We’re writing on behalf of the Veterans First Program at Las Positas College and the Cycling 4 Veterans (C4V) volunteer group organizers. We’re seeking donations for the C4V cycling event which will be starting and ending at Las Positas College in Livermore, CA on Saturday October 17, 2015. The C4V event will include competitive and recreational riders, and it is also a family event with refreshments and entertainment at the campus. C4V has great potential and our goal is to make it a key cycling event in the tri-valley area, along with it being an important fund raiser for Veteran and active military organizations.

The goal of C4V is to raise funds that will directly go to support a group of Veteran and active military organizations. Funds will be directed into various services and programs such as the Las Positas College Veterans First Program, Sentinels of Freedom, Valley Veterans Foundation, Blue Star Moms – Danville Chapter, and Pleasanton Military Families (PMF).

ABOUT THE VETERANS GROUPS/ORGANIZATIONS – All groups are 501(c)(3) organizations

Las Positas College Veterans First – Provides services and programs for students who are Veterans, active duty, reservists, and spouses/dependents of Veterans. Programs and services range from Veterans Resource Center on campus to a Veterans text book program.

Las Positas College Student Veterans Organization (SVO) - LPC student Veteran are automatically members of the SVO. The SVO is committed to providing support to our veterans, school, and community.

Sentinels of Freedom - Provides 2-4 year, multi-faceted scholarships to severely wounded post 9/11 veterans who pursue higher education after transitioning from the military.

Valley Veterans Foundation – Offer scholarships to student veterans and honor our hereos returning home with "Welcome Home" celebrations!

Rotary Assisting Veterans Education – RAVE’s goal is to counsel veterans to confirm the veteran is enrolled at Los Positas College in G.I. Bill approved academic and vocational programs.

Blue Star Moms of Danville - The Blue Star Moms, Chapter 101, is a support through service organization of moms from all over Contra Costa and Alameda Counties, joining together to share pride, devotion and worries for their sons and daughters currently serving in all branches of the Armed Forces of the United States, and their sons and daughters who are now Veterans.

Pleasanton Military Families (PMF) – Is a support through service organization of family members with sons and daughters in the active military. In addition, they provide volunteer services to multiple Veteran

Purpose of Form

A person who is required to file an information return with the IRS must obtain your

correct taxpayer identification number IlN) to report, for example, income paid to
