Six Sigma Black Belt Certificate Requirement:

To successfully complete the Six Sigma Black Belt Certificate Program, a participant must attend all four blocks of one-week workshops within a two-year period; complete all work assigned; and be present for all class activities. Each workshop will be scheduled at least once a year.

Each participant must also complete a Six Sigma Black Belt Application Project. The application project should be selected from your workplace to acquire hands-on knowledge and demonstrate comprehension of the Six Sigma Black Belt tools and methodology. The projects will be selected by the participants and approved by the instructor. The instructor will facilitate and provide consultation for the project in class during the workshops as part of the certificate program workshop activities. Workplace project consultation may be arranged through

Note: The Six Sigma Management Courses are not a part of the certificate programs and are offered to provide managers, supervisors, team members, and key organizational personnel involved in the six sigma process an opportunity to acquire specific knowledge and skill sets.

DONALD SMITH, MBA, SIX SIGMA MASTER BLACK BELT,

is the CEO/President and Founder of Donald E. Smith Associates, Inc., an international business management consulting firm specializing in Lean Six Sigma Process Improvement, Total Quality Management, and Leadership Development. Don has extensive experience in directing leadership teams at all organizational levels in the planning, design, and implementation of business process excellence systems, including Six Sigma and Lean Enterprise.

Don is an accomplished and certified Six Sigma Master Black Belt and Black Belt facilitator who has coached process improvement teams at numerous companies across the globe. An active member of the American Society for Quality, Don is a charter member of the Service Industries Division and the Past Regional Counselor for Region 15. He is also a member of the Quality Management Division and the Human Resources Division of the ASQ. Don is a certified Total Quality Management and Quality Action Team facilitator. He has designed and delivered several internal and external workshops stressing TQM awareness and application. At ease in multicultural environments, he has provided services to customers in Australia, Austria, China, the Czech Republic, France, Germany, Holland, Hungary, Ireland, Scotland, Singapore, South Korea, the United

Kingdom, and throughout the United States. Mr. Smith holds an M.B.A. from Texas, A&M in Commerce, Texas. He is a recipient of the 2007 Teaching Excellence Award from the University of Alabama, College of Continuing Studies. He is among one of the first to be certified in The Great Discovery, Coach and Facilitator, The Fourth Generation of Six Sigma in 2008.

DIANNA SMITH, MBA, SIX SIGMA MASTER BLACK BELT,

is Chief Operating Officer and part owner of Donald E. Smith Associates, Inc., a business management consulting firm specializing in process improvement initiatives using the Lean Six Sigma Methodology. She is a multi-talented consultant and senior management leader with over 30 years of experience working in industry. She is a dynamic, energetic individual whose expertise in understanding and managing the dynamics of change has saved companies millions of dollars. Ms. Smith is skilled at applying Six Sigma and Total Quality Management principles to corporate process improvement initiatives and meeting facilitation, interventions, project management, team participation, strategic planning, training development and delivery. Dianna delivers Six Sigma Process Improvement courses both Green Belt and Black Belt, Champions Training and numerous leadership courses. She provides application support and coaching of projects. Companies that she has worked with are: Compass Bank, NCR, First Data Resources, First Data Merchant Services, Corning, Longaberger, Color Spot, TRW, KPMG, BellSouth Telecommunications, Jefferson State Community College, and others. She is adept at applying Six Sigma and Total Quality Management principles to corporate process improvement initiatives and process re-engineering teams, delivering courses on

She led a massive re-engineering effort to outsource non-core processes for a large telecommunications company resulting in $22 million saved.

Ms. Smith holds an M.B.A. from the University of Alabama at Birmingham (UAB). She is a Cum Laude graduate of UAB in Business Management and a member of both the Golden Key National Honor Society and the Beta Gamma Sigma National Business Honor Society. Dianna studied Statistical Methods for Quality Improvement at The University of Alabama in Tuscaloosa. Dianna was selected as one of 2,000 Notable American Women in 1996. She is a recipient of the 2007 Teaching Excellence Award from the University of Alabama, College of Continuing Studies. She is among the first to be certified in The Great Discovery, Coach and Facilitator, The Fourth Generation of Six Sigma in 2008.

