This is paid in addition to any other benefits provided by any other insurance policy, workers compensation or Social Security.

What is Combined?

Good question. Combined Insurance is a

supplemental policy that helps cover the “money gap” left from any other coverage you may have.

Combined pays cash DIRECTLY TO YOU or to whomever you choose, and PAID IN ADDITION to benefits provided by any other insurance policy, workers compensation, or social security.

This is money you can use however you see fit. Groceries. Car payment. Mortgage or rent. School. You get the picture. Health insurance may pay the doctor, but it does nothing towards your other bills.

HOW DOES IT WORK?

Combined has two kinds of coverage, Accident and Sickness. Each is separate coverage, but both work in a similar fashion. Just like the name says, one pays benefits for loss from accident, the other from sickness.

IS COMBINED AFFORDABLE?

Coverage for both plans comes in two levels, half-unit and full-unit. Basically if you want to save a little money you can by half the benefit for half the cost. This means you can have Accident coverage for as low as $3 a week.

Pricing for the Sickness coverage varies by age, but is also available in half and full levels.

WHAT ABOUT GOLD SHIELD?

Combined works in addition to Gold Shield, and together that’s a strong 1-2 punch.

WE DO COVER YOU BY:

• Being struck, knocked down, or run

over.

• Driving or riding in ANY automobile, bus, taxicab, truck, or any farm machinery!

• We EVEN cover you on a motorcycle, bicycle, in a boat or ANY

recreational vehicle. • Flying or riding in ANY aircraft

(private or commercial).

• At the hands of ANY burglar, robber, or by personal assault.

• We cover you by drowning. • Fire or smoke inhalation. • Hunting, fishing, or camping, while

participating in ANY sporting event or recreational activity.

• In other words, regardless of your trade or profession, we cover you if you are hurt

AT HOME, AT WORK, AT PLAY OR ANY ACTIVITY WHATSOEVER

24 HOURS A DAY... 7 DAYS A WEEK... 365 DAYS A YEAR.

HALF UNIT

Should any accident require CONFINEMENT IN A HOSPITAL overnight as an inpatient, we

pay beginning with the very first day. Or if you wish to, you can do as most people

do and take the Full Unit and receive a wonderful income month after month after

month... even for the rest of your life while confined in a hospital.

Should you be confined in an INTENSIVE CARE UNIT one day or more from any

accident, we will pay an additional lump sum benefit.

If, because of any accident, you require EMERGENCY TREATMENT at a hospital emergency room within 48 hours of the

accident, we pay a benefit of...

In addition to all of this, for minor injuries such as cuts, burns, or bruises caused by any accident, we will even pay an income benefit for TREATMENT BY A PHYSICIAN at a hospital, doctor’s office, clinic or elsewhere.

$50.00 PER ACCIDENT UP TO $750.00 A MONTH (6 MONTHS MAXIMUM)

NOW, HERE IS SOMETHING THAT YOU WILL FIND IN VERY FEW POLICIES AVAILABLE TODAY.

FOLLOWING HOSPITAL CONFINEMENT overnight as an inpatient, OUTPATIENT SURGERY, or treatment for FRACTURES, there is usually a recovery period. Should that happen, we will continue to pay you an income at home, while you are totally disabled, for up to six (6) consecutive

months. UP TO $1,500.00 A MONTH (6 MONTHS MAXIMUM)

THIS RECOVERY BENEFIT IS PAYABLE TO YOU WHILE YOU’RE RECOVERING AT HOME OR ANYWHERE!

In order to help protect against higher costs, both premiums and benefits

increase 5% each year for the first ten years your policy is in force, up to a maximum increase of 50% from the original amounts .

This policy is absolutely

NON-CANCELLABLE, GUARANTEED RENEWABLE for the REST OF YOUR LIFE...

That means your right to renew this policy is guaranteed for your lifetime as long as the scheduled premium is paid.

We CANNOT CHANGE THE PREMIUMS SHOWN IN THE SCHEDULE OF,PREMIUMS AND BENEFITS and WE CANNOT CANCEL YOUR POLICY regardless of your age,

health condition, or the number of claims you file .

ONLY ONE BENEFIT AMOUNT, THE HIGHEST, WILL BE PAID FOR MULTIPLE INJURIES RESULTING FROM ONE ACCIDENT AND LOSS MUST OCCUR WITHIN 180 DAYS FROM THE DATE OF THE ACCIDENT THAT CAUSED THE INJURY.

Of course, PRE-EXISTING CONDITIONS ARE NOT COVERED DURING THE FIRST YEAR YOU HAVE THIS POLICY. “Pre-existing condition” means a bodily illness or disease which was diagnosed or treated by a physician within the 12 months preceding the issue date of the policy.

However, we do cover you for ANY OTHER SICKNESS starting immediately. Take for example: Appendicitis, Ulcers, Gall Bladder, Respiratory, Kidney Problems, and Pneumonia or more serious conditions such as Cancer, Stroke, or Heart Trouble.

HOSPITAL CONFINEMENT INCOME BENEFIT

$50.00 A DAY $1,500.00 A MONTH

Should any covered sickness require confinement in a hospital overnight as an inpatient, we pay $50.00 a day starting with

the very first day, or $1,500.00 a month. Or if you wish to, you can do as most people

do and take the Full Unit and receive a wonderful income of $100.00 a day while hospitalized. In other words, up to $3,000.00 a month... month after month after month… even for the rest of your life while confined in

a hospital. $100.00 A DAY $3,000.00 A MONTH $100.00 A DAY

DAILY INTENSIVE CARE INCOME BENEFIT

In the event of a serious sickness it is sometimes necessary to be confined in an

intensive care unit. Should this occur, for each day you are confined in an intensive care unit, we will pay an additional benefit of $100.00 a day on the Half Unit; $200.00 a

day on the Full Unit.

is usually a recovery period. If, because of covered sickness, you are continuously

totally disabled beginning within 30 days following a period of confinement in a hospital for which benefits are payable under Section B for such sickness, or outpatient surgery; we will pay you, while

you remain totally disabled for up to a maximum of 6 consecutive months.
