Getting to know your Simply Cash Plan

Your Simply Cash Plan is there to give you money back towards your everyday healthcare costs, whether you use the NHS or have your treatment privately.

It is there for you to use whenever you need it. Details of your benefits and the amounts you can claim each claiming year are shown in your table of cover which can be found in the welcome pack sent to you when you first took out your plan.

This guide gives you all the information you need to get started with your plan including a three step guide and details of what you can claim for. If you have any questions, please do call us on 0800 980 7890 weekdays 9am to 8pm and Saturdays 9am to 5pm.

A cash plan is:

✔ Great for people who want to stay on top of their health

✔ Useful for both day-to- day healthcare costs and unexpected costs

✔ A plan you don’t have to be ill to benefit from

✔ Suitable for private and NHS patients

✔ Easy to use with one simple claim form

A cash plan isn’t:

✗ An expensive health insurance policy

✗ A plan you might never use

✗ A plan you’ll only need in the event of serious illness

✗ Only for people who don’t have private health insurance

Your benefits

To find details of how much you can claim each year, please see your table of cover in your Policy Document or log on to the self service area of our webiste. If you can’t find the brochure we sent to you when you took out your cash plan then don’t worry just contact us and we’ll be happy to send you another copy.

For each benefit you have an allocated ‘pot of money’ you can claim back depending on the cover taken. This can help you to spread the cost of your everyday healthcare appointments:

Dental

Check-ups and treatment like fillings, dentures, bridges, crowns and hygienist fees Optical

Sight tests, prescription glasses, contact lenses, frames or repairs. We even cover prescription sunglasses or prescription goggles Complementary therapies Such as physiotherapy, osteopathy, chiropractic treatment, acupuncture and homeopathy without waiting for a GP referral

Consultations

With a medically qualified specialist or surgeon Unexpected costs Money back to cover the inconvenience of staying in hospital for up to 20 days/

nights a year. Please note that pre-existing conditions are not covered for the first 12 months from your policy start date 24 hour helpline

Did you know?

You can log on to the self service area of our website to view details of your plan. All you need is your policy number, which is on your summary of cover, date of birth and postcode, then register your details online and you’re ready to manage your plan.

How do I make a claim?

We’ve made it so simple to claim, all you need to do is ask for a receipt when you pay for your treatment, then send it to us along with a completed claim form.

1. Go to your appointment and pay your bill as normal at the treatment provider - make sure you ask for the receipt

2. Fill in the details on the front of your claim form. We sent you a claim form when you first took out your plan but if you haven’t got one, please call us on 0800 980 7890 and we’ll be happy to send one to you. Send the claim form and the receipt back to Simplyhealth within six months of the treatment

3. We aim to send you the money back within a few days Your claim form is personalised

with your details which is why we need to post it to you. When you submit a claim, we’ll send you a new claim form each time.

You’ll receive money back in your bank account usually within a few days

3 How do I make a claim if I stayed in hospital overnight?

If you stayed overnight in hospital then please check your policy document to see if we’ll cover your stay. Please note that pre-existing conditions are not covered for the first 12 months from your policy start date.

2. Send us a copy of your hospital discharge letter (on the hospital’s headed paper) along with your claim form 3. Sign and date the

declaration on the front of the form and send it to us within six months of discharge

How to make a claim for a new child

You can make a claim for a new child providing you have had your cash plan for 12 months at the date of birth or adoption. Just fill in the new child payment section on the back of the claim form and send it to us with your child’s birth certificate within six months of giving birth.

Receipt requirements

Always get a receipt after your check-up or treatment which confirms payment. Simplyhealth will only pay claims for treatment provided by healthcare professionals who are members of registered organisations and recognised by Simplyhealth. Please see our Terms and Conditions to find out which registered organisations are recognised by Simplyhealth.

The receipt should be an original on official headed paper and show the following:

• the full name, address and qualifications of the person providing treatment

• The patient’s full name

• A full description of the treatment given including dates and amounts paid

• If the receipt is for more than one person, then a breakdown of the details of treatments and cost for each person must be shown on the receipt

Glossary and key terms explained

Claiming year The claiming year is the period of time during which you can claim benefits for your chosen level of cover.

Your first claiming year starts on the date you registered with us and runs for 12 months. Subsequent claiming years start on the anniversary of your registration date.

Registration date The date the policy begins, which you can find in your welcome letter which we sent to you when you first took out the plan.

Table of cover A table issued by us giving the levels of cover that apply to each of the premium levels of the policy.

The table of cover forms part of the policy document.

Policy document This policy document and the table of cover, which consists of the terms and conditions that relate to the policy.

Date of treatment The date the treatment was supplied, or the date when you were discharged from hospital.
