ABOUT THE KTC SCHOOL OF

PRACTICAL NURSING

The KTC School of Practical Nursing operates within the philosophical framework of Kiamichi Technology Center. The faculty of the KTC School of Practical Nursing embraces the mission, values, and philosophy of Kiamichi Technology Center. The school prepares an individual to practice as a Licensed Practical Nurse. The purpose of Kiamichi Technology Center’s School of Practical Nursing is to educate men and women to function as competent nurses qualified to apply to take the NCLEX-PN for licensure as a Licensed Practical Nurse (LPN). Learning opportunities are provided for each student to develop basic knowledge and skills as a practitioner of practical nursing. The faculty of the School of Practical Nursing incorporates the concepts of holistic health, nursing and education into their philosophy.

The KTC School of Practical Nursing was initially established/ approved by the Oklahoma Board of Nursing (OBN) and the Oklahoma Department of Vocational Technical Education, now known as the Oklahoma Department of Career and Technology Education (ODCTE/CareerTech), in 1970.

The first KTC campus to admit classes in 1970 was McAlester where faculty and curriculum were transferred from the hospital-based St. Mary’s School of Practical Nursing. Approval was granted by the OBN and ODCTE for each Practical Nursing program as follows:

Currently 10 program sites operate as one program under the direction of one Director of Practical Nursing and one Assistant Director of Practical Nursing. Two faculty members are responsible for instruction and supervision of 20 students at the McAlester, Poteau, Durant, Hugo and Idabel campuses. One faculty member is responsible for instruction and supervision of 10 students at Antlers, Atoka, Durant, Stigler and Talihina. The maximum faculty to student ratio is 1:10. When combined, the 15 faculty members and 150 student nurses comprise one of the largest practical nursing programs in Oklahoma. Thousands of Practical Nurses have graduated since the inception of the program.

KTC’s School of Practical Nursing works diligently to prepare its students for the health care workforce and for the national licensure exam. As a District, KTC continues to strive and excel beyond the state and national, first attempt, NCLEX-PN Candidate Pass Rate. KTC’s NCLEX-PN Pass Rate as a District for the past five years (2011-2015 YTD) is 93.19%.

Covering a 13 county area and approximately 11,200 square miles, the KTC Practical Nursing Program is not only the largest program of its kind in number of students but also in the number of square miles the school district encompasses. (Refer to map inside front cover.) The Full-Time and the Part-Time Programs are both post-secondary programs designed to provide students with basic training in Practical Nursing. An important phase of the training is the practical experience provided throughout this program. Upon graduation, students will be eligible to take the NCLEX-PN examination, which if successfully passed, will provide excellent employment possibilities in the healthcare field in which there is presently a shortage of trained practical nurses.

The Full-Time course is eleven-months or 1508 clock hours. The Part-Time course is eighteen-months or 1508 clock hours. The Part-Part-Time course covers two school years with a summer break in June and July. The curriculum includes class, laboratory practice and clinical experience. There are 780 hours spent in class, laboratory practice and group conferences. This enables the students to gain basic information and skills needed in the hospital or other health organizations and 728 hours are spent in the practice of nursing patients of all age groups, under the direction of professional instructors.

The Kiamichi Technology Center Practical Nursing program is designed to meet the needs of students who reflect a wide range of academic abilities and socio-economic backgrounds.

REFUND POLICY

The Kiamichi Technology Centers’ policy regarding refunds is as follows:

A refund will be made for tuition based on the following schedule:

EDUCATION PROGRAM

The Kiamichi Technology Center Practical Nursing Program is approved by the Oklahoma Board of Nursing (OBN). Graduates of this state-approved program are eligible to apply to write the National Council Licensure Examination (NCLEX) for practical nurses. Applicants for Oklahoma licensure must meet all state and federal requirements to hold an Oklahoma license to practice nursing. In addition to completing a state-approved nursing education program that meets educational requirements and successfully passing the licensure examination, requirements include submission of an online application for licensure, a fingerprint-based background check conducted by the FBI and the Oklahoma State Bureau of Investigation, and evidence of citizenship or qualified alien status. To be granted a license, an applicant must have the legal right to be in the United States (United States Code Chapter 8, Section 1621). In addition, Oklahoma law requires the Board of Nursing to issue a license only to U.S. citizens, U.S. nationals, and legal permanent resident aliens. Other qualified aliens may be issued a temporary license that is valid until the expiration of their visa status, or if there is no expiration date, for one year. (updated December 2015)