JONATHON ANDELL, SIX SIGMA MASTER BLACK BELT,

has been a quality professional since 1987, a consultant since 1998, and a Fellow of the American Society for Quality since 2008. As his career advanced, he has implemented a variety of Lean (and Six Sigma) initiatives. He is equally effective leading strategic planning sessions with executives, providing training and coaching for technical staff, and implementing hands-on improvements on the work floor.

He has streamlined processes and operations at such diverse organizations as manufacturing facilities, hospitals, blood banks, construction firms, software developers, design firms, and a variety of service organizations. He is renowned for his ability to diagnose a situation or organization quickly, and to hone in on the most value-added interventions.

Note: The first two weeks of the Black Belt program also count as the Green Belt Certificate Program.

WHO SHOULD ATTEND:

These Six Sigma Certificate Programs are designed for executives, managers, business managers, engineers, financial managers, supervisors, quality improvement personnel, team members, leaders, and change agents participating in the Six Sigma process improvement at any level. The Six Sigma process improvement is applicable to service, manufacturing, production, telecommunications, travel, and numerous other industries.

GENERAL INFORMATION

REGISTRATION

The cost for attending each certification and management course varies and is specified on the Registration Form in this brochure. The fee covers the cost of the sessions, handout materials, refreshment breaks, and luncheons.

Organizations registering three or more persons for the same course listed in this brochure will receive a 10% discount. Please specify attendance preferences on the Registration Form. Participants who register less than five working days prior to the session starting date should do so by phone to ensure confirmation of their registrations. Treasury regulations may permit an income tax deduction for educational expenses (registration fees, travel, meals, and lodging) undertaken to maintain or improve professional skills.

SCHEDULE OVERVIEW

Registration check-in process will be from 7:30 - 8 a.m. on the first day of each program, with instruction commencing thereafter, and wrapping up at 5:00 p.m. On the following days, instruction lasts from 8 a.m. - 5 p.m. (class will end at 3 p.m. on the last day of the program). Lunch is from noon to 1 p.m.

ALLSIX SIGMA CERTIFICATION AND MANAGEMENT COURSE sessions will be held in the Bryant Conference Center, a premier learning facility located on The University of Alabama campus in Tuscaloosa, Alabama, at 240 Paul W. Bryant Drive.

Sports fans - make plans to visit the Paul W. Bryant Museum ($2/person) located between the Conference Center and the Hotel Capstone.

PARKING

From I-20/59, take the McFarland Boulevard/US 82 exit and proceed north to the University Boulevard interchange. Exit and turn right. Stay on University Boulevard down the hill and turn left on Second Avenue. Open parking will be on your left, the Bryant Conference Center complex on your right. In addition to the parking lot on Second Avenue, you may also park in the Coleman Coliseum Parking Lot. You will be able to ride a Crimson Route bus to Second Avenue at the Bryant Conference Center.

COURSES may provide the Professional Development Hours (PDH) required by State regulations for maintaining Continuing Professional Competency. Please check with your appropriate state agency for this requirement.

CANCELLATIONS and REFUNDS

If you must cancel, a full refund may be granted up to two weeks prior to program date. After that date, an administrative fee equal to 25% of the registration fee (minimum $25) will be charged against your refund. Refunds cannot be made after the program has begun, but a substitute participant may be designated. The University of Alabama reserves the right to cancel, postpone or combine class sections; to limit registration; or to change instructors. Any money-back guarantee offered on specific programs does not necessarily apply to other programs of The University of Alabama College of Continuing Studies. Note: Participants who have taken the two-week Green Belt program or the Black Belt program may sign up for additional coaching and project oversite through The University of Alabama. The projects must be completed before the Six Sigma Green Belt or Black Belt is awarded.

Certificate Description:

This Six Sigma Green Belt Certificate Program will enable you to implement all of the five-step methodology in the process improvement of DMAIC (Define, Measure, Analyze, Improve, and Control). This program is completed in two separate weeks of training. The Green Belt candidate will bring an improvement project to class to apply the method as the tools and techniques are taught.

Laptop computers and Minitab Statistical software will be provided for the Green Belt workshops.

You will take a comprehensive exam at the end of each week to assess competence in the use and application of the tools and techniques you will have learned. To be certified as a Green Belt, you must present and defend a completed project.

To help equip you to be a successful project team leader, you will also learn and practice group facilitation skills. At the end of this two-week period, you will be able to lead small-scale projects as well as support any Black Belt projects.

