Cicero Public School District 99, hereinafter named “School District 99” or the “District”, is seeking qualifications from firms interested in providing architectural services for various upcoming projects.

RFQ Submittal:
Four (4) copies must be submitted on the RFQ forms provided by the District, with these specifications and must be contained in a sealed envelope, which shall be labeled on the outside with the following information: Firm name and address, “Architect RFQ, Do not open prior to Tuesday December 8, 2015, 10:00 a.m.” Four hard copies of your proposal are mandatory. In addition, a copy is to be emailed to Asif Dada [email protected] on Wednesday, December 9, 2015, the day after the RFQ opening.
Selection Process
The District intends to enter into a contract with the most responsible firm whose Qualification Submittal is determined to be in the best interest of the District. The District reserves the right to reject any or all qualifications or parts thereof for any reason, to negotiate changes to Qualification terms and to waive minor inconsistencies with the RFQ. The District may accept qualifications which take exception to any requirement in this RFQ or which offer any alternative to a requirement herein. Any exception or alternative must be clearly

delineated and cannot materially affect the substance of this RFQ. The District reserves the right to make a selection based on architectural qualifications, experience in providing similar services elsewhere, the submitter’s responsiveness to the RFQ requirements; and to negotiate a contract with the successful firm. After reviewing the Request for Qualifications, the District will develop a shortlist of 2-3 firms. The shortlist of firms will then present their qualifications for services at an interview with the School District Interview team. After interviewing all the shortlisted firms, the District will rank the firms in order and will begin negotiations. Firm information
Provide a brief history of your firm, including the number of years in the business under the present name and number of architectural/engineering and support staff employed by your firm.
List the name and responsibilities of the team members that would be assigned to the project(s). Include principals, architects, project managers, mechanical engineers, electrical engineers, and other personnel.
List your firm’s banking, bonding and insurance references and limits. List a contact (name and phone number) for each reference.
Indicate if your firm is owned or partially owned by any other organization or individual(s). Provide any additional information which distinguishes your firm other architectural/engineering
firm.
Architectural/Engineering Experience
List all current projects for which your firm is under contact as architect.
List all similar School districts that your firm represents.
List projects for which your firm has been or may be sued in relation to your performance. List any pertinent circumstances regarding these projects.
Project Approach
Describe how the project(s) would be managed beginning with the feasibility phase through occupancy and project evaluation, especially in coordination of assignments with the district’s construction team.
Describe your firm’s estimating and cost control procedures.
Describe your firm’s philosophy on the handling of change orders. Do you expect added compensation for change orders?
Identify services your firm will be coordinating through consultants. Name the consulting firm and its address. Describe project experiences your firm has had with the consultant on similar projects. Compensation

