Day Programs

Day Programs at the Melbourne Clinic (TMC) offer therapy treatment to people with a range of psychiatric conditions. The programs are evidence based and are facilitated by qualified health professionals.

Day Programs will assist clients with:

• The transition period following hospitalisation;

• Outpatient treatment and support during crisis or increased stress;

• Ongoing treatment to maintain wellbeing, enhance functioning and facilitate personal growth to prevent readmission;

• Avoiding hospitalisation.

Attendance requirement:

• A referral is required from The Melbourne Clinic accredited psychiatrist to attend the day program;

• Entry assessment by the day program clinician;

• A range of suitable programs are recommended based on assessment;

• Regular appointments with a TMC accredited psychiatrist is essential for continued attendance;

• The client can attend more than one program;

• Private health insurance is recommended

• Other agencies (work cover, TAC) can cover day program participation

Who:

The program is designed for adults with a range of mental health problems.

Content:

Participants will take part in Acceptance and Commitment Therapy (ACT) in a group setting, where they will be assisted in:

• Learning how to practice acceptance of unhelpful experiences, including thoughts, urges, memories and emotions.

• Developing mindfulness skills - the ability to be in the present moment and to focus attention.

• Learning cognitive de-fusion techniques to change the way they relate to the thoughts they experience.

• Identifying personal values and understanding what really matters to them.

• Becoming involved in committed actions toward their valued life direction.

When:

This ten-week foundation program is offered both within business hours and in the evening.

Who:

The program is designed for individuals who have completed the ten-week Foundation ACT Program. This program is designed to integrate and build on the skills to create ongoing change in peoples lives.

This program will assist participants in:

• Deepening their understanding of ACT principals.

• Integrating ACT skills learned into their lives.

• Developing and maintaining regular mindfulness practice.

• Maintaining committed action toward valued life direction.

Criteria for attendance:

• Willingness to use ACT principals.

• Completion of the 10 week ACT – Foundation Program.

• Working towards long term valued life directions and goals.

Supporting Trauma and Recovery (STAR)

Who:

This program is designed for people who have experienced trauma in their past, resulting in significant and on-going emotional and interpersonal difficulties. The program is best suited for people who are interested in making long-term commitment to improving the management of their trauma-related symptoms.

The aim of the treatment program is to help individuals gain greater control of their symptoms and thus regain control of their emotional lives. The treatment involves participating in 15 months of group and individual therapy.

Content:

The program aims to help people enhance their quality of life by:

• Improving emotional regulation

• Processing and moving forward from trauma;

• Letting go off unpleasant beliefs;

• Improving relationships;

• Clarifying and improving one’s sense of self

Structure:

The program is made up of two components that must be completed one after another:

1. Safety and Stabilisation Group (14 weeks), consisting of:

• Psychoeducation about trauma and complex traumatic stress reactions.

• Teaching crisis management skills.

• Introduction of grounding and mindfulness skills.

• Establishing a sense of safety.

• Engagement in treatment.

2. Trauma Treatment Program (12 months of weekly skills groups and weekly individual therapy sessions), consisting of:

• Teaching individuals specific strategies to manage their symptoms and instil in the individual a sense of efficacy about managing stressors.

• Processing traumatic memories incorporating behavioural, physiological and cognitive components.

• Improving emotion regulation skills

• Clarifying and integrating sense of self.

• Improving interpersonal issues.

Ongoing Support:

For clients who require extra support in maintenance and reintegration additional 12 month group program will be offered.

Who:

This program is designed for people diagnosed with depression and various forms of anxiety disorders. The program aims to assist people in gaining insights on the impact of thoughts, feelings and behaviours on people’s mood to improve their quality of life.

Content and Structure:

The program consists of two parts: The CBT Foundation and CBT Follow-up.

CBT foundation - 12-week program. First 6 weeks provides comprehensive training in core CBT concepts and techniques, consisting of:

• Recognizing distorted thinking, and how this affects depression/anxiety

• Identifying common distorted thinking styles

• Evaluating depressive and anxiety-related thinking

• Disputing depressive and anxiety-related thinking

Second 6 weeks of separate subgroups-one for depression and one for anxiety, where the CBT concepts are applied and practiced in a manner tailored to the needs of the group members.