Applicants who are qualified aliens must present, to the Board office, in person, valid documentary evidence of:

1. A valid, unexpired immigrant or nonimmigrant visa status for admission into the United States;

2. A pending or approved application for asylum in the United States;

3. Admission into the United States in refugee status;

4. A pending or approved application for temporary protected status in the United States;

5. Approved deferred action status; or

6. A pending application for adjustment of status to legal permanent resident status or conditional resident status. The Oklahoma Board of Nursing has the right to deny a license to an individual with a history of criminal background, disciplinary action on another health-related license or certification, or judicial declaration of mental incompetence. Further, the OBN shall impose a disciplinary action against the person who is attempting to procure a license as a LPN and is guilty of deceit or material misrepresentation upon proof of any of the previous items [59 O.S. §567.8]. These cases are considered on an individual basis at the time application for licensure is made, with the exception of felony convictions. An individual with a felony conviction cannot apply for licensure for at least five years after completion of all sentencing terms, including probation or other requirements imposed by the sentencing court, whichever shall last occur, or a presidential or gubernatorial pardon is received [59 O.S. §567.6]. Applications for the KTC Practical Nursing Programs are only accepted from January 1st through April 15th each year for the classes that

begin in June and August of that year.

The PN Application is available beginning January 1 each year. An application packet and PN Program Catalog containing frequently asked questions can be obtained at your local KTC campus or on the KTC website: www.ktc.edu.

The deadline for submitting your application which includes: the PN application form, required and related documents, and a nonrefundable application fee is April 15th, 5:00 PM at your KTC

campus. If the deadline falls on a Saturday or Sunday the applications will be accepted through the following Monday until 5:00 PM. Your application must be submitted in person to the campus you wish to attend. DO NOT submit applications or related documents to the KTC PN Director’s Office.

ADMISSION REQUIREMENTS

The KTC Practical Nursing program is for ADULT STUDENTS ONLY.

Because class sizes are limited, the selection for entrance into the Practical Nursing Program is based upon a point system arrived at through the application process. This process includes:

Applicant must be 18 years old when the program begins. Copy of birth certificate is required.

Applicant must be a graduate of an accredited high school accredited by an appropriate agency of the home state or accredited by the appropriate regional association; or a General Education Development (GED) Certificate. An official high school transcript or a copy of the GED Certificate is required.

Applicant must be able to provide evidence of citizenship (birth certificate) or qualified alien status. To be granted a license, an applicant must have the legal right to be in the United States (United States Code Chapter 8, Section 1621). In addition, Oklahoma law only allows a license to be issued to U.S. citizens, U.S. nationals, and legal permanent resident aliens. A copy of birth certificate or appropriate document is required. Submission, by the deadline, of a complete and legible application along with required documents, and nonrefundable Application Fee. Applications must be submitted in person.

Submission of related documents regarding educational background. Qualifying pre-admission examination scores.

If applicable, submission of “certified/official” Arrest Records.

If you have ever been summoned, arrested, taken into custody,

indicted, convicted or tried for, or charged with, or pleaded guilty to,

the violation of any law or ordinance, or the commission of any misdemeanor or felony, or have you been requested to appear before any prosecuting attorney, or investigative agency, in any matter, with the exception of minor traffic violations, then you must submit certified/official copies of the court documents and a signed and dated written statement of explanation WITH THIS APPLICATION.

If applicable, submission of signed and dated handwritten letter describing the date, location, and circumstances of any disciplinary action or investigation taken against any health-related license or certification or registration. Certified copies of the Charges/Complaints, Findings of Fact, and Orders must be sent by the licensing agency to: KTC School of Practical Nursing, PO Box 70, Antlers, OK 74523.

