Circle ONE time REGARDLESS OF EXEMPTION STATUS Details in the CAP are outlined in the Student Handbook. By signing this form, student/

parent agree to abide by these assurances and understand that they will be held responsi- ble for paying for discrepancies related to violating the assurances agreed to on this CAP form. Feel free to contact administration fro clarifi cation or questions about the CAP.

1. Submit a CAP form and proof of a complete schedule, in person, on time, EACH semester to be enrolled at NSHS. NSHS will then send tuition payment to the colleges or classes may be dropped and/or charged to the student.

-All courses, including 4, 6, or 8 week sessions must be on the CAP and on the proof of schedule at the start of the semester.

-Students must obtain NSHS approval for an extra course approval and will be fi nancially responsible for any courses added during the semester.

2. Follow the NSHS HS Academic Plan for the allowable number of college courses or incur fi nes for the courses and/or disciplinary action.

3. Maintain the required number of courses each semester to be enrolled at NSHS (6 classes for 11th & 4 classes for 12th). Winter courses are counted as part of the spring semester and the 2-Week Course is counted as part of the fall semester.

-COMPASS does not count as a class but is required for 2nd year students to meet NSHS graduation requirements.

-Science classes with a lab are considered ONE class

-Students are fi nancially responsible for labs taken without a concurrent lecture course (must be approved on Extra Class Appeal Form).

4. Submit a new CAP form for all schedule changes.

5. Students are also responsible for college fees incurred for course changes made aft er the colleges’ last day to add/drop without fees (dates determined by colleges).

6. Ensure the classes listed on the CAP form match the courses completed at the end of semester grades or student/parent must pay for both class discrepancies.

7. Students must submit a .pdf of all college transcripts through the NSHS ticket system or in person to NSHS within three days aft er semester grades post. If grades are not submitted, they are treated as withdrawn or incomplete. Incomplete or unapproved withdrawn courses are coded as Fs on the high school transcript and incur course fi nes.

8. Maximize your college courses. Complete an Extra Course Appeal form to obtain written permission from NSHS to take and pay for extra courses (including mid-semes- ter starting courses) See Extra Class Appeal Checklist at the bottom right.

9. Continue taking math courses (by obtaining a C or better) each semester. Math exemptions are available under limited circumstances highlighted in the handbook. See the Math Exemption Checklist.

10. Study Skills is mandatory each semester. Passing the class or meeting exemption expectations are required to earn a diploma from NSHS. Eligible students may request an exemption. Exemptions are a PRIVILEGE and may be revoked at any time. See Study Skills Exemption Checklist to the right.

11. NSHS may pull payment to the colleges on students with no CAPs or unapproved courses. NSHS is not responsible for college actions which may include being purged from classes, billing the students, sending them to collections, or keeping them enrolled in college courses, which may result in Fs on the permanent college transcript.

12. Students who withdraw from NSHS during the semester are responsible for with- drawing from and paying the fi nes for their semester college courses. If students neglect to withdraw from their courses or withdraws too late, they risk receiving failing grades on their college transcripts.

13. NSHS will notify students/parents when high school semester grades are available for pick up at the school. Parents must pick up grades regardless of student age.

14. A signed CAP form from NSHS does not guarantee students are on track to graduate from NSHS. It is for payment purposes only.

15. Students who are late or miss course verifi cation documentation or fall under the required number of courses may be charged for their courses, for adding extra courses to become eligible, or withdrawn.

16. NSHS has extra graduation standards including being out of remedial college math and passing Study Skills, Transitions and COMPASS.

17. Do not pay the college unless noted by NSHS. NSHS pays the colleges directly for fall, spring, and winter. If a student has approved extra courses, NSHS pays the college and the student pays NSHS. Students are responsible for summer tuition.

18. NSHS does not have to take unapproved college courses for dual credit and cannot take 300-400 level courses or 1-2 credit courses for dual credit.

Type II – Enrichment Class Appeal:

Type III – Upper Level Course Appeal:

NSHS does not reimburse for textbooks for Type II, II or IV appeals. These students will have their study skills exemption revoked, and be limited on the number of college courses for future semesters.

By signing the form, parents and students acknowledge that they have followed the NSHS Academic Plan and agree to pay NSHS for the applicable extra course fees. All approved appeals must be granted in writing by NSHS and recorded on and attached to the CAP form.
