Our Heritage

Academy’s service experience history started with the Rose Family

getting into the fire protection industry in the early 1930’s with the

acquisition of the S&I Plumbing and fire protection Service Company.

The company’s founder Anthony Rose (Grandpa) passed the company

to his son Michael Rose Sr. Dad operated the business from the early

60’s until his retirement in the late 90’s. He then passed the torch to

his only son Michael Rose Jr., who now runs the company and has

focused on global services. The acting CEO is anxiously waiting the

time when he can work his now five-year old son, Michael Rose the

third, to strategize about the company’s future.

Services We Perform

Academy’s national service program consists of providing service and maintenance necessary for your facilities. AFP will help you stay in compliance as well as reduce liability. For a complete list of service coverage, please see below:

Service Program

Standard Service

Service requests are received by our service department via an email, fax or phone. Our Customer Service Representative enters the request into our system and generates a work order. A Service Coordinator then calls your location to verify the scope of work and to schedule the service with a location manager. A technician is then sent to the job site to perform the repair or service within one to three days of the original request. In the event that this service exceeds the dollar amount allotted to us by your company, a customer service representative will contact your corporate office to explain the scope of work using our quoting template and to ask for a more adequate Do Not Exceed (DNE). Once the quote has been approved, our technician will complete the work and have the Manager sign and stamp our work order.

Violations

If a location was visited by a Fire Marshall or received a citation, it is important for our customer to fax us any documentation relevant to that visit. Our service coordinator will contact the Fire Marshall or Authority Having Jurisdiction with a date of proposed abatement.

Emergency Service

Emergency service calls are responded to the same day. Once an emergency service call is entered into our system, our service coordinator will contact the location to verify the urgency of the call and dispatch a technician to the location. If upon contacting the location manger the call is not an emergency the service request will be downgraded and we will contact your corporate office.

After-Hours Service

Academy Fire Protection is available 24 hours a day 7 days a week. Call 1-800-773-4736 and select Option 1. Our regular working hours are between 8 a.m. and 5 p.m. EST.

Estimating Job Costs

Estimates are provided when the work needed goes outside the original scope of work or the Do Not Exceed (DNE) price.

Construction

We resolve your fire alarm needs quickly, efficiently, and in a cost-effective manner.

Emergency Lighting

We can inspect and service the exit emergency lighting and exit panic alarm devices at each location independent of, or bundled with, the annual fire extinguisher maintenance program.

Fire Alarms

From a routine fire alarm inspection to large scale installation projects, Academy is the company you can count on to keep your fire alarm equipment maintained and within code. Our team of project managers and fire alarm specialists are well versed in NFPA codes and local AHJ requirements throughout the country.

Fire Sprinklers

Our licensed and insured team of professionals will provide a full servicing of all sprinkler needs you may have to ensure that your sprinkler system is working properly and effectively according to local and national code.

Academy Fire Protection currently manages over 100,000 preventive maintenance services, in accordance with NFPA and local codes. We maintain the following types of fire protection equipment.

Preventive Maintenance is extremely important in maintaining the integrity of your fire protection equipment, satisfying local and national codes and extending the longevity of your fire protection equipment. Academy Fire’s maintenance program has been successful in assisting our customers reduce their expenses on repairs. Whether it is a smoke detector or a fire hydrant, these devices are responsible for protecting your property, customers, employees and product. Fire Protection equipment requires routine cleaning, testing, lubrication and inspection in order to assure that they operate as expected in a fire emergency.

When signing with Academy Fire, we will assist you in coordinating the best possible schedule to meet your fire safety and budgetary needs. Many of Academy’s customers consolidate their inspections to specific months. For example, April is extinguisher month, October is alarm and sprinkler month. This enables the site managers to prepare for the inspection, decrease the risk of violations and managing the schedule is simplified and efficient. The consolidation of PM schedules enables easier forecasting and expense management.

As a customer of Academy Fire Protection, we offer a simple (10) steps set up your locations under our PM Programs.

Step 1

 Provide Academy Fire with an updated store list in excel format. Our Database Administrators upload your information into our database.

Step 2

 AFP confirms the number of locations to be added to the program, specify PM procedures which include: not to exceed, purchase order procedures, close out and IVR procedures, billing guidelines and special reporting arrangements.