If applicable, applicants who have ever been judicially declared incompetent are required to submit certified copy of Court Order and a signed and dated handwritten letter describing the date, location and circumstances of the incident(s), and, if applicable, the resulting action(s) taken by the court.

NOTE: Meeting the stated requirements does not guarantee admission to the PN program. Qualified applicants will be contacted for an interview. Selection for entrance into the Practical Nursing Program is based upon a point system. Applicants are evaluated on a number of variables including: evidence of motivation, academic ability, personal understanding of and propensity for practical nursing, references, interview, and potential for continued constructive use of professional education. The KTC Practical Nursing program is governed by the Oklahoma Board of Nursing (OBN). Each instructor cannot supervise more than 10 students in the clinical area. Therefore, our programs are limited to a 1:10 ratio which is 10 students in each part-time program and 20 students in each full-time program. 7

PRE-ADMISSION EXAMINATION

The KTC Practical Nursing program utilizes the Evolve Reach (HESI) Admission Assessment Exam (A2). You will register for this exam when you submit your PN Application. More detailed information provided on pages 27-29.

PREREQUISITES REQUIRED

The only prerequisite other than a high school diploma from a state accredited school or a GED, both of which are explained in detail on the previous page under “Admission Requirements”, is the Healthcare Provider CPR Certification. This CPR certification must include infant, child, adult and be for the “health care provider”. We prefer the American Heart Association BLS for Health Care Providers Program (CPR/AED).

Students must be CPR certified for the entire program. KTC will offer a class to obtain this certificate prior to the first day of your PN class. If you already have CPR certification, it must meet the KTC requirements and not expire before the end of your PN Program. It is your responsibility to register for the CPR class when it is offered at your KTC campus.

TUITION?

Your shoes, watch, basic school supplies, pre-graduation background check, OBN application fee, and NCLEX-PN testing fee are not included in tuition. The shoes and watch must meet certain criteria. Do not purchase them before the program begins. You will not be required to have these items until your clinical rotations begin. Your instructor will give you the criteria for these items.

In March/April students must have approximately $350 set aside for OBN application and NCLEX-PN testing fees that must be completed online and paid for by the student with a credit or debit card. Students should also be prepared at this time to pay for the required OBN national background check.

INSURANCE

HEALTH/HOSPITALIZATION INSURANCE is not required but highly recommended. KTC and clinical facilities are not responsible for student’s personal injury. The student is not covered by sick benefits provided to employees of KTC or clinical facilities.

*PROFESSIONAL LIABILITY INSURANCE is insurance that protects the student & KTC in the event of a patient accident/injury. Professional liability insurance is required and is included in the tuition.

PHYSICAL/MENTAL

REQUIREMENTS

The following minimum physical and mental requirements are reflective of industry standards for an LPN job description:

• Requires corrected hearing and vision within functional ranges.

• Requires frequent standing/walking, usually conducted over linoleum or hard-surface floors, for up to 12 hours a day.

• Work is moderately heavy with frequent lifting and carrying of objects weighing 30 to 50 pounds, and pushing and pulling 100 to 150 pounds.

• Requires bending, stooping, kneeling, pushing and pulling, reaching and occasionally climbing and/or balancing.

• Expected to use proper body mechanics at all times. • Mental demands include analytical skills, including

Immunization/Health Screening requirements are based on clinical institutions’ required documentation to attend clinicals.

We believe Education requires the active participation of the adult learner in an organized program of study utilizing critical thinking, acquired knowledge, skills and attitudes. Learning is a shared process between student and faculty. The students maintain an individual accountability for assimilating knowledge and skills for their own personal satisfaction, as well as to benefit society. The faculty provides a learning environment in which the curriculum is developed to proceed from the simple to the complex and provides close correlation between theory and practice through concurrent and/or sequential instruction. Integration of their knowledge is a challenging educational undertaking and remains a lifelong process for any individual.

Nursing Education requires the active participation of the adult learner and emphasizes the technical skills necessary to recognize and meet the health care individual’s basic physical, emotional, spiritual, and socio-cultural needs. We believe nursing education should assist the student in acquiring the knowledge, skills and attitudes necessary to function as a member of the health team meeting the basic human needs of clients of all ages where nursing takes place, or wherever the community dictates. THE KTC PRACTICAL

CLINICAL ROTATIONS

In the KTC PN Program, you will receive hands-on training at various clinical sites. These clinical experiences are provided in a wide variety of agencies including but not limited to: hospitals, doctors’ offices, nursing homes, health clinics, etc.

