APPENDIX C

GUIDELINES & EXAMPLES FOR CAPSTONE FIELD EXPERIENCE PROPOSAL

The Capstone Field Experience Proposal is a one-page document written prior to meeting with the Capstone Faculty Advisor and beginning the Field Experience. This proposal is submitted to the Capstone Supervisor along with the signed Community Partner Agreement. The student and the Community Partner then ascertain the type of activities that the agency will be able to provide for the student to meet their objectives. This document serves as an overview of how the student will gain an understanding of a public health agency, its programs, the population it serves, and potential projects. Below is the format for the Capstone Field Experience Proposal:

Introductory Paragraph

Brief description of why you chose this site; what population it serves; and the public health significance of their work.

Objectives

Example 1. By completion of the field experience the student will be able to describe the organization and

operations of the agency; describe the services provided, population served, and the public health significance of the work.

Example 2. An example of what the student should be able to accomplish is planning, implementing and

evaluating a smoking cessation program for Hispanic employees. Sample Activities

1. Review the mission and policies governing the agency and read program manuals.

2. Participate in an orientation program that includes spending time in selected departments. Attend staff meetings, conferences, etc.

3. Schedule meetings with administrators, Community Partners and other relevant staff. 4. Gain an understanding of the population and communities served

5. Identify areas of deficit or need in terms of programs/services provided 6. Collect information about smoking in the general Hispanic population.

7. Conduct a survey to determine the magnitude of the problem at this site and interest in employee participation.

8. Develop a program plan, objectives, methods and evaluation. 9. Assemble materials and equipment needed for implementation. 10. Schedule meeting and present program

11. Implement program. 12. Assess program results.

CAPSTONE FIELD EXPERIENCE PROPOSAL EXAMPLES: Example #1

Introduction

I would like to do my Capstone Field Placement with the Miami Dade County Health Department’s Office of HIV/AIDS. Specifically, my capstone field placement hours will be focused on the “Test Miami” initiative. I chose this project because of my interests in learning more about the processes of health social marketing - an important component of public health advocacy.

The agency’s Office of HIV/AIDS is the largest county HIV/AIDS health department in the state of Florida. They provide a variety of programs that range from prevention and health education to surveillance and patient care services. Many of these services are funded by federal and state revenues and grants. Because of South Florida’s diverse population, the Office of HIV/AIDS places an emphasis on reducing health

disparities in minorities; one of the main reasons for which I would like to work with this particular agency. Objectives

By completing the field placement, I will have collaborated with the Miami Dade County Health

Department’s HIV/AIDS Program Coordinator to collect, analyze, and interpret data for the “Test Miami” social marketing campaign. Recommendations for improvement will also be created, reported, and presented to the Health Department. My faculty advisor, Dr. Lisa Metsch, will also be supporting and guiding my experience.

Activities

1. Gain insight into the daily operations of a local/county public health organization 2. Observe and participate in multisectoral collaboration and cooperation

3. Observe and evaluate HIV/AIDS prevention and treatment programs 4. Engage in the sharing of ideas in a group setting

5. Encourage and promote the development of evidence-based and cost-effective HIV/AIDS prevention and treatment programs

Example #2 Introduction

My main goal in doing my Capstone Field Experience with the UM HELC is to understand and assess the importance of this agency in Miami communities. By helping to implement a case tracking database, the impact on public health and public health economics can ultimately be measured. In turn, these outcomes can be used to inform Health Services Administration and Policy.

(13)

MPH and MD/MPH Degree Program

Revised 2014-02-20 Miller School Of Medicine (University Of Miami) 13 Objectives

During the Field Experience, I will gain an understanding of the mission and goals of the UM HELC, by observing the casework performed on behalf of underserved clients from the local community. By working with UM HELC staff, I will learn about the services provided to clients, and in turn, I will understand how this agency ultimately affects clients’ health care outcomes. By the end of the field placement, I will have contributed to UM HELC by helping to create a database for tracking all case information, including case results and the economic impact resulting from favorable case outcomes.

Activities

The following activities will serve to complete the Capstone field placement objectives:

1) Study the UM HELC handbook for incoming law students to learn about the history, mission and services offered by UM HELC.

2) Observe the activities performed by HELC students and supervising attorneys 3) Attend morning rounds to learn about case status updates and HELC operations 4) Assist with interpretation for Spanish-speaking clients

5) Gather data from closed case files in storage to fill in missing data from existing case log 6) Create a workable database which can be used to analyze UM HELC data and outcomes 7) Enter existing data into the HELC database from the firm management software (AMICUS) 8) Enter archived data from storage files into HELC
