Grand Rapids Ophthalmology was first in West Michigan to offer refractive surgery in 1985, first to offer Custom Wavefront LASIK, and first to offer IntraLase® – a completely BLADE-FREE LASIK procedure that makes a flap in the cornea with a computer-controlled laser instead of a blade for a more precise and predictable result.

Scheduling and Planning Your Appointment

Call our office at 616.949.2600 or 800.968.2600 Monday through Friday to schedule an appointment at any of our West Michigan locations. Evening and Saturday morning appointments are also available by request.

Most appointments, such as an initial consultation or complete exam, will take approximately 1-1/2 to 2 hours. Both eyes will be dilated for complete exams, laser vision and retinal evaluations. Dilation causes sensitivity to light and an inability to see things clearly at close ranges – such as words on a page. If your eyes will be dilated, take the precaution of having someone drive you home after your appointment.

Please bring your photo ID, insurance cards, Social Security number and a list of your current medications for your record. Insurance copay and/or out of pocket fees are due in full at the time of service. If patient is a minor, parent or legal guardian must be present at initial visit. A waiver may be signed for subsequent visits.

Please be considerate of our patient/doctor time when unable to keep your scheduled appointment; kindly give 48 hours notice. Failure to do so will result in a $35.00 fee.

For the consideration of others, please plan to arrive 10-15 minutes prior to your appointment time for administrative details regarding your visit. Patients arriving more than 15 minutes after their time may need to be rescheduled based on the specific schedule and day. Patients will be placed in the next available appointment with their doctor.

Billing and Insurance

Grand Rapids Ophthalmology participates with most vision and medical insurance plans. Vision plans cover exams specific to how your eyes see with glasses and/or contact lenses. Your medical insurance covers medical health issues affecting your eyes. Insurance co-pay and/or out-of-pocket fees are due in full at the time of service. If you have questions regarding your specific plan, feel free to call our billing department direct at 616.447.9450. Emergencies

In case of emergency, Grand Rapids Ophthalmology maintains physicians on call around the clock. After normal business hours our answering service will page the doctor to answer your call as soon as possible. If you are experiencing a life-threatening medical emergency, please call 911 or proceed to the nearest hospital.

Kenyon Kendall, DO

Dr. Kendall is a glaucoma specialist and utilizes the most modern no-stitch and small incision cataract surgery with implants including LifeStyle Lenses. He received his undergraduate degree from the University of Michigan, graduated from Michigan State Osteopathic School of Medicine, and completed a glaucoma fellowship at Kentucky Lions Eye Research Institute. He has been in private practice since 1987.

Scott Weber, MD

Dr. Weber’s expertise includes Custom and BLADE-FREE LASIK as well as cataract surgery with implants using the latest small-stitch incision and no-stitch techniques including LifeStyle Lenses. He also treats glaucoma and general eye care patients. Dr. Weber obtained his undergraduate degree from Notre Dame, graduated from Indiana University School of Medicine, and did his residency at Bethesda Naval Hospital in Maryland. He has been in private practice since 1987.

David Barrett, MD

Dr Barrett specializes in laser vision correction and small-incision cataract surgery. He has performed thousands of Custom and BLADE-FREE LASIK procedures. Dr. Barrett also performs the most advanced techniques in small-incision cataract surgery including LifeStyle Lenses. He received his undergraduate degree from Michigan State University, graduated from Wayne State Medical School and completed his Ophthalmology residency at the prestigious Jules Stein Institute at UCLA in Southern California. Dr. Barrett has been in private practice in West Michigan since 1990. Thomas Cowden, MD

Dr. Cowden is fellowship-trained in cornea/external diseases/ anterior segment and refractive surgery. He was involved in the initial FDA clinical trials for LASIK in 1996 and since has performed thousands of refractive procedures including Custom and BLADE-FREE LASIK. He also specializes in all forms of corneal transplant surgeries (including PKP, DSEK, DMEK, DALK, and Limbal Stem Cells) tailored to the patient’s unique needs. In addition, Dr. Cowden specializes in cataract surgery, utilizing the latest Intraocular Lens technology, lasers and peri-operative testing in order to customize his patient’s choice for optimal vision correction. Specialties also include advanced anterior segment reconstruction as a result of trauma, or congenital defects. He earned degrees from the University of Michigan, from Wayne State University School of Medicine, completed his Ophthalmology Residency at the University of Wisconsin, and his subspecialty Fellowship in Corneal and Refractive Surgery at the University of Minnesota. He has been in private practice since 1997.