Many of these clinical facilities are located over 45 miles away. KTC does not guarantee you will attend clinical at the health care facility closest to your home. Also, some PN programs may travel to Arkansas or Texas clinical facilities to receive the necessary clinical training. STUDENTS MUST HAVE DEPENDABLE TRANSPORTATION AND BE PREPARED TO MEET THE EXPENSES OF COMMUTING. Many students carpool if the clinical site is very far from their campus.

STUDENTS WHO ARE PARENTS OF YOUNG CHILDREN MUST HAVE RELIABLE CHILD CARE SUPPORT SERVICES AVAILABLE DURING EARLY HOURS (5:00-6:00 AM) AS WELL AS LATER HOURS (6:00-11:00 PM). It is advisable to also have a good “back-up plan”.

CLINICAL HOURS

DAYTIME

Full-Time and Part-Time Programs - clinical hours begin at 6:30 or 7:00 AM These clinical days are 8-12 hours for full-time and 7-10½ hours for part-time.

EVENING

Part-Time Programs – clinical hours begin at 2:30 PM These clinical days are 7-10½ hours long.

ALL PN PROGRAMS – Days for class/clinical may vary to accommodate various learning experiences. There may be occasions when evening or night clinical times are required for daytime programs and day clinical times may be required for evening part-time programs.

A student may be placed on academic probation if their theory or clinical grade falls below an 80% and/or if a student fails two or more unit tests, considering the student to be “at risk” for failing the course. Any student placed on probation may be subject to dismissal from the program.

Students who fail to achieve a passing grade of 77% in EACH course and clinical component of the Program will be dismissed from the program for academic failure.

ATTENDANCE POLICY

THE KTC PRACTICAL NURSING PROGRAMS’ ATTENDANCE POLICY IS VERY STRICT. A student must attend 1508 hours of class and clinicals to graduate from the program and be eligible to take the licensing exam. A Full-Time Program student will not be allowed to miss more than 60 hours of excused absences during their 11-month course. A Part-Time Program student will not be allowed to miss more than 84 hours of excused absences during their 18-month course. However, TWO (2) “UNEXCUSED” ABSENCES will result in dismissal from the program.

NOTE: Students receiving PELL grants must adhere to the KTC Academic Policies, Section II.B Attendance Policy or risk losing PELL funding.

Kiamichi Technology Center and the School of Practical Nursing consider student conduct and discipline to be very serious matters. Kiamichi Technology Center School of Practical Nursing (“KTC”) only serves adult students. KTC has established this Policy on Nursing Student Conduct and Discipline (“Policy”), which is applicable to all nursing students, full-time and part-time. This Policy explains the standards of conduct and civility expected of nursing students and also explains the disciplinary actions that may be taken when a student’s conduct violates those standards. Educational opportunities available to students may be cut-short or terminated in instances where a student’s conduct violates the approved standards outlined in this Policy or when a student, for other reasons, cannot fulfill program requirements essential to successful course completion. In instances involving a student’s dismissal from a course or program, KTC will utilize procedures that are fair and reasonable. Each student is encouraged to cooperate and comply with the standards of conduct set out in the Policy. This Policy is designed to ensure that all students have an opportunity to benefit from educational opportunities available at KTC. Conduct or behavior that violates any of KTC’s policies, rules, regulations or practices, or which interferes with or disrupts the educational process, must be and will be addressed by KTC administration. This Policy explains KTC’s standards of conduct, the disciplinary actions that may occur as a result of violations of the standards, and the procedure that will be used when it is necessary to dismiss a student from a course or program.

DRUG TESTING

Students of the Kiamichi Technology Center School of Practical Nursing will be required to submit to routine, random and reasonable suspicion drug testing throughout the program.