CBT follow-up - offered upon completion of the CBT foundation to enable on-going support and opportunities to practice CBT.

Cognitive Behaviour Therapy for Depression and Anxiety

Who:

The program is designed for individuals with long-standing personality and emotional difficulties, in particular, those with a diagnosis of Borderline Personality Disorder (BPD) or Borderline Personality Traits. The features of BPD include both a chronic history and a pervasive pattern of emotional lability (marked reactivity of mood states lasting relatively brief periods of time), instability of interpersonal relationships and self-image, and impulsive behaviour, including suicidal thoughts and behaviours.

The DBT Program consists of the following:

a. DBT Pre-Commitment Group b. DBT Skills Group

c. DBT Graduates Group

A. DBT Pre-Commitment Group

This program is designed to give potential clients some information about Borderline Personality Disorder (BPD) and the DBT program.

It will also include considerable discussion and exercises to allow clients to explore the changes they wish to make in their lives, as well as some of the potential barriers that can get in the way.

Duration: 8 weeks, once weekly, open group.

B. DBT Skills Group

Components of the Skills Program:

• Full day of group work:

£ Skills Coaching in:

• Core Mindfulness

• Interpersonal Effectiveness • Emotion Regulation • Distress Tolerance

• Individual Therapy

• Phone Coaching

Participants attend one full day of group work plus one 50-minute individual session per week throughout each module.

Program Length and Intake

The program is 12 months in length. It includes three 14-week modules of skills training and individual therapy, involving weekly monitoring of symptoms, generalisation of skills and telephone coaching.

C. DBT Graduates Group

Following completion of the DBT Skills Group, participants are invited to attend the DBT Graduates Group. The graduates group is available weekly or fortnightly and addresses skill generalisation, continuing reduction of quality of life interfering behaviours and development of life enhancing behaviours, including structured goal setting.

Dialectical Behaviour Therapy Program

Who:

The program treats individuals with eating disorders and is designed to foster the restoration of health and weight, and to explore the underlying issues that accompany the illness.

Content:

The program aims to:

• Provide a comprehensive service for eating disorder patients.

• Provide an integrated transitional program for inpatient and outpatient treatment.

• Provide treatment with minimal disruption to daily living and social networks.

• Provide a range of skills to enable patients to manage a healthy weight range.

When:

This program is run during business hours.

Content:

This program aims to:

• Support peoples recovery from bulimia and binge-eating.

• Facilitate development of skills and techniques in order to:

£ Explore the connection between thoughts, feelings and actions.

£ Increase awareness and step out of the vicious cycle of bulimia and binge eating behaviours.

£ Positively transform our relationship with ourselves and others.

£ Foster committed action to change.

When

This program is run during business hours.

Eating Disorders Day Program

Who:

This program is broad in scope, targeting a wide range of mental health concerns. The open and flexible nature of this program enables participants to choose how regularly and for how long they wish to attend. There is no waitlist, making this program ideal for individuals who are waiting for other closed programs to commence. This program is also suitable for individuals transitioning from in-patient to out-patient care as well as those seeking ongoing support and structure.

Content:

Groups are facilitated by a multi-disciplinary team of Mental Health Practitioners. An eclectic approach is used to provide participants with skills from a range of therapeutic approaches, including:

• Cognitive-behavioural strategies,

• Mindfulness and acceptance based approaches,

• Humanistic and positive psychology,

• Creative art therapy.

When:

The program runs during business hours 3 days per week and in the evening. Participants may attend as often as required.

Who:

The program is designed for individuals diagnosed with Bi-Polar I and Bi-Polar II Disorders.

Content:

It is a comprehensive manualised program that assists people to cope with:

• Ways of minimising stress and triggers that may set off an episode of Bipolar Disorder.

• Improving communication with health professionals/ doctors due to you having a better understanding of your own illness.