Note: Participants who have taken the two-week Green Belt program or the Black Belt program may sign up for additional coaching and project oversite through The University of Alabama. The projects must be completed before the Six Sigma Green Belt or Black Belt is awarded.

Certificate Description:

This Six Sigma Green Belt Certificate Program will enable you to implement all of the five-step methodology in the process improvement of DMAIC (Define, Measure, Analyze, Improve, and Control). This program is completed in two separate weeks of training. The Green Belt candidate will bring an improvement project to class to apply the method as the tools and techniques are taught.

Laptop computers and Minitab Statistical software will be provided for the Green Belt workshops.

You will take a comprehensive exam at the end of each week to assess competence in the use and application of the tools and techniques you will have learned. To be certified as a Green Belt, you must present and defend a completed project.

To help equip you to be a successful project team leader, you will also learn and practice group facilitation skills. At the end of this two-week period, you will be able to lead small-scale projects as well as support any Black Belt projects.

GENERAL INFORMATION

REGISTRATION

The cost for attending each certification and management course varies and is specified on the Registration Form in this brochure. The fee covers the cost of the sessions, handout materials, refreshment breaks, and luncheons.

Organizations registering three or more persons for the same course listed in this brochure will receive a 10% discount. Please specify attendance preferences on the Registration Form. Participants who register less than five working days prior to the session starting date should do so by phone to ensure confirmation of their registrations. Treasury regulations may permit an income tax deduction for educational expenses (registration fees, travel, meals, and lodging) undertaken to maintain or improve professional skills.

SCHEDULE OVERVIEW

Registration check-in process will be from 7:30 - 8 a.m. on the first day of each program, with instruction commencing thereafter, and wrapping up at 5:00 p.m. On the following days, instruction lasts from 8 a.m. - 5 p.m. (class will end at 3 p.m. on the last day of the program). Lunch is from noon to 1 p.m.

ALLSIX SIGMA CERTIFICATION AND MANAGEMENT COURSE sessions will be held in the Bryant Conference Center, a premier learning facility located on The University of Alabama campus in Tuscaloosa, Alabama, at 240 Paul W. Bryant Drive.

CANCELLATIONS and REFUNDS

If you must cancel, a full refund may be granted up to two weeks prior to program date. After that date, an administrative fee equal to 25% of the registration fee (minimum $25) will be charged against your refund. Six Sigma Black Belt Certificate Requirement:

To successfully complete the Six Sigma Black Belt Certificate Program, a participant must attend all four blocks of one-week workshops within a two-year period; complete all work assigned; and be present for all class activities. Each workshop will be scheduled at least once a year.

Each participant must also complete a Six Sigma Black Belt Application Project. The application project should be selected from your workplace to acquire hands-on knowledge and demonstrate comprehension of the Six Sigma Black Belt tools and methodology. The projects will be selected by the participants and approved by the instructor. The instructor will facilitate and provide consultation for the project in class during the workshops as part of the certificate program workshop activities. DONALD SMITH, MBA, SIX SIGMA MASTER BLACK BELT,

is the CEO/President and Founder of Donald E. Smith Associates, Inc., an international business management consulting firm specializing in Lean Six Sigma Process Improvement, Total Quality Management, and Leadership Development. Don has extensive experience in directing leadership teams at all organizational levels in the planning, design, and implementation of business process excellence systems, including Six Sigma and Lean Enterprise.

Don is an accomplished and certified Six Sigma Master Black Belt and Black Belt facilitator who has coached process improvement teams at numerous companies across the globe. She is a multi-talented consultant and senior management leader with over 30 years of experience working in industry. Ms. Smith is skilled at applying Six Sigma and Total Quality Management principles to corporate process improvement initiatives and meeting facilitation, interventions, project management, team participation, strategic planning, training development and delivery. JONATHON ANDELL, SIX SIGMA MASTER BLACK BELT,

has been a quality professional since 1987, a consultant since 1998, and a Fellow of the American Society for Quality since 2008. As his career advanced, he has implemented a variety of Lean (and Six Sigma) initiatives. He is equally effective leading strategic planning sessions with executives, providing training and coaching for technical staff, and implementing hands-on improvements on the work floor.

He has streamlined processes and operations at such diverse organizations as manufacturing facilities, hospitals, blood banks, construction firms, software developers, design firms, and a variety of service organizations.