All students will be tested for drugs and alcohol prior to their participation in clinical rotations. Additionally, students may be tested for drugs and/or alcohol at any time on a random selection basis while enrolled in the Practical Nursing Program. Finally, reasonable suspicion testing will be performed as needed, per the judgment of the Director of Practical Nursing. All drug and alcohol testing will be conducted in compliance with applicable law.

Testing positive for drugs or alcohol, or refusing or failing to participate in or cooperate with any part of the drug testing process is a direct violation of Kiamichi Technology Center’s Policy on Nursing Student Conduct and Discipline. This violation is taken very seriously. Any student who tests positive for drugs or alcohol or refuses or fails to participate in or cooperate with any part of the drug testing procedure will be immediately dismissed from the KTC Practical Nursing Program.

ARREST RECORD

KTC School of Practical Nursing complies with the Oklahoma Board of Nursing relative to felony convictions and application for licensure. Based upon a history of arrest and/or convictions, students may be unable to perform clinicals at certain facilities.

If you have ever been summoned, arrested, taken into custody, indicted, convicted or tried for, or charged with, or pleaded guilty to, the violation of any law or ordinance, or the commission of any misdemeanor or felony, or have been requested to appear before any prosecuting attorney or investigative agency, in any matter, with the exception of minor traffic violations, then you are considered to have an arrest record. (Please note that charges including, but not limited to DUI or DWI are not considered minor traffic violations and must be reported.)

If you have an arrest record (misdemeanor or felony) you are required to submit, with your PN Application, “certified” copies of the court documents and a statement of explanation regarding the arrest. Refer to Questions 25 & 26 in the FAQ section of this catalog, for additional detailed information regarding arrest records.

The Oklahoma Board of Nursing may or may not allow you to apply for licensure with such a record. Clinical facilities may refuse to allow you to perform clinicals at their facility which could hinder or prevent your ability to complete the program. It depends on the severity of your arrest record. An arrest will not necessarily keep you out of the KTC PN Program.

• Affidavit of Probable Cause,

• Information Sheet (a brief summary of the incident prepared by the court),

• Charges (a listing of the charges brought against you),

• Judgment and Sentence (findings of the court and sentence imposed), and

• Verification that sentencing requirements are complete,

• Signed and dated handwritten letter describing the date, location and circumstances of the incident(s), and, if applicable, the resulting action(s) taken by the court or agency. If you have more than one incident you are reporting, you must describe every case/charge separately that has been filed.

These documents may be obtained from the courthouse in the county/city or in the federal court of the district in which the court action occurred.

If no records are found, have the agency provide a certified letter stating no records were found in a search from the date of offense through current date.

Applicants who have:

• ever had disciplinary action taken against another health-related license or certification or registration,

• or if there is currently any investigation on another health-related license or certification or registration

are required to submit, with your application, a signed and dated handwritten letter describing the date, location, and circumstances of the incident(s), and, if applicable, the resulting action(s) taken by the disciplinary board. If you have more than one incident, you must describe every case separately that has been filed.

Please have the licensing agency submit certified copies of the following to: Kiamichi Technology Center, School of Practical Nursing, PO Box 70, Antlers, OK 74523:

• Charges/Complaints

• Findings of Fact

• Orders

Applicants who have ever been judicially declared incompetent are required to submit the following documents with your application:

• Certified Copy of Court Order

• Signed and dated handwritten letter describing the date, location and circumstances of the incident(s), and, if applicable, the resulting action(s) taken by the court. If you have more than one incident you are reporting, you must describe every case separately that has been filed.

Certified copies are copies of court records obtained directly from the courthouse in the county/city or the federal court of the district in which the action occurred. These documents are dated and signed by the court clerk, and affixed with the court seal.

Expunged Record – If the offense has been expunged, you must submit a certified copy of the Order of Expungement. You obtain these documents from the courthouse in the county/city or in the federal court of the district in which the court action occurred.

Internet court documents (such as OCIS case reports) and faxed recordswill not be accepted.

Felony Record - A candidate for a license to practice as a licensed practical nurse shall submit to the Oklahoma Board of Nursing “certified written evidence that the applicant has never been convicted in this state, the United States or another state of any

felony, unless five (5) years have elapsed since the date of the criminal conviction or the termination of any probation or other requirements imposed on the applicant by the sentencing court, whichever shall last occur, or a presidential or gubernatorial pardon for the criminal offense has been received” [59 O.S. §567.6].

