writing procedure, a consumer report may be obtained or an investigative consumer report may be prepared. Such reports may

DEFINITIONS: As used herein, ‘you,’ ‘your,’ and ‘I’ means the applicant. ‘Member of your household’ means your spouse

and any person related to you by blood, marriage or adoption who either lives with you or is away at school and anyone who

lives with you and is in your or a relative’s care or custody. ‘Driver’ means ‘you’ and ‘members of your household’ who operate

motor vehicles licensed for road use, plus any other person who operates a vehicle 50% or more which is owned, leased, rented

or regularly operated by you or a member of your household.

UNINSURED/UNDERINSURED MOTORIST COVERAGE: Uninsured/Underinsured Motorist coverage is provided in the

states of Florida, Georgia, Indiana, Louisiana, New Hampshire, Vermont, and West Virginia for an additional premium. You

are required to maintain Uninsured/Underinsured Motorist coverage limits equal to the limits required for your underlying

automobile liability coverage. If you elect to reject Uninsured/Underinsured Motorist coverage you must complete form: PUP

257-A in New Hampshire, PUP 257-B in Florida, Georgia and Indiana, PUP 517 in Louisiana, PUP 257-D in Vermont, or PUP

547B in West Virginia. Receipt of the applicable form by the company will result in a reduction in the premium charged for

your Personal Umbrella Liability Policy.

Youth is defined as any driver under the age of 22. If the risk has any drivers under the age of 22, underlying automobile limits of

$500/500/50 or $500 CSL are required. All other risks require underlying automobile limits of $250/500/50, $300/300/50 or $300 CSL.

Standard II premiums are NOT available if there are any drivers over the age of 69 in the household.

To qualify for the Preferred rate, all answers to questions 1 through 9 on the Application must fall under the Preferred column. Even if one

answer falls under the Standard column, you use the Standard rate; and even if one answer falls under the Standard II column, you use

the Standard II rate.

Add $25.00 per Antique/Classic vehicle to the premiums above (count not to exceed 25). Antique/Classic vehicles are defined as private