Yosef Gindzin, MD

Dr. Gindzin is a fellowship-trained retinal specialist, treating retinal problems such as age-related macular degeneration, diabetes, retinal detachment and macular pathologies – often using advanced laser techniques and drug therapy. He completed his retina and vitreous surgery fellowship at the Cleveland Clinic, received his medical degree from the University of Michigan and undergraduate degree from the University of California at Irvine. Dr. Gindzin is conversant in Spanish and joined Grand Rapids Ophthalmology in 2002.

Marcus Muallem, MD

Dr. Muallem has a dual fellowship in cornea and refractive surgery, as well as glaucoma. Dr. Muallem’s sub specialty training in both fields were completed at the prestigious Bascom Palmer Eye Institute in Miami. His residency was completed at the University of Illinois in Chicago. Dr. Muallem performs Custom BLADE-FREE LASIK , Traditional LASIK and PRK. He also performs the most modern no-stitch and small incision cataract surgery utilizing traditional intraocular lenses, toric implants for astigmatism, and premium multi-focal lenses including LifeStyle Lenses. He specializes in diagnosis, treatment and surgical management of glaucoma and corneal diseases, including MIGS (minimally invasive glaucoma surgery), full thickness (PKP) as well as partial thickness (DALK, DSEK, DMEK) corneal transplantation. Dr. Muallem is well published in peer reviewed literature and has presented his research at national and international meetings.

Michael Boyle, MD

Dr. Boyle specializes in cosmetic and reconstructive surgery of the eyelids, face, orbit, area around the eye and lacrimal (tearing) system. He also performs BOTOX® Cosmetic procedures and works closely with Skin Solutions, our aesthetic skin care department, to provide patients with the best possible result following their eyelid procedures. Dr. Boyle completed a two-year Fellowship at the University of Alabama-Birmingham, and is a Fellow of The American Society of Ophthalmic Plastic and Reconstructive Surgery (ASOPRS). He is also board certified by The American Board of Ophthalmology. His residency and internship were completed at Indiana University, and he received his medical degree from the University of Illinois College of Medicine.

Dr. Gohel is a fellowship-trained retinal specialist treating retinal problems such as age-related macular degeneration, diabetes, retinal detachment and macular pathologies, using advanced laser techniques and drug therapies. Dr. Gohel received his bachelor’s degree from Michigan State University and his medical degree from Wayne State University School of Medicine. He completed an Internal Medicine internship at The Lahey Clinic-Tufts University, his ophthalmology residency at The University of Colorado, Rocky Mountain Lions Eye Institute and completed fellowships in Vitreoretinal Surgery at Associated Retina Consultants in Phoenix, Arizona and Henry Ford Hospital in Detroit, MI. Dr. Gohel was a vitreoretinal surgeon at Gunderson Lutheran Medical Center from 2010 until 2014 when he joined Grand Rapids Ophthalmology. Gilbert Vanderveen, MD

Dr. Vanderveen specializes in providing post operative care for cataract patients along with routine eye exams. In addition, he performs eye exams on diabetic patients with the goal of maintaining their vision. Dr. Vanderveen received his undergraduate degree from Calvin College, graduated from the University of Michigan Medical School & Ophthalmology Residency program and completed his oculoplastic surgery fellowship at New York Medical College. He has been in practice since 1979. Robert Roosenberg, MD

Dr. Roosenberg specializes in cataract surgery with implants, performing the most modern surgical techniques including no-stitch cataract surgery. In addition, Dr. Roosenberg was one of the first doctors in the United States to use LifeStyle Lenses. After graduating from Andrews University, he received his medical degree from Loma Linda and completed his residency at the University of Michigan. Dr. Roosenberg then performed three years of surgical residency at Butterworth Hospital and has been in private practice since 1979. Mark Sheldon, MD

Dr. Sheldon is a fellowship trained corneal specialist and sees patients for medical eye exams. In addition, he performs certain specialized procedures for post-cataract, post-refractive and unique surgical situations. We are proud to acknowledge him as the founder of the Cherry Street Eye Clinic, where he also performs pro bono eye surgery for the uninsured. Dr. Sheldon obtained his undergraduate degree from The University of Michigan and graduated from The Kresge Eye Institute of Wayne State University where he also completed a corneal fellowship. He has been in private practice since 1985.