Therefore, applicants for licensure in Oklahoma with one or more felony convictions cannot apply for LPN licensure in Oklahoma for at least five years after completion of all sentencing terms, including probation and suspended sentences, unless a presidential or gubernatorial pardon is received.

PLEASE NOTE THAT YOU MUST REPORT ALL ARRESTS AND/OR CHARGES THAT HAVE BEEN BROUGHT AGAINST YOU.

IF YOU ARE FOUND TO HAVE BEEN DISHONEST CONCERNING ANY OF THE ABOVE, YOU MAY BE DISMISSED FROM THE KTC PN PROGRAM. Failure to submit the above information in a timely manner may result in a delay in processing the OBN application. After the OBN staff reviews this information, the applicant will be notified of any additional action that must be taken. The applicant may be required to appear before the OBN. Additional documentation that may be requested prior to this appearance, includes, but is not limited to, letters of reference from a clinical faculty member, the director/dean of program, a probation officer, employer or other. Please be aware that the applicant may not be eligible for licensure or endorsement to surrounding states due to individual states’ restrictions, even if the applicant is able to be licensed in Oklahoma. Individuals who plan to apply for licensure in other states must check with that state’s board of nursing to obtain information on requirements.

ADMISSION/SELECTION PROCESS

1. How are the Practical Nursing applicants selected?

Because class sizes are limited the selection for entrance into the Practical Nursing Program is based upon a point system arrived at through the application process. This process includes the following entrance requirements that must be met for consideration of an interview with the Admissions Committee:

 The KTC Practical Nursing program is for ADULT STUDENTS ONLY.

Applicant must be 18 years old when the program begins. A copy of birth certificate is required.

 Applicant must be a graduate of an accredited high school accredited by an appropriate agency of the home state or accredited by the appropriate regional association; or a General Education Development (GED) Certificate.

An “Official” high school transcript showing the date of graduation (not a copy) or submit a copy of the GED Certificate with scores (copy acceptable), whichever applies to you; must be submitted with your PN Application. A copy of your diploma is not acceptable.

 Applicant must be able to provide evidence of citizenship (birth certificate) or qualified alien status. To be granted a license, an applicant must have the legal right to be in the United States (United States Code Chapter 8, Section 1621). In addition, Oklahoma law only allows a license to be issued to U.S. citizens, U.S. nationals, and legal permanent resident aliens. A copy of birth certificate or appropriate document is required.

 Applicant must submit a KTC PN Application and related documents, in person, by the deadline along with the nonrefundable Application Fee. The PN Application includes information on:

 Employment Background information; if any

 Educational Background information; (Verification is required; for example, official college transcripts, certificates such as CNA, CMA, Career Readiness Certification, etc.)

 Signed Reference Forms to be mailed by KTC (not by applicant). References from family or friends will not be accepted.

 Signed Criminal History Records Search form to be performed by KTC PN Administration. Make certain that you do not submit an incomplete form;

 If you have an arrest record (misdemeanor or felony) you are required to submit, with your PN Application, “certified/official” copies of the court documents and a written statement of explanation regarding the arrest(s). Refer to Page 13 & 14 and also to Questions 25 & 26 in the FAQ section of this catalog, for additional detailed information regarding arrest records.  If applicable, submission of signed and dated handwritten letter describing the date, location,

and circumstances of any disciplinary action or investigation taken against any health-related license or certification or registration. Certified copies of the Charges/Complaints, Finds of Fact, and Orders must be sent by the licensing agency to: KTC School of Practical Nursing, PO Box 70, Antlers, OK 74523.

NOTE: Meeting the stated requirements does not guarantee admission to the PN program. Qualified applicants will be contacted for an interview. Applicants are evaluated on a number of variables including: evidence of motivation, academic ability, personal understanding of and propensity for practical nursing, references, interview, and potential for continued constructive use of professional education. Following the interview process with the Admissions Committee, qualified applicants with the highest interview scores will be admitted into the program.

