1. Complete the North Shore Bank Checking Account Switch
Kit Information Sheet.
Once the Information Sheet is completed (including Step 4
which will tell us which of our locations is most convenient
for you), drop it off at any branch or mail it to our Retail
Banking Division.
2. Set up a meeting with one of our Customer Service
Representatives.
Once we receive your Information Sheet, we will have a
Customer Service Representative from the North Shore Bank
location that is most convenient for you, contact you to set
up an appointment. At this meeting, we will open your
account and On-line banking, as well as help you set up
Direct Deposit to your new account and switch your
automatic payments.
3. Stop using your former account and begin using your new
North Shore Bank account.
Be sure to leave sufficient funds in your former account until
all of your checks have cleared and any automatic
withdrawals have been successfully transferred to North
Shore Bank.
4. Change your Employer Direct Deposit to North Shore Bank.
Use the attached Employer Direct Deposit Form to change
any direct deposits. Remember to attach a voided North
Shore Bank check to the form.
5. Change your Automatic Payments or Withdrawals to
North Shore Bank.
Use the attached Automatic Payment or Withdrawal
Authorization Form to change any automatic payments or
withdrawals. Our Automatic Payment Checklist will help you
keep track of the payments that need to be switched.
6. Close your former account at the other institution.

When you come into the Bank, please bring your driver’s license or other form of photo identification, your opening deposit ($10 minimum), and the enclosed Employer Direct Deposit Form, Automatic Payment Check List and Automatic Payment Authorization Form. If you have any questions, please call a Customer Service Representative at 877-712-7000.
