Closing date for subscriptions: Friday 12 June, 2015

To purchase units you will be required to complete this Subscription Form.

First Citizens Brokerage and Advisory Services Limited can facilitate the opening of a Brokerage account

to enable you to participate in the IPO. In addition to the completed Subscription Form, please provide the

following to complete the brokerage account opening and application process at First Citizens Brokerage

and Advisory Services:

a.

Proof of address (utility bill, not more than three months old).

b.

Proof of income (payslip/job letter not more than 1 month old).

If retired, a recent pension/bank statement can be used.

c.

Two forms of valid national identification (ID card, drivers permit or passport).

d.

Proof of chequing/savings account number (required as refunds and future dividend payments will be made

via direct deposit to your bank account).

e.

Full payment by cheque payable to “FCBAS-Stallion IPO” for the number of units being subscribed.

f.

Proof of Brokerage account number (if your brokerage account is held with a broker other than FCBAS).

submitting a Unit Subscription Form shall not be binding on me/us if I/we provide written notice to the Lead Broker, within two business days after submission of this application that I/we intend to withdraw my /our application.