I. Instructions to Vendors
A. The Board of Education and School District 99 administration reserve the right to reject any and all RFQ’s, to negotiate changes and modifications in the vendor's response to this proposal, or to waive any informalities, irregularities or defects in any proposals, should it deem to be in the best interest of School District 99. While the financial responsibility of the vendor is a significant concern, the Board is equally concerned with the proven ability of the vendor to satisfactorily perform the contract so that service will be provided in accordance with proposed contract documents. School District 99 is not bound or committed to action or a decision based on this qualification/proposal specification, and may cancel proposal proceedings at any time prior to the awarding of a contract.
1. Please note that this is a request for qualifications and not a formal bid. As such, the Board of Education and administration are not bound by the lowest cost qualification/proposal, and can negotiate anytime during the process at its discretion but will not be unreasonable or unfair in its evaluation and review of qualifications/proposals.
2. So, the District may choose any firm/vendor/company whether it is the lowest or highest cost based on a number of criteria which the District will decide upon, including but not limited to: price, references, growth and overall architect/engineering/construction reputation, service reputation, product warranties and repairs (where appropriate), possible future capabilities, past work with the District and other viable considerations. B. Proposals will be considered firm for 90 days from January 13, 2016. References must be
provided.
C. School District 99 is exempt from the payment of federal excise taxes and the payment of state sales and use tax on all tangible, personal property for its use or consumption. Tax exemption certificates will be furnished upon request.
D. Inquiries may be made as to the financial stability of the firm, and the Board of Education may request financial references.
E. All firms/vendors/companies submitting an RFQ are required to sign and notarize, where applicable, firm/vendor/company forms/certificates in the Submittal Section (see pages 9 – 22). F. The District will not be liable for costs incurred in the preparation of the response to this RFQ or
in connection with any presentation before a selection committee.
G. Invoicing – Billing shall be monthly in one consolidated invoice for the entire Architecture Services, broken down by type of and location of Architecture Services, etc. Payment will coincide with District School Board Meetings. Invoices must be received three weeks prior to School Board Meeting in order to receive payment the next day after said Board Meeting. Upon request, please provide a sample invoice.
H. Insurance and other requirements:
1. Insurance Requirements: The successful firm/vendor/company shall obtain and keep in force during the term of the Contract, for the protection of the firm/vendor/company and School District 99, comprehensive Commercial General Liability Insurance in the combined single limit of not less than $1,000,000.00 including, but not limited to, Personal Injury Liability, Broad Form Property Damage Liability, Blanket Contractual Liability and Products Liability, and Business Auto Liability covering the operations and activities of the firm/vendor/company described in this document.
2. Professional Liability Insurance: Issued in a claims made basis for the term of the contract, with a limit of $1,000,000 with a tail coverage for two (2) years following completion of the contract.
3. The firm/vendor/company shall accept, in so far as the workers covered by this contract are concerned, the statutory provision of the State of Illinois Workmen’s Compensation Act, any supplements or amendments there to, and will insure its liability thereunder, and furnish proof thereof, to the School District or file with the School District a certificate of exemption from insurance from the Bureau of Workmen’s Compensation of the Department of Labor and Industry. In addition, the firm/vendor/company must purchase insurance from companies lawfully authorized to do business in the State of Illinois and that the insuring company maintains at least an AVII Key Rating from the A.M. Best Rating Guide.
4. The firm/vendor/company must furnish the district with Certificates of Insurance evidencing the existence of the above coverage and naming Cicero Board of Education, Cicero Public School District 99, and Cook County, Illinois as additional insured. 5. The insurance policy shall contain a covenant by the issuing company that coverage shall
not be cancelled unless a minimum of thirty (30) day prior written notice is given to School District 99. Notification shall be via certified mail and addressed to: Assistant Superintendent for Finance and Operations/CSBO, Cicero Public Schools District 99, 5110 West 24th Street, Cicero, Illinois 60804.
I. Any contract resulting from this RFQ is not assignable.
J. The successful firm/vendor/company agrees to indemnify, defend and hold harmless School District 99, its officers, agents and employees from any claims, damages, suits, actions, liabilities and costs of any kind or nature, including attorney’s fees, arising from or caused by the provision of any services, the failure to provide any services or the use of any services or materials furnished (or made available) by the successful vendor.
K. Any written contract resulting from this RFQ shall contain a severability clause, which provides that each paragraph and provision of the contract will be severable from the entire agreement and if any provision is declared invalid the remaining provisions shall nevertheless remain in effect.
L. If the successful firm/vendor/company is considered found to be wholly responsible for failure to in any way to perform as specified herein, School District 99 may consider the successful vendor to be in default. In the event of default, the District will provide the successful vendor with written notice of default, and the successful firm/vendor/company will be provided twenty (20) calendar days to provide a plan to correct said default.
If the successful firm/vendor/company fails to cure said default within twenty days, the District, among other actions, may complete the consulting/training etc. through a third party, and the successful vendor shall be responsible for any amount in excess of the agreement price incurred by the District in completing the consulting/training etc., to a completion equal to that specified in the contract.
M. The Compensation/Fee Structure form shall be fully completed (Additional pages may be added for further information).
N. The contract period shall be for the term as determined by the process.
O. The firm/vendor/company agrees and covenants that its agents and employees shall comply with all county, state and federal laws, rules and regulations applicable to the business to be conducted under the contract.
P. Firm/vendor/company must examine these specifications in detail as to the intent of the specifications. After the submission of the qualifications, School District 99 will not accept any complaint or claim that there was any misunderstanding in regard to items listed on the RFQ. Q. All firms/vendors/companies are responsible for understanding and complying with the
specifications detailed in this request for qualifications. All claims, offerings, and prices quoted in the qualification/proposal will be considered contractually binding.
R. Any questions regarding this RFQ must be referred to Asif Dada, Assistant Superintendent for Finance and Operations/CSBO, via email [email protected] and Robert B. Holmes, Business Manager/CSBO, via email [email protected] not later than 4:00 p.m. on Thursday, December 3, 2015. Use “Architect RFQ” in the subject line. Questions will be answered via addendum, which will be emailed to all participating firms/vendors/companies.
S. Termination of Contract: Cicero Public Schools District 99 reserves the right to terminate the contract/purchase order immediately in the event that the vendor discontinues or abandons operations; if adjudged bankrupt, or is reorganized under any bankruptcy law; or fails to keep in force any required insurance policies or bonds. Failure of the vendor to comply with any section or part of this contract will be considered grounds for immediate termination of the lease by School District 99.
T. Notwithstanding anything to the contrary contained in the contract between School District 99 and the vendor, School District 99 may, without prejudice to any other rights it may have, terminate the contract/purchase order for convenience and without cause, by giving 30 days written notice to the vendor.
U. If the termination clause is used by School District 99, the firm/vendor/company will be paid by the District for all scheduled consulting/work completed satisfactorily by the vendor up to the termination date set in the written termination notice.
II. Support and Training
A. The successful firm/vendor/company must provide weekly and monthly update reports and other detailed reports as required to School District 99 designated personnel.
B. All professionally-trained technical staff, must be proficient and knowledgeable for the specific assignment.
C. The firm/vendor/company will ensure adequate on-site training is provided where appropriate.
III. OTHER
A. Cicero School District 99 may wish to review the selection of particular individuals for the project team and interview these individuals before a final selection is made. For periods of on-site consulting/support individuals will be required to have a criminal background check performed and may be required to have a negative TB test as well as additional requirements at no cost to the District. Changes in key staff assignments will not be allowed without prior approval of Cicero School District 99. Cicero School District 99 reserves the right to terminate the agreement at any time if any of the staff assignments are unacceptable to Cicero School District 99.
B. Unless otherwise indicated in the proposal response, the selected firm/vendor/company must perform all work. Any work that is to be subcontracted by the firm/vendor/company will be subject to the terms and conditions of this RFQ and any resultant contract or purchase order awarded.

EVALUATION
In evaluating the qualifications/proposals submitted, School District 99 will apply the "Best Value" standard in selecting the firm/vendor/company to be awarded a contract for these projects. Price is not the sole criterion that will be used in the evaluation process. Any award resulting from this RFQ will be made to that firm/vendor/company whose offer conforms to the RFQ and is determined to be the most advantageous, of “best value” to the District, in the sole judgment of the District. The selection process will include, but not be limited to, the following considerations:

1. By signing and submitting this Certification, the prospective lower tier participant is providing the certifications set out herein.
2. If it is later determined that the prospective lower tier participant knowingly rendered an erroneous certification, in addition to other remedies available to the Federal government, the department or agency with which this transaction originated may pursue all available remedies, including suspension and/or debarment.
