Create an Account
Please visit OralDNA.com/Setup to create your account with OralDNA Labs. This form will allow you to create your username and password and designate contact personnel. To complete this form you will need the following information:
1. Information about your practice (name, address, phone number, and e-mail address)
2. How you would like to be billed (account information for secure on-line credit card or bank ACH transactions) and who will be the billing contact
3. The National Provider Identifier (NPI) number and contact information for the person in your office who will be ordering tests. NOTE: Canadian offices will not have an NPI number, instead the Registration number will be required.
4. Upon completion of your account set-up, your account will be activated in 2 hours or less (if your account is created after business hours or during the weekend, your account will be activated the following business day).
Order Saliva Collection Supplies (Part Number BM-000613)
1. From OralDNA.com, select: “Login” and enter your username and password
2. Select: “Order Collection Supplies” and indicate the quantity (each) desired, and submit. For special or expedited shipping requests, please enter the notes in the text box. NOTE: Requests for overnight shipments cannot be fulfilled for Canadian offices.

Collect a Specimen
A single sample may be used to order one or all tests; in any test combination for MyPerioPath®, MyPerioID®, OraRisk® HPV and DNA DrugMap™ testing.
1. Open Saliva Collection Supplies box. Visually inspect entire contents. Proceed with use only if saline solution is sealed, clear, & colorless. Set corrugated box, specimen bag, barcode labels and FedEx Clinical Pak shipping envelope aside for later use.
2. Ask patient to SWISH and GARGLE with the saline solution for 30 seconds and EXPECTORATE into the funneled collection tube. NOTE: Specimen should be collected prior to cleaning (e.g. debridement or rinsing with antimicrobials). Probing and other evaluations are OK.
NOTE: When ordering MyPerioPath it is recommended that the patient has NOT been on systemic antibiotic in the last 6-8 weeks as this could reduce the bacterial load

3. Remove (and discard) the funnel, then seal collection tube with provided red cap.
4. Print Patient Name & DOB on the provided barcode labels. Affix one label lengthwise onto provided collection tube. The second label is provided for office use.
5. Place sealed collection tube into the plastic zip seal specimen bag with absorbent pad. Seal the specimen bag and place it into the corrugated box. Store until ready to ship.

(Select the green arrows: “Prepare to Submit”, “Next”, “Submit Orders”)
Once you order is submitted, it is placed in the Ship Queue. When you are ready to ship your sample(s), go to the menu bar and hover over “Orders,” then select “Ship”.
Check mark each patient sample you are ready to ship, then select: “Prepare to Ship,” then “Create Shipment”.

Print FedEx Shipping Label and Shipping Manifest and schedule FedEx pickup (If asked if you would like to Open or Save the file, select: Open.)

Package and Ship Specimens
1. Place specimen box(es), folded shipping manifest and test consent forms (if required by local regulatory guidelines) into the FedEx Clinical Pak shipping envelope. NOTE: The State of New York and Canada are currently requiring the consent form to be included in the shipment to OralDNA Labs. It is every practice’s duty in any U.S. state or Canadian province to secure a signed consent form for the genetic testing of a patient and to maintain a signed consent form on file.
2. Attach the Shipping Label and seal the FedEx Clinical Pak.
a. OralDNA Labs must receive specimens within 14 days of collection.
b. Whenever possible, batch samples for shipment. Up to 6 boxed samples can be combined into the same FedEx Clinical Pak shipping envelope.
c. To ensure safe transport to OralDNA in addition to both HIPAA compliance and biological specimen transport regulations, verify the specimen has been placed in the specimen bag and corrugated box, the outside of the box does not display any patient information, and the shipping manifest and test consent forms are folded inward to protect patient privacy and the FedEx Clinical Pak is sealed.
3. Schedule a FedEx Pickup by phone or online
a. Do NOT place FedEx Clinical Paks in FedEx drop boxes. You may drop off your OralDNA shipment at a physical FedEx location.
b. To schedule a FedEx pickup, call: 1.800.GO.FEDEX (463.3339). NOTE: See Page 9 of the Quick Start Guide for instructions on how to schedule FedEx pickup via your OralDNA account.

Select: “Schedule Pickup/View Calendar”
Click the “Schedule Pickup” link for the day you require FedEx to pick up your shipment. Select the appropriate times (24Hr) for the “Shipment Ready” and “Business Close”. FedEx Express allows pickup scheduling up to 2 business days in advance.