Step 3

 PM schedules for each location and each fire trade are uploaded into our database in addition to the PM process for your locations.

Step 4

 A notification letter is sent to the locations by your office authorizing Academy Fire to gain access to your locations.

Step 5

 An authorization letter is sent by your company to Academy Fire for our technicians to carry as a proof of legitimacy and authorization to perform the service.

Step 6

 Academy performs its first roll out; work orders for the scheduled locations are generated. A list of these locations are sent to your office to verify that they are still open for business.

Step 7

 An instruction guide on your account is created for our administrative staff and field technicians. This guide explains the full scope of work and specific instructions pertaining to your account. The guidelines are distributed to the administrative staff and field technicians to sign and return to our office.

Step 8

 Work orders are dispatched to the technicians in the field 14 days before the month due. This allows Academy Fire to route the technicians for the upcoming month.

Step 9

 Technicians complete the scope of work. Deficiencies found at the location are called into our Call Center and quoted to you as necessary. Upon completion of the work, the technician submits the completed paperwork to our office for

processing. All paperwork is reviewed to ensure procedures were followed and the service was completed.

Step 10

 Academy Fire submits the signed work order and invoice for payment.

Academy Fire’s PM Program is second to none and allows our customers to focus on business, profits, employees and customers without the distraction of managing their fire protection equipment.

Location Compliance

Inspect Exit And Emergency Lighting Fixtures

The General Manager is required to ensure an inspection is conducted at least monthly of all exit and emergency lights and a 30-second functional test be conducted on all battery powered fixtures. If deficiencies are noted, immediate corrective action must be taken. Personnel making inspections must keep records of all units

inspected, including those found to require corrective action.

Academy Fire Performs…

 The 90-Minute Full Functional Test

 Disconnecting AC power supply to each emergency light

 The cleaning of each emergency light and lens for visibility

 Alignment correction, if needed

 Check batter fixture for sulfation

 Maintain log and place testing label

Your Personnel Performs…

The 30- Second Functional Test. Test the integrity of the lamp

and battery through test button for 30 seconds. Check each light for physical damage. Report any deficiencies.

Please note all deficiencies on your safety checklist and fax it to the Facilities Manager. Keep the form accessible to all managers in the event of a site inspection from any of the Local Authorities.

Inspect Fire Extinguishers

Fire extinguishers require inspection on a monthly basis according to OSHA and state safety regulations. The following is a list of items to check when performing a

monthly inspection:

Extinguisher should be in the designated location (mounted and fixed on a bracket)

Easily accessible (keep all items away from extinguisher)

Pin is in place and tab is not broken

The charge indicator, if present, should be in the “Full” range (Green area)

No indication of physical damage or tempering

Inspection is documented – initial the back of the inspection tag

Summary of Agreements

This “Summary of Agreements” highlights some key portions of the NATIONAL ACCOUNT SERVICE AGREEMENT. This standalone does not bind nor detail the full perspective of the agreement of service between Academy and the “subscriber”. Any further disclosures of service are referenced in the NATIONAL ACCOUNT SERVICE AGREEMENT.

In order to better service your needs- Academy encourages effective and thorough communication with all subscribers of our service. If there is a requirement for the addition of subtraction of service location, please contact us at 800-773-4736.

Automatic Inspections

Upon completion of the initial inspection, AFP will automatically set up the next inspection per the required frequency of the State. A list of locations due for service will automatically be emailed to the subscriber 30 days prior to the month of due for inspections. At this time it is the subscriber’s

responsibility to reply via email of any locations closed or schedule changes within eleven days of AFP’s email date.

Servicing Pre-Authorization

The pre-authorization of work is permitted under the standard conditions set forth by the DO NOT EXCEED of the specific service costs for standard and emergency conditions. For a detailed scale of pricing, please refer to the NATIONAL ACCOUNT SERVICE AGREEMENT.

Standard Service provided during the normal working hours (Mon.-Fri. a.m. EST to p.m. EST, excluding holidays).

Emergency Service available all week, including weekends, 24 hours 7 days.

Service Accessibility

If devices are out of reasonable reach (in excess of 10 feet) subscriber will need to provide a safe access to equipment for our technicians (scaffolding, mechanical lift, ladders, etc.) If requested by the customer, AFP can provide such equipment at an additional charge. (See attached unit pricing for lift charges). Unreasonable delays and unintended visits cause a disturbance in the scheduled work flow of a technician and our business. Please be sure the location is prepped for the visit by our

technician before we dispatch him/her to the location.