A. International Applicants ONLY — Official TOEFL Total Scores: 500 on paper-based test or 61 on internet-based test; International English Language Testing System (IELTS) minimum academic band score required is 5.0 (applicable only to International applicants for whom English is a second language – English Language Learners - ELL).

NOTE: The following exceptions apply to the English proficiency requirement: Students from Australia, New Zealand, the British Isles, and the English speaking provinces of Canada are exempt. Students from other countries where English is the primary language of instruction and the language spoken at home and by the indigenous population may petition for an exception.

Students who have earned a baccalaureate degree from a regionally accredited United States college or university and students who have earned a high school diploma from a United States high school with passing scores are exempt.

NOTE: Applicants who received their high school education outside the United States must submit official documentation that their high school education and subsequent graduation is equivalent to that of a high school graduate in the United States. This may be accomplished by one of the following methods:

a) Take the GED exam and submit scores/documentation indicating high school equivalency. The applicant may contact KTC counselor for assistance with this process.

b) Request a review/evaluation of academic records from an official agency that specializes in validating academic credentials of persons educated outside the United States. Applicant may contact the World Education Services at www.wes.org

or refer to the list of Education Credential Evaluating Agencies located in the back of this FAQ Section (see page 38 for a list of agencies available.) Provide proof/documentation of such to KTC with the application packet. Such documentation must indicate that the applicant’s education meets high school graduate equivalency in the United States.

c) If the applicant has completed a minimum of 30 college hours from an accredited/approved higher education institution within the state of Oklahoma, the applicant may be eligible to petition the Oklahoma Department of Education to be awarded high school equivalency. For more information the applicant may contact the Oklahoma Department of Education at 1-800-405-0355 or 1-405-521-3301.

It is the applicant’s responsibility to provide one of the above forms of documentation. The applicant is responsible for any cost that might be incurred.

2. Why doesn’t KTC accept all applicants?

The KTC Practical Nursing Program is governed by the Oklahoma Board of Nursing (OBN). Each Instructor cannot supervise more than 10 students in the clinical area. (The maximum ratio is 1:10 in both the Full-Time and Part-Time programs.)

3. What is the deadline for returning my completed PN Application?

April 15th by 5:00 PM is thelast day

(deadline date) to submit your application for the upcoming program in July or August. If the deadline date falls on a Saturday or Sunday the applications will be accepted through the close of business (5:00 PM) the following Monday.

PLEASE BE AWARE OF THE ADDITIONAL DOCUMENTS REQUIRED FOR A COMPLETE APPLICATION. Your application must be submitted in person and must contain your “official” high school transcript showing the date of graduation (not a copy) or submit a copy of your GED Certificate with scores (copy acceptable), whichever applies to you. Copy of diploma is not acceptable.

An official transcript contains the school’s seal and must be in a sealed envelope mailed to you from the educational facility and must be unopened. If a school will not send an official transcript to you have it sent to the Practical Nursing Instructor’s attention at the campus you are applying at.

Do not request OFFICIAL TRANSCRIPT(S): high school, college, other, to be sent to the Practical Nursing Director’s office. KTC PN Director’s office will not be responsible for any transcripts mailed to our office.

In order to receive additional points for any education beyond high school or GED, your application must also contain your “official” college

transcripts, copies of certificates (i.e., CNA, CMA, Career Readiness Certification, etc).

Complete all the information accurately and legibly on your application before submitting it to the campus in person. The application should be typewritten or printed legibly in black ink. Use your

LEGAL NAME on your application and throughout the entire program. Now is a good time to double check your personal records, such as your driver’s license and birth certificate. Make sure it contains your LEGAL NAME and that it is spelled correctly. Also, make certain that your driver’s license address is current.

4. How important is my high school transcript or GED scores?

Your application is considered incomplete if it does not include either your “official” high school transcript or a copy of your GED Certificate with scores. Therefore, you would not qualify for an interview.

Your OFFICIAL TRANSCRIPT(S) must be submitted with your application. The transcript(s) must be in a sealed envelope from the educational facility and unopened. If a school will not send an official transcript to you have it sent to the Practical Nursing Instructor’s attention at the campus you are applying at.