• Identifying and finding ways of managing warning signs of illness, and taking steps to prevent relapse.

• Reducing damage done by an episode of illness, for example reducing the chances of spending too much money when manic.

• Gaining control over the illness so that you can better plan a future, rather than allowing it to control you.

When:

This 10 week program is offered during the evening.

Life Strategies Program

Bi-Polar Management Program

Who:

Originally designed to reduce the risk of relapse in people who have experienced recurrent episodes of depression, the program has been extended to address difficulties with anxiety.

It is also suitable for managing chronic pain and maintaining abstinence from substance abuse.

10-Week Program

This program is offered during business hours. It is designed to teach participants the following:

• Increasing awareness of state of mind, emotions, physical sensations and urges to act.

• Learning about the connection between thoughts, emotions and physical sensations.

• Observing the effect of actions, and learning to refrain from actions that intensify negative emotions, with a particular focus on reducing rumination, worry, and compulsive thinking.

• Learning how to sit with negative emotions, thereby reducing associated distress and emotional reactivity.

Follow-up Program

This ongoing group for graduates of the 10-week program is run on the 3rd Saturday of each month. Apart from reinforcing mindfulness skills for managing negative emotions, participants are also introduced to practices for cultivating positive emotion, such as self-compassion, gratitude and forgiveness.

Mindfulness Based Cognitive Therapy - (MBCT)

Who:

Art Therapy group is designed for adults with a wide range of mental health problems who are interested in using creative expression as a starting point for therapeutic exploration.

No creative experience or talent is required to benefit from this program.

Purpose:

The purpose of Art therapy is for participants to learn about themselves through creative expression and therapeutic group work, and then to use this knowledge to make meaningful, life enhancing change.

Through the process of Art Therapy group participants can expect to:

• Gain insight

• Make sense of feelings and reactions they don’t yet have words to explain

• Come to understand and make meaning of their personal experiences

• Catch sight of that which has been buried or blocked out but which impacts on their lives and recovery

• Get in touch with their personal strengths and resources.

When:

This program is open and ongoing once per week during business hours.

Arts Therapy

Who:

This program is designed for people diagnosed with Adult Attention Deficit Hyperactivity Disorder (ADHD) and Related Disorders. Clients with difficulties of attention and concentration, impulsivity, hyperactivity and/

or executive functioning dysregulation will benefit from this program.

Content:

The program aims to help people enhance functioning in a number of areas, including:

• Understanding how these difficulties have impacted the individual’s life;

• Developing skills and management to better regulate emotions, behaviours and relationships;

• Developing management techniques to improve executive functioning (eg, planning, organisation, self-monitoring)

• Improving relationships;

• Developing goal setting and problem solving skills

When:

This program is offered during the evening.

Adult Attention Deficit Hyperactivity Disorder (ADHD)Program

Who:

The program is designed for individuals, experiencing gambling related problems (eg. spending more money and time than they would like on gambling, chasing losses, gambling despite negative consequences, struggling with relationships, having trouble controlling gambling, etc).

Content:

• Understanding addiction theories and models;

• Reflecting on different types of gambling;

• Exploring changing behaviours;

• Working with ambivalence;

• Exploring readiness to change;

• Identifying strengths and resources;

• Understanding the links between emotions and thinking;

• Connecting with goals and values to live a meaningful life

When:

The program is open & on-going; once per week during evening hours.

Gambling Program - Let’s Talk Gambling

Who:

This program is designed for individuals with a psychiatric diagnosis, who are experiencing difficulties with weight, obesity and related co-morbidities.

Content:

The program aims to improve the health and life style choices of the above adults by providing them with strategies to avoid or reduce weight gain and related illnesses through:

• Education about the effects of obesity on the body, the brain, quality of life and longevity;

• Advice on how to make and incorporate better nutritional and life style choices;

• Development of appropriate regimes for on-going fitness

When:

This program is open and ongoing once a week during business hours.