Thomas Dunning, OD

Dr. Dunning specializes in primary eye care for all ages. He has a special interest in contact lenses, especially patients with “difficult to fit” eyes. He has participated in several contact lens and solution studies. Dr. Dunning also has participated in instructional study and treatment regarding dry eye syndrome. He is the current President of West Michigan On-Site Eye Care, and is a past Associate Clinical Professor for the Michigan College of Optometry and also past Vice President of the West Michigan Optometric Association. After completing undergraduate studies at Lake Superior State University he received his Doctor of Optometry degree at the Michigan College of Optometry. He has been in private practice since 1989. Gerard Choryan, OD

Dr. Choryan specializes in contact lenses, dry eye treatment and urgent eye care. His contact lens work involves people with all types of vision problems including astigmatism, keratoconus and corneal transplants. He has successfully treated many dry eye patients allowing most to return to contact lens wear or experience renewed ocular comfort. He is available for routine eye examinations and red-eye treatment involving foreign bodies, infections and trauma. Dr. Choryan received his Doctor of Optometry degree from Michigan College of Optometry. His undergraduate degree was completed at Ferris State University and he has been in private practice since 1990. Laura Piippo, MD

Dr. Piippo specializes in pediatric ophthalmology, treating children with crossed and lazy eyes as well as providing comprehensive eye exams for children. Dr. Piippo received her undergraduate degree at Lake Superior State University and her medical degree from Wayne State University School of Medicine. She completed her ophthalmology residency at the University of Illinois, Chicago and her fellowship in Pediatric Ophthalmology at Baylor College of Medicine. She joined Grand Rapids Ophthalmology in 2002. Tara Golisch, MD

Dr. Golisch specializes in treating glaucoma patients in addition to performing the most modern no-stitch and small incision cataract surgery utilizing traditional intraocular lenses, toric implants for astigmatism and premium multi-focal lenses. She received her undergraduate degree from Washington University in St. Louis and her medical degree from Robert Wood Johnson Medical School. Dr. Golisch completed her Ophthalmology residency at Wayne State University-Kresge Eye Institute, and her Glaucoma Fellowship at Cincinnati Eye Institute. Dr. Golisch joined Grand Rapids Ophthalmology in 2014.

9 10 Charlene Hamilton, OD

Dr. Hamilton provides routine eye care for patients of all ages. She has over 17 years of experience with contact lens patients along with providing pre- and post-operative care for laser and medical patients. Dr. Hamilton earned her Bachelor of Science and Doctor of Optometry degrees from Indiana University. She is a member of the American Optometric Association (AOA) and Michigan Optometric Association (MOA). Gregory Patera, OD

Dr. Patera provides routine eye care for patients of all ages and is certified in the treatment of external eye diseases, injuries and glaucoma management. Dr. Patera has 28 years of experience in custom contact lenses, including bifocal design, toric (for astigmatism), special purpose soft lenses and rigid gas permeable lenses. He attended Michigan State University and received his Doctor of Optometry degree in 1973 from Illinois College of Optometry. Dr. Patera practices at Grand Rapids Ophthalmology-Ionia. David Den Braber, OD

Dr. Den Braber obtained his undergraduate degree from Ferris State University. He received a Doctor of Optometry degree from the Michigan College of Optometry in 1992. He provides comprehensive eye care for all ages specializing in contact lenses and the treatment and management of ocular disease. He especially enjoys working with children and co-manages eye muscle dysfunction. Dr. Den Braber practices at Grand Rapids Ophthalmology-Ionia and at the East Beltline location in Grand Rapids.

