Step 1 : Getting started

Verify if you are eligible for Medicare by checking this

EHR Financial Incentive Fact Sheet on OHIP’s Physician & Provider page.

Step 2: Login instructions

Go to https://ehrincentives.cms.gov to begin the registration process.

Enter your National Plan and Provider Enumeration System (NPPES) web user account, user ID and

Enter your business address, phone, email. Your

business address needs a zip code plus 4 digits, and you may not enter a P.O. box.

 Data is pre-populated from the NPPES system.  If you make changes to address and phone, the

information does not go back to NPPES.

Eligibility requirements

• Determine your eligibility.

• For quick fact sheets, go to the physicians’ page on the OHIP home page and click on Eligible Professionals. If you’re eligible for both Medicare and Medicaid, you need to decide when you register, which one you want to choose.

• Before 2015, an eligible professional may switch programs only once after the first incentive payment is initiated.

• Most eligible professionals will maximize their incentive payments by participating in the Medicaid EHR Incentive Program.

PECOS numbers

• If you’re an eligible professional, you must have an

enrollment record in the Provider Enrollment, Chain and Ownership System (PECOS) in order to receive a

Medicare Electronic Health Record (EHR) incentive

payment.

• If you are a physician working in a group, you need your own individual number to receive the incentive

payment directly.

Appropriate SS/Tax ID, EIN

• Have your Social Security number ready to input as an individual for your Tax ID Number (TIN).

• If your group is to receive the payment, it goes to the Employer Identification Number (EIN).
