RN EVALUATION PROCESS
MONTH PRIOR TO EVAULATION (Seniority) MONTH:
The month prior to a staff member’s evaluation month (seniority month), staff will receive an e-mail notice from the unit’s AA that their annual self-evaluation is due.
Within the e-mail evaluation notice, staff will receive the RN Evaluation Procedure (Sample below)
RN WHO RECEIVES THE E-MAIL NOTICE: Follow the procedure as listed on the RN Evaluation Procedure and on the cover page of the evaluation.
Retrieve the correct evaluation for your level and save to your H drive or other location of your preference
Retrieve the peer feedback document(s). Domains are separate documents.
Select and request a minimum of 3 peers to perform peer review.
Those selected must be educated in the peer review process. At least one peer must be an RN.
Each nurse will be asked to evaluate you on the selected domains. All domains should be reviewed by peers.
You will send the selected peers the domain documents for feedback electronically (via e-mail),
The Clinical Skills and Knowledge domain must be completed by an RN.IT IS TIME FOR YOUR ANNUAL EVALUATION
You can find evaluation tools and peer input tools at the JIT website . Be sure to SELECT THE PERFORMANCE EVALUATION FOR YOUR LEVEL.

Key Dates to remember – the Peer input should be returned to you & your manager electronically within 7 days.
You are to review the content and summarize the information on the performance review form in the
peer feedback section of the self-evaluation.
Peer feedback that is not summarized and included within your performance review form will not be typed in for you.
Rationale for rating other than “met,” must be provided in the evaluation summary section
Self-performance review is to be submitted electronically to the manager or designee (it will not be typed into the performance review for you).
Upon receiving notification of the annual evaluation, you have Three Weeks to complete a self-evaluation (including peer feedback). You will submit your completed Staff Performance Planning and Evaluation form including a summary of the Peer Review responses via e-mail to the manager (or designee) by (insert date here).
If materials are not submitted within two weeks of the due date, the manager may proceed with completing the evaluation process.

Be sure to include an Annual Plan (Goals) for the next year
Goals should be Specific, Measurable, Achievable, Realistic, and Timed. i. Example: I will become an ONS member by July 20XX.1. This goal is
a. specific – becoming an ONS member,
b. measurable – either achieved or not achieved, c. achievable – not hard to do
d. realistic – nurse should be able to do this e. time – there is a date listed
PEERS WHO ARE ASKED TO PROVIDE FEEDBACK:
The peers selected will use the Peer Input tool forward by the Evaluatee RN.
Peers should indicate the appropriate behavioral level they feel the Evaluatee RN displays the majority of the time.
Peers should describe an example of when they feel the Evaluatee RN was at their best. Be as concrete as possible.
Peers should provide opportunities for growth for the Evaluatee RN. Be as concrete as possible.

Peers should return the completed feedback electronically (via e-mail) to the Evaluatee RN and to the manager/designee within 7 days
RN COMPLETING SELF EVALUATION:
The Evaluatee will complete the level (A, C, D, E, RSAM) appropriate self-evaluation and submit electronically to the manager or designee (via e-mail).
The Evaluatee will consider the input provided by the peer evaluation(s) when completing their self-evaluation and will summarize the peer feedback on the performance review form in the Required Peer Feedback Section.
Supportive responses for behaviors will be included in the summary section(s).
Upon receiving notification of the annual evaluation, the Evaluatee nurse has Three Weeks to complete the self-evaluation (including peer feedback) process.

MANAGER COMPLETING EVALUATION
The manager will review the peer review forms, peer summaries and Evaluatee’s self-evaluation prior to completing the manager section of the evaluation form within the anniversary month of the nurse.
The manager will utilize peer and self-evaluations as well as own knowledge of employee performance in determining ratings on the Performance Planning and Evaluation form.
Supportive responses for behaviors will be included in the summary section(s) including rational for other than “meets expectations”.
The manager will arrange an appointment to meet with the nurse.
Sending the completed evaluation to the Evaluatee nurse electronically for review prior to the meeting will allow conversation to be focused on goals. Note: Any area of the evaluation the employee or manager would like to discuss more in-depth will also occur during the meeting.
If materials are not submitted within two weeks of the due date, the manager may proceed with completing the evaluation process.
A signed copy will be given to the nurse after the evaluation meeting.
The manager will enter the date completed and overall performance rating into the evaluation log.
