Addendum No. 3

ITB 21-05-406 Kimberly Boulevard Roadway and Sidewalk

This Addendum shall modify and become a part of the original ITB document. The

following clarifications, changes, additions, and/or deletions are hereby made part

of the Contract Documents for ITB 21-03-406 Kimberly Boulevard Roadway and

Sidewalk.

TO ALL PROSPECTIVE BIDDERS

The City has extended the bid due date to Wednesday, June 30, 2021,

at 3:00 PM EDT

THE CITY WOULD LIKE TO PROVIDE THE FOLLOWING

CLARIFICATION TO BIDDERS:

1. SCOPE OF WORK 3.4 OWNER ENHANCEMENTS

Words stricken are deletions; words underlined are additions to the RFP.

The City may elect to add owner enhancements to this SURTAX project. These enhancements may include, without limitation, decorative lighting, decorative paving, and improvements that are not within the public right of way. These items shall be funded solely by the City of North Lauderdale with non-surtax funding. Bidder shall price these items accordingly with the entire bid price submittal, but it shall be noted, the County shall have no funding responsibility for any of these costs through the SURTAX program. For this project, the City plans to utilize two (2) items that would be considered owner enhancements. These line items include:

The City will award this project based on OPTION B - CONSTRUCTION WITHOUT DECORATIVE PAVERS AND HEADER CURBING outlined within the bid schedule. The City reserves the right to award the options requested to the primary Contractor or other contractors as needed and determined in the City's best interest. See Bid Schedule - REVISION1.

QUESTIONS AND ANSWERS

Florida Engineering & Development Corporation

Q1. The Maintenance of Traffic is going to be approved by the City or Broward County?

A. The Maintenance of Traffic will be reviewed and approved by the City of North Lauderdale.

Q2. Who pays for the Quality Control (Proctor, density test, concrete test, and asphalt) the City or the Contractor?

A. Quality control (proctor, density test, concrete test, and asphalt) will all be paid for by the Contractor. These tests are paid as Bid Item 3. Refer to Technical Specifications (Section 01450).

Q3. Could you please provide the dimension of the Control Structure?

A. Dimensions for the proposed 5'x8' control structure are provided on plan sheet DET-2 in the detail named "CONTROL STRUCTURE W/WIER."

Q4. There are 21 palms in the median removal zone. In that area are there going to be the new pavers. Does the Contractor have to remove those palms?

A. Trees are to be removed within 5 feet of paver areas. These removals are included in Bid Item 6. A tree removal schedule has been added to plan sheet NOT-1, and the individual tree removals are now shown on the demolition plan.

Q5. Please confirm the Detail of the Pavers shown on Sheet DET-1.

A. The paver detail has been revised for clarity. There will be 2 separate Bid Items for the Decorative Pavers.

The vehicular-rated pavers shall be installed at the roundabout intersection, including the median noses approaching the roundabout. The Technical Specifications and Plans have been revised to reflect this change.

Q6. The detail for the Reconstruction Pavement is confusing. On sheet DET-1, the details call for S-III, 2 Lifts ( Typical Pavement Section ), SP-12.5 and SP-9.5 Friction Course, 2 Lifts ( Typical Pavement Interface ), and the item # 16 is SP-9.5. On Sheet SEC-1, Note 1, says VIRGIN ASPHALT Type SP-9.5. Please clarify.

A. Asphalt does not need to be virgin.

The details named "TYPICAL PAVEMENT SECTION" on sheet DET-1,

"PAVEMENT RESURFACING" on sheet DET-1, and NOTE 1 ON SEC-1 have been updated to reflect this clarification.

Q7. What kind of asphalt is the 1st lift for the Reconstruction (Item # 17)?

A. The first lift (bottom) shall be shall be one-inch (1") SP-9.5 asphaltic concrete for pavement reconstruction. Details updated accordingly.