Monica Fenton, OD

Dr. Fenton completed her undergraduate education at Arizona State University followed by her Doctor of Optometry degree at Michigan College of Optometry at Ferris State University. After completing her primary care residency at Hu Hu Kam Memorial Hospital in Arizona, Dr. Fenton returned to West Michigan and practices at Grand Rapids Ophthalmology-Holland. Rachel Hollenbeck, OD

Dr. Hollenbeck is a graduate of Michigan College of Optometry at Ferris State University. She is available for routine eye exams, contact lens patients, and pre- and post-operative medical exams. Dr. Hollenbeck practices at Grand Rapids Ophthalmology-Greenville. Lindsay Basler Weatherhead, OD

Dr. Basler is a graduate of Michigan College of Optometry at Ferris State University and Kalamazoo College. She completed a Primary Care residency with emphasis in Ocular Disease at Nova Southeastern University College of Optometry where she gained experience in all specialties including retina, glaucoma, cornea and neuro-ophthalmology. Dr. Basler is available for routine eye exams, pediatrics, contact lens patients along with pre- and post-operative medical exams. She is a member of the American Academy of Optometry, Fellowship of Christian Optometrists, Michigan Optometric Organization and Volunteers for Optometric Service to Humanity. Lewis DeKuiper, OD

Dr. DeKuiper provides primary eye care for the entire family. He is experienced in all aspects of contact lenses including premium lenses for astigmatism and presbyopia. He has a special interest in consulting with patients interested in laser vision correction and providing appropriate pre and post-operative care. Dr. DeKuiper also provides comprehensive management and treatment of ocular disease. He is a graduate of Calvin College and received his Doctor of Optometry from Illinois College of Optometry. Dr. DeKuiper is a member of the Michigan Optometric Association and the American Optometric Association. He is past president of West Michigan Optometric Association. Dr. DeKuiper was in private practice in Holland before joining Grand Rapids Ophthalmology.

Annessa Dix, OD

Dr. Dix received her Doctor of Optometry degree from Michigan College of Optometry in 2004 and provides medical based eye care, specializing in retina and glaucoma management. In addition, she is also available for routine eye exams. Dr. Dix joined Grand Rapids Ophthalmology in 2011 and practices primarily at the Walker office with our retina team.

Angela Huizenga, OD Dr. Huizenga completed her

undergraduate education at Ferris State University where she received a Bachelors degree in Vision Science. She graduated from the Michigan College of Optometry in 2008. While in optometry school, she completed externships in pediatrics, binocular vision, specialty contact lens fitting and primary care. She then completed a one year residency specializing in ocular disease at the Eye Center of Texas where she evaluated cataract, LASIK, glaucoma, cornea and retinal patients. She completed three VOSH mission trips to the island of Dominica and joined Grand Rapids Ophthalmology in 2012. Abigail Veldkamp, OD

Dr. Veldkamp received her Doctor of Optometry degree from the Michigan College of Optometry after graduating from Calvin College. She specializes in complete eye care for pediatric patients, including routine exams, binocular vision, and contact lenses. In addition, she is also available to provide comprehensive adult eye care. Dr. Veldkamp is a member of the American Optometric Association, the Michigan Optometric Association, and the West Michigan Optometric Association. She practices at various Grand Rapids Ophthalmology locations. Shannon Stanek, OD

Dr. Stanek received her undergraduate degree at Michigan State University and a Doctor of Optometry from The Michigan College of Optometry in 2004. She provides complete eye care for the entire family. She has completed advanced training and education in fitting specialty and unique contact lenses, including RGP, Hybrid and Scleral lenses. Dr. Stanek is a member of the West Michigan Optometric Association and practices at the East Beltline office, as well as various other Grand Rapids Ophthalmology locations.

12 Holly Hoffman, OD

Dr. Hoffman is available for routine eye exams for all ages, contact lens patients, management of ocular disease and pre- and post-operative medical exams. She received her Doctor of Optometry at The Michigan College of Optometry in 2010 after receiving her undergraduate degree at Calvin College. Dr. Hoffman served as an optometrist in the United States Air Force in Texas for four years before moving back to West Michigan to join Grand Rapids Ophthalmology. She is a member of the American Optometric Association, Michigan Optometric Association and a Fellow in the American Academy of Optometry. Andrew Wilke, OD

Dr. Wilke provides eye care for the whole family, from routine eye exams and specialty contact lens fit to the management of ocular disease and low vision services. In addition, he provides pre- and post-operative care for surgical patients.