BILLING & PAYMENT PROCEDURES

Subscriber will be invoiced as follows:

Weekly / Monthly consolidated invoices which are to indicate all service related calls on one and all “PM” or inspections performed on the other. Both are sent to the maintenance coordinator responsible for the corresponding cost of service. We will also safe keep all backup paperwork pertaining to the service or inspection in-house. Subscriber may request a copy of such backup at anytime. If Subscriber requires a copy of the backup paperwork with each invoice please feel free to call our main hotline.

We simplify the billing process for the Subscriber. AFP’s Central Billing Office utilizes a “Billing Rider” with consolidated monthly billing. The appropriate reports will be attached as backup and will be included as part of the consolidated invoice. A copy of the inspection reports shall be left with the location manager. The AFP’s Central Billing Office will accumulate all Subscribers’ billings for the month and submit to

the Subscriber one consolidated invoice with a listing of the Locations inspected. The list will be accompanied by all inspection reports for all Locations listed in the billing. Inspections invoiced to the Subscriber shall be forwarded by the Central Billing Office directly to the Subscriber’s listed payment address. The Subscriber will remit one payment to AFP within 30 days and AFP’s Central Billing Office will distribute such payment to the proper AFP office. Additional changes may apply if we are required to invoice on 3rd party programs.

Acceptable Methods of Payment

Academy Fire Protection accepts all major Credit Cards, Corporate Checks and PayPal. Enroll in our services and we accept deposits through an Automated Clearing House (ACH) Direct process to monthly deduct a set amount from the total invoice.

Payment Terms

The Subscriber will, in agreement with in our terms, remit payment within thirty (30) days after receipt of an itemized invoice; with exceptions for any amounts in dispute by Subscriber. Subscriber will pay any such disputed amount within 30 days after resolution of dispute. Payment shall include but not be limited to State and Local taxes, excise, use and sales, permits, however designated, levied or base the service charges pursuant to our Agreement. Charges outside the scope of the Agreement shall be billed at the applicable Contract rates for labor, materials, and travel provided Subscriber has approved in writing in advance such additional Service and the contract of such Services. If Subscriber fails to pay any amount due, AFP, may at its option and sole discretion, terminate by providing the Subscriber (30) thirty days prior written notice of termination.

NFPA SERVICE REQUIREMENTS

System Inspection 4-1.1 At least annually, all system shall be

thoroughly inspected and tested for proper operation by competent personnel. Discharge tests are not required.

Weight Cylinders 4-1.3 At least semi-annually, the agent quantity

and pressure of refillable containers shall be checked.

Container Test 4-2.1 U.S. Department of Transportation (DOT),

Canadian Transport Commission (CTC), or similar design clean agent containers shall not be recharged without retesting if more than five years have elapsed since the date of the last test and inspection.

Hose Test 4-3.2.1 All hose shall be tested every five years.

Room Integrity 4-4 At least every 12 months, the enclosure

protected by the clean agent shall be thoroughly inspected to determine if penetrations or other changes have occurred that could adversely affect agent leakage or change volume of hazard or both.

Carbon Dioxide

Systems NFPA 12

System Inspection 1-11.1 At least every 30 days, an inspection shall

be conducted to assess the system’s operational condition.

Fire protection equipment requires care and maintenance at scheduled intervals to assure it will operate properly and safely when needed. Only personnel who are trained and qualified to maintain the equipment described below should do these services.

The following outlines the service requirements of the equipment described in this catalog. These requirements are minimum as contained in the National Fire Codes ® published by the National Fire Protection Association. System Test 1-11.3.2 The following shall be verified by

competent personnel at least annually using available documentation required in 1-7.2.6

Weigh Cylinders 1-11.3.5 At least semiannually, all high-pressure

cylinders shall be weighed and the date of the last hydrostatic test noted (see 1-9.5.1)

Container Test 1-9.5.1 High-pressure cylinders used in

fire-extinguishing systems shall not be recharged without a hydrostatic test (and remarking) if more than 5 years have elapsed from the date of the last test. Cylinders continuously in service without discharging shall be permitted to be retained in service for a maximum of 12 years from the date of the last hydrostatic test. At the end of 12 years, they shall be discharged and retested before being returned to service.

