CPR – 24 months, Respiratory Protection and Bloodborne Pathogens – 12 months).

Document ation

Section 4.7.5. · Training documents are maintained by the individual's Business Unit with the exception of those exclusive to the Rescue Team (Rescue Training, Drill Records, etc.).

· Rescue Team-specific training records are maintained by the Site Safety COE Confined Space Optimization Team.

Contractor Training

Section 4.7.6. The Permit Issuer:

· Verify contractors have the necessary confined space training prior to issuing permit.

· Ensure the contractor has received proper orientation in accordance with

WYN032.006, Contractor EHS Qualification, Orientation and Oversight.

· Inform contractor that Confined Spaces exist and entrance is only allowed in compliance with BASF Confined Space entry program.

· Inform the contractor of the hazards associated with the Confined Space. · Advise contractor of BASF policies and procedures for the protection of

employees.

· Debrief the contractor of any hazards confronted or created during entry operations at the conclusion of the entry.

· Provide rescue team service when it is not provided by the contractor.

·

4.7.

Management System Review

An annual review of the Confined Space Program shall be conducted to verify

effectiveness of the Confined Space Entry program and practices.

As a minimum, the following elements of the Confined Space Entry management

system shall be included in the review:

·

Review the site procedure and permit to ensure all currently required elements

are included.

·

A review of all incidents and other nonconformance reports involving Confined

Space Entry activities that have occurred since the last Confined Space Entry

management system annual review. The review should verify root causes

have been properly identified and appropriate corrective actions have been

implemented.

·

Inspection of at least one Confined Space Entry job-in-progress to verify site

practices comply with the site procedure. Verification of the training personnel

involved in this job-in-progress have received.

·

Interviews with a representative number of Permit Issuers, persons performing

Confined Space Entry, and other affected employees, including Contractor and

Contracted Employees where applicable, to verify they understand their

role/responsibility.

·

A review of applicable equipment and instrument maintenance and calibration

records.

The review shall be documented. For an example, see the Confined Space

Management Review Checklist in Appendix 3 of BC032.017, Confined Space

Entry.

5.

RESPONSIBILITIES

5.1.

Confined Space Entrants, Attendants, Permit Issuers, Permit Acceptors and

Rescue Personnel

Have responsibility for

·

understanding and adhering to the requirements contained in this and the

5.1.1. Duties of Entrant

·

By way of signature on the entry permit the entrant attests that they are

properly trained and able to recognize the hazards associated with entry,

and understands the consequences of exposure to the hazards.

·

The entrant must maintain contact with the attendant and notify the

attendant if he/she recognizes a warning sign of exposure to hazards,

detects a prohibited condition, or before initiating an evacuation from the

Confined Space.

·

The entrant is trained and understands how to use equipment such as air

monitors, radios, lighting, rescue devices, blowers, personal protective

equipment, barricades, etc.

·

The entrant is trained and knows that they must exit the Confined Space if:

·

the attendant orders an evacuation,

·

an automatic alarm is activated, or

·

the entrant feels they may be in danger.

5.1.2. Duties of Attendant

By way of signature on the entry permit the attendant attests that they are

properly trained so that they:

·

may serve as Confined Space watch while an entry is in progress,

·

devote his/her entire attention to that task,

·

know and recognize the hazards associated with Confined Space

entry,

·

understand the consequences of exposure to the hazards,

·

know how to recognize the symptoms of exposure in others.

·

be in constant communication with the entrants,

·

order an entrant out of the Confined Space if they detect:

·

a prohibited condition or unusual behavior;

·

any condition outside of the Confined Space that could endanger

the entrant;

·

report to their supervisor when they cannot effectively perform their

duties.

·

control access to the space, keep an accurate count of entrants,

and allow only trained entrants to enter the Confined Space,

·

keep unauthorized people away from the space,

·

notify emergency rescue personnel via emergency radio channel 6

before taking any other actions then initiate non-entry rescue

·

do not enter a Confined Space for a rescue unless they are relieved

by another trained attendant.

·

Any person entering a Confined Space for rescue must be trained in

Confined Space entry rescue techniques per the requirements

outlined in Section 4.7.

5.2.

Duties of Entry Supervisors (Permit Issuer)

The Permit Issuer

·

verifies entrants and attendants have signed the entry permit attesting to

proper training, equipment and instruction for safe entry.

·

understands and communicates the hazards of the Confined Space,

·

verifies that required atmospheric monitoring has been conducted and is

within acceptable limits.

·

verifies Lock, Tag and Try procedures have been followed and all hazardous

energy sources addressed.