Q8. What kind of asphalt is the last lift in the reconstruction area and the resurfacing area ?

A. The last lift (top) shall be shall be one-inch (1") SP-9.5 asphaltic concrete for pavement resurfacing and reconstruction. Details updated accordingly.

Q9. The Asphalt of the last lift for the entire project has to be VIRGIN?

A. No, the last lift does not have to be virgin. Details updated accordingly.

Q10. What is the thickness of the Limerock Base for the 6" Concrete Drive?

A. Limerock base for the 6" concrete drive is not required. Concrete will be installed on existing subgrade compacted to 98% of max. density per AASHTO T-180. Details updated accordingly.

Q11. What is the thickness of the Limerock Base for the 2" Asphalt Drive?

A. Limerock base for the 2" asphalt drive shall be 6". Details updated accordingly.

Q12. There are some Loop Detectors on Kimberly Blvd ( Rock Island Road and US-441 ).

Who repairs these Loop Detectors, the City or the Contractor . If it is the Contractor, please add an item for that work.

A. Contractor shall restore all loop detectors. This has been added to the Measurement and Payment to be included in the cost of Bid Items 16 and 17.

Q13. Please verify the size of pavers and the color of the "decorative pavers" for the above project.

A. The following table shows the paver sizes and colors for the decorative pavers.

Q14. Please confirm the thickness of 3 1/8" according to the detail in DET-1 or if 2 3/8"

pavers will be sufficient.

Q15. On page 21, the Bid Documents state that the Payment and Performance Bond is 100 % and on page 41, the documents in Warranty Bond say "the Contractor shall provide a warranty bond shall be submitted to the City for Twenty-Five ( 25%) of the total value of the contract". Is it an additional Bond? Please clarify.

A. The warranty bond is an additional bond and shall be 50% of the amount of the performance bond. The warranty bond shall be in place for 1 year after the completion of construction. The warranty bond must be in place before the release of payment and performance bond.

Q16. Please confirm that Contractor is responsible for preparing a Signed and Sealed Traffic Control Plan to be approved by Broward County.

A. The Maintenance of Traffic will be reviewed and approved by the City of North Lauderdale.

Q17. In consideration of Note 1 on CVR-1, please verify which permits the Contractor will be responsible for obtaining. If City and County permits are required, will the application fees be waived?

A. The Contractor is responsible for getting all necessary permits from the City and from the County. The City portion of the fees will be waived. All County fees will have will be paid by Contractor. The City estimates approximately $2,100 in associated permit fees.

Q18. In consideration of Note 17 on NOT-1, please provide a table for the existing pipes and structures to be cleaned.

A. All existing storm pipes and structures within the project shall be cleaned.

Q19. The ITB indicates speed tables in the project title. Please provide a detail and the location of the speed tables. Which pay items are these to be included in the bid pricing?

A. Speed tables have been eliminated from the scope of this project.

Q20. Adjust Existing Utilities - Please provide a table for the Existing Utilities to be adjusted under this LS pay item. The description in the Technical Specifications is

A. Utilities will be adjusted as necessary to accommodate the proposed work. An estimate of 17 sanitary manholes, 62 storm structures, and 35 water valves may require adjustment.

Q21. Irrigation questions / clarifications - Based on the language in the Technical Specifications, Pay Items #52 and #53 appear to have the quantities and units under the wrong item. Please confirm.?

A. Confirmed. The units and quantities for items #52 and #53 were switched. The Bidder's Schedule of Prices has been updated to match the Technical Specifications, which was correct.

Q22. Irrigation questions / clarifications - Please provide the Allowance amount for Bid Item #53 once the quantity and unit have been corrected.

A. An allowance of $20,000 is estimated.

Q23. General - Addendum 2 clarifies the bid options on the bid form. Please confirm the basis of award of the project. Will a final determination of award be made at the bid opening?

A. See CLARIFICATION TO BIDDERS # 1 above.