Hose Test 1-11.2.1 All system hose including those used as

flexible connectors shall be tested every 5 years in accordance with 1-11.2.

Halon 1301 Systems NFPA 12A

System Inspection 4-1.1 At least semiannually all systems shall

be thoroughly inspected, tested, and documented for proper operation by trained competent personnel. Tests shall be in accordance with the appropriate NFPA or Canadian standards.

Container Test 4-2.1 DOT, CTC, or similar design Halon 1301

cylinders shall not be recharged without a retest if more than five years have elapsed since the date of the last test and inspection.

Hose Inspections 4-3 All hoses shall be examined annually

for damage

Hose Test 4-3.2 All hoses shall be tested every 5 years in

accordance with 4-3.1.

Agent Examination 9-3.1d. Examination of the dry chemical (if there

is evidence of caking, the dry chemical shall be discarded and the system shall be recharged in accordance with the manufacturer’s instructions).

7-3.2 Testing shall be performed in accordance with the schedules in Chapter 7 or more often if required by the authority having jurisdiction. If automatic testing is performed at least weekly by a remotely monitored fire alarm control unit specifically listed for the application, the manual testing frequency shall be permitted to be extended to annual. Table 7-3.2 shall apply.

Fire Extinguisher NFPA 10

Inspection 4-3.1 Fire extinguisher shall be inspected when

initially placed in service and thereafter at approximately 30 day intervals.

Maintenance 4-4.1 Fire extinguishers shall be subjected to

maintenance at intervals of not more than 1 year, at the time of hydrostatic test, or when specifically indicated by an inspection.

Hose Conductivity Testing

4-4.1.2 A conductivity test shall be conducted annually on all carbon dioxide hose assembles.

Fire Hose Inspection NFPA 1962

Hose 2-4.1 The hose shall be service-tested as

specified in Section 5-4 within 90 days prior to being placed in service for the first time and at least annually.

Couplings 4-2.1 Couplings shall be kept in serviceable

condition. After each use, and during each hose service test, they shall be

visually inspected.

Gaskets and Seals 4-3.1 The thread gasket in couplings and

nozzles shall be inspected for presence, tight fit, and lack of deterioration. If defective, it shall be replaced with a new gasket.

Nozzles 4-1.2 All nozzles shall be inspected after each

use and at least annually.

Re-Rack Hose 2-3.2 In-service hose shall be unracked,

unreeled, or unrolled and physically inspected as specified in Section 2-5 at least annually. Hose shall be racked, re-reeled, or re-rolled so that any folds do not occur at the same position on the hose.

Sprinkler Systems Inspections

NFPA 25 Gauges - Dry,

Pre-Action and Deluge

2-2.4.2 Gauges on dry pre-action, and deluge systems shall be inspected weekly to ensure that normal air and water pressures are being maintained. Gauges - West Pipe

Systems

2-2.4.1 Gauges on wet pipe sprinkler systems shall be inspected monthly to ensure that they are in good condition and that normal water supply pressure is being maintained.

Control Valves (See “Valves” below)

Alarm Devices 2-2.6 Alarm devices shall be inspected

quarterly to verify that they are free of physical damage.

Hydraulic Nameplate 2-2.7 The hydraulic nameplate, if provided, shall

be inspected quarterly to verify that it is attached securely to the sprinkler riser and is legible.

Buildings 2-2.5 Annually prior to the onset of freezing

weather, buildings with wet pipe systems shall be inspected to verify that window, skylights, doors, ventilators, other openings, and closures, blind spaced, unused attics, stair towers, rood houses, and low spaces under building do not expose water-filled sprinkler piping to freezing and to verify that adequate heat (minimum 40˚F (4.4˚C)) is available.

Hangers/Bracing 2-2.3 Sprinkler pipe hangers and seismic

braces shall be inspected annually from the floor level.

Pipe & Fittings 2-2.2 Sprinkler pipe and fittings shall be

inspected annually from the floor level.

Sprinkler Heads 2-2.1.1 Sprinklers shall be inspected from the

floor level annually.

Spare Sprinkler Heads 2-2.1.3 The supply of spare sprinkler shall be

inspected annually.