Addictive Behaviours Program

Who:

This program is designed for individuals who experience problems with dependence and addiction. Inclusive are a variety of addictions, primarily alcohol and other drugs, gambling and any other addictive behaviour that is disruptive and has negative consequences in a persons life. This may include health related problems, relationships issues, impact on daily living, increase in tolerance and any other consequences that have been problematic in life. This program is for individuals interested in recovering from addiction and either maintaining or working towards abstinence.

Content:

The program includes:

• Psycho-education relating to a variety of topics, issues specific to addiction and co-morbidity concerns such as depression and anxiety.

• Mindfulness-Waking up the body and mind

• Emotional intelligence and regulation

• Understanding and managing cravings

• Group Support Therapy focussed on the development of socialisation skills, interpersonal learning, exploring experiences understanding personality and behavioural traits. This therapy allows individuals to receive and provide feedback, support and encouragement to other members of the group in an open and honest environment.

• Developing empathy, insight, self awareness and mood management.

• Understanding and developing healthy ways to relate to others.

• Relapse prevention strategies

• Relaxation techniques

All groups form an integrated continuing care approach in the journey to understanding the self and recovering from addiction.

When:

This program is offered 3 days per week, both during business and evening hours.

Who:

This therapy program aims to offer psychological treatment and support to young people aged 18 – 25 with a range of psychiatric and psychological conditions. The program provides a safe, supportive and structured environment in which they can learn about and explore the experience of young adulthood, life difficulties, mental health and mental illness. The therapeutic techniques used in this program are drawn from current evidence based therapeutic frameworks, including ACT, DBT, Art and Narrative Therapies.

Content:

This program aims to help young people to:

• Better understand themselves and their emotions

• Learn skills to deal with the stress of every day life

• Understand and improve their relationships

• Learn more about mental health issues

• Identify and live according to their values

• Learn skills for independent living

• Strengthen their skills to prevent relapse

• Use creativity to explore themselves

Facilitators:

Facilitators are experienced mental health clinicians including psychologists, occupational therapists and art therapists.

‘CHOICES’ Young Persons Day Program

Who:

This program is designed for individuals experiencing chronic pain conditions. Mindfulness meditation techniques are used to assist participants to manage chronic pain and increase quality of life..

Content:

• Theoretical understanding of chronic pain and how it develops;

• Experiential mindfulness training to assist in managing chronic pain;

• Minimising the impact of accompanying emotional distress;

• Enhanced self-care focusing on the role of diet, sleep and exercise;

• Understanding and managing factors that can exacerbate chronic pain, including stress and relationship conflict;

• Using artistic expression to promote healing, acceptance and self-understanding.

When:

This ten week program is offered during business hours

Mindfulness based Chronic Pain Management

Who:

This program is designed for adults who are interested in empowering themselves and improving their relationships by increasing their personal sense of self worth.

This program provides an opportunity to break the sense of isolation associated with low self esteem and experience the safety and camaraderie of supportive group therapy.

Content:

This program utilises Narrative Therapy to reconnect participants with:

• Personal qualities, which they perhaps have not been aware of.

• The preferred aspects of themselves which they may have lost touch with due to illness or traumatic life events.

• What it is that really matters to them in life and how to create more of it.

Participants will also learn to:

• Gently view the contributing factors which degrade self worth

• Recognise the ways in which we unknowingly become engaged in allowing it to happen.

When:

This ten week group program is offered during business hours.

Discovering Self Worth And Self Esteem

Who:

The program is designed for individuals experiencing both chronic or transient psychotic experiences.

It offers a range of opportunities to explore and better understand the experience of psychosis.

It is an open and flexible group with no waitlist, allowing participants to attend as regularly as required for however long they wish.

This program is ideal for people who are looking for peer support and psycho-education around the experience of psychosis and hearing voices.

It also includes opportunities for family and friends to gain more understanding and skills as significant supports.

Content:

Content is designed to:

- Increase understanding of self and relationship with diagnosis.

- Improve awareness of and response to stressors and triggers.

- Clarify and develop relationships with supports and treating team.

- Increase resilience by exploring strengths and resources.

- Clarify values and goal directed behaviours.