·

verifies that all necessary equipment is in place, and

·

verifies that Rescue Team members are on-site and notified of entry and a

means for summoning the Rescue Team members via radio or other

arrangements are effective before signing the permit.

·

terminates the entry and ensures the attendant understands that

unauthorized individuals cannot enter the Confined Space, and

·

obtains the necessary exception signatures, as necessary.

·

Notifys Rescue Services that the work is complete and permit terminated.

5.3.

Duties of Permit Acceptor

·

The Permit Acceptor attests that the service person entering the Confined

Space meets all training requirements for Confined Space Entry.

·

Reviews details of each entry with the Permit Issuer to ensure compliance with

applicable Corporate and Site Standards.

·

Signs the Entry Permit certifying achievement of requirements for each entry.

·

Signs for equipment or condition exceptions along with the Process Owner (or

trained staff-level designee) in the appropriate section of the Entry Permit.

5.4.

Duties of Contractors

·

The confined space contractor is responsible for maintaining a confined space

entry program that complies with BASF and MIOSHA requirements.

·

The contractor provides the written program to BASF upon prequalification

request and periodically upon request, thereafter.

·

The contractor attests that their confined space employees comply with the

BASF permit-required Confined Space program and have the training necessary

to comply with MIOSHA and BASF requirements.

·

Obtains the necessary permits (Safework, Hotwork, Confined Space) prior to

conducting work.

·

Obtains necessary information from BASF regarding permit space hazards.

·

Coordinates entry operations with BASF personnel.

·

Informs BASF of any hazards created or observed in a Confined Space.

5.5.

Site General Manager

Responsible for site-wide implementation of this procedure.

5.6.

Responsibility for the initiation, issuance, administration, auditing and

interpretation of this procedure and reviewing/approving contractor company

qualifications and programs in accordance with WYN032.006, Contractor

Selection, Orientation and Oversight.

5.7.

The following equipment is required to be present/utilized for confined space entry:

Fire Extinguisher - This item is a requirement for which an exception is NOT permitted. The fire extinguisher must be placed near the confined space entry port.

Safety Harness and Line - This item is required to be worn by each person who enters the confined space. An exception is permitted only when the confined space design prohibits use of the harness and tag line for rescue.

Continuous Flammable Vapor and O2 Monitor - This item is a requirement for which an exception is not permitted. This monitor must be hung inside the confined space in close proximity to the entrants.

Ladder (Secured in place) - This item must be secured in place if it is needed for entry or exit. This item may not be excepted if it is required for exit or entry.

Lighting - This item may be excepted if lighting is not required for visibility and work will be completed prior to sunset. Lighting will have guards to prevent contact with the bulb.

GFCI (Ground Fault Circuit Interrupter) - Electrical devices taken into a confined space (including lighting) must be protected by a 110 VAC GFCI or have self-contained batteries. The GFCI MUST BE TESTED PRIOR TO CONFINED SPACE ENTRY. The Electrical classification of the area may necessitate more stringent standards.

Two-Way Radio for Attendant - This item may only be excepted if an alternate means of communicate is available which does not require the attendant to leave the space.

Air Mover (Blower) - This item is required to be placed and operating to blow fresh air into the confined space. IT IS UNSAFE AND NOT ACCEPTABLE to only exhaust air from the confined space such that fresh air is drawn in; It is acceptable to utilize local exhaust ventilation at a point source of contamination (e.g. welding) while blowing fresh air in.

Non-Entry Rescue Equipment - This item may NOT be excepted.

Supplied Air Respirator (SAR) - If the Confined Space does not achieve necessary air quality parameters (Toxicity <PEL, LEL=0% and Oxygen = 19.5 -21%) or air quality cannot be guaranteed by means of monitoring, or changing conditions (i.e. painting, welding, etc.) may compromise the integrity of air quality, SARs must be used by the Entrants and be available to the Attendant.

If the hazardous material can be monitored meets air quality criteria (toxicity, LEL and Oxygen) and changing conditions are not

expected, SARs may be excepted by designated Operations/Owning personnel trained and authorized for exception signatures. Excavations:

Excavation SARs may be excepted if the requirements below are also met: 1. One side has a slope of 1:1 or shallower.

2. An air mover of sufficient size to cause noticeable air movement at the bottom of the excavation is used. NOTES:

SARs shall be tested outside the confined space for a minimum of 10 seconds prior to entry in order to assess working condition. In addition, all fit test, medical clearance, training, donning/doffing and respirator inspection provisions of the Unit Respirator Program applies.