Alarm Devices 2-3.2 Water flow alarm devices including, but

not limited to, mechanical water motor gongs, vane-type water flow devices and pressure switches that provide audible or visual signals shall be tested quarterly.

Main Drain (See “Valves” below)

Antifreeze Solution 2-3.4 The freezing point of solutions in

antifreeze shall be tested annually by measuring the specific gravity with a hydrometer or refractometer and adjusting the solutions if necessary.

Gauges 2-3.2 Gauges shall be replaced every 5 years or

tested every 5 years by comparison with a calibrated gauge. Gauges not accurate to within 3 percent of the full scale shall be recalibrated or replaced.

Sprinkler Heads 2-3.1.1 Where sprinklers have been in service

for 50 years, they shall be replaced or representative samples from one or more sample areas shall be submitted to a recognized testing laboratory acceptable to the authority having jurisdiction for filed service testing. Test procedure shall be repeated at 10-year intervals.

Sprinkler Heads Extra High Temp

2-3.1.1 Exception No. 3: Representative samples of solder type sprinklers with a temperature classification of extra high (325 º F (163 º C)) or greater that are exposed to semi-continuous to continuous maximum allowable ambient temperature conditions shall be tested at 5-year intervals.

Sprinkler Heads - Fast Response

2-3.1.1 Exception No. 2: Sprinkler manufactured using fast response elements that have not been in service for 20 years shall be tested. They shall be retested at 10-year intervals.

Obstruction Investigation

10-2.2 Systems shall be examined internally for obstructions where conditions exist that could cause obstructed piping. If the condition has not been corrected or the condition is one that could result in obstruction of piping despite any previous flushing procedures that have been performed, the system shall be examined internally for obstructions every 5-years.

Valves NFPA 25

Control Valves 9-3.3.1 All valves shall be inspected weekly

Alarm Valves 9-4.1.1 Alarm valves shall be externally inspected

monthly.

Check Valves 9-4.2.1 Valves shall be inspected internally every

5 years to verify that components operate properly, move freely, and are in good condition.

Pre-action/Deluge Valve 9-4.3.1.3 The interior of the pre-action or deluge

valve and the condition of detection devices shall be inspected annually when the trip test is conducted.

Dry Pip Valves/Quick Opening Devices

9-4.3.1.4 The interior of the dry pipe valve shall be inspected annually when the trip test is conducted.

Backflow Prevention Assemblies

9-6.1.2 All backflow preventers installed in fire protection system piping shall be tested annually.

Testing 9-2.6 A main drain test shall be conducted

quarterly at each water based fire protection system riser to determine whether there has been a change in the condition of the water supply piping and control valves.

Main Drain

Water Flow Alarm 9-2.7 All water flow alarms shall be tested

quarterly in accordance with the manufacturer’s instructions.

Control Valves 9-3.4.1 Each control valve shall be operated

annually through its full range and returned to its normal position. Pre-action/Deluge

Testing 7-9.3 A functional test shall be conducted on

every required emergency lighting system at 30-day intervals for not less than 30 seconds.

Annual Test 7-9.3 An annual test shall be conducted

on every required battery-powered emergency lighting system for not less than 1 ½ hours. Equipment shall be fully operational for the duration of the test.

Exit Signs/Lighting NFPA 101

Visual Inspection 7-10.9.1 Exit signs shall be visually inspected for

operation of the illumination sources at intervals not to exceed 30 days.

Annual Test 7-10.9.2 Exit signs connected to or provided with

a battery-operated emergency illumination source, where required in 1.10.4, shall be tested and maintained in accordance with 7.9.3.

 By selecting the W.O.# particular information relating to that store; Store Address, Billing Address, Store Contact information as well as the DNE and salesperson.

 The Invoice tab shows items by description & price, and any credits or payment made for this particular location.

3. Information

 As described above, the Info tab shows the client-side information related to the work.

Work Orders

The Work Order tab shows all WEB PORTAL – CUSTOMER INFORMATION you may choose to filter by PM or Service. You may choose to narrow your search further by using the Select Status Drop Down Box

Invoices

From the main page, clicking on the Invoices tab displays all invoices sent to the designated Biller. This is sorted by the latest bill date.

Clicking on the Invoice # displays the individual invoice with a breakdown of the items.
