Sacred Heart Mission (SHM) works with people whose capacity to participate fully in community life is affected by deep, persistent disadvantage and social exclusion. PROGRAM INFORMATION
Sacred Heart Mission (SHM) is made up of four Divisions, Client Services, People and Strategy, Business Development and Business Services. Within the Client Services Division, programs are organised across two streams of services, as per the SHM service model: Engagement Hubs and Individualised Planned Support, and Ongoing Support.
Engagement Hubs
The Engagement Hub services at Sacred Heart Central and the Women’s House provide people with a safe space that is welcoming and supportive. Hub services provide access to the necessities of life: healthy food, personal hygiene, laundry facilities and primary health support through our GP Clinic.
The Hubs also facilitate access to ongoing support through the provision of information and advocacy and our case management services which include referral to specialist services (mental health and AoD) as required. For clients with a limited social support network, Engagement Hubs provide social

participation and pathways out of social exclusion. The Hubs are a place to build social connections and to link with professional service providers (such as lawyers, nurses and vets) or to range of health and wellbeing practitioners through our Wellness Place service.
Individualised Planned Support
Individualised Planned Support is an outcome focused; time limited-service response tailored to a client’s situation. In many cases, this will involve resolving a crisis such as addressing immediate homelessness and improving safety and wellbeing.
Individualised Planned Support is provided through our Case Management services at Sacred Heart Central, the Women’s House, Homefront (Crisis Accommodation Service, the Journey to Social Inclusion Program, and the GreenLight Supportive Housing Program.
GreenLight Supportive Housing Program
The GreenLight Program & Partnership Manager is a key leadership role providing cross agency program support across SHM, VincentCare Victoria (VCV), and The Salvation Army Australia (TSA), service and program oversight support as well as direct service and team management, supervision, and coaching support to GreenLight Supportive Housing Program team members employed by SHM.
GreenLight Supportive Housing Program teams in SHM, VCV, and TSA work with people making the transition from chronic homelessness and rough sleeping to placement in longer term housing to address their unique experience of homelessness and the individual health and wellbeing factors to maintain their housing. The key outcome is stable housing for people and no return to homelessness.
The program provides tailored flexible case management and service coordination services through a multi-disciplinary team, including supportive housing workers, a peer support settlement worker, and mental health workers.
GreenLight has been funded for two years through the Victorian State Government’s Homelessness to Home Program to provide 150 (across SHM and TSA) integrated housing and support packages.

PURPOSE OF THE POSITION
To lead and provide oversight of the multi-agency program ensuring successful stable housing outcomes for the identified client group across the GreenLight consortia and delivering on contracted program aims.
KEY RESPONSIBILITIES
Accountability Major Activities
Management of the GreenLight Program
▪ Manage the GreenLight program in a way that is consistent with SHM’s Vision, Mission and Values and with legislative requirements.
▪ Ensure staff are supervised and supported in a way that ensures development of professional skills and the best possible outcomes for clients. Provide direct supervision to the Team leader and 1-2 other staff members.
▪ Provide program oversight, guidance, and support across the consortia (TSA and VCV) and also for the SHM sub-team- the SHM Continuum of Care Project, ensuring seamless
collaboration and consistent practice.
▪ Ensure compliance with Occupational Health and Safety policy, procedures and legislative requirements.
▪ Conduct regular reviews and monitoring of the service model to ensure the model is as effective as possible in generating positive outcomes for clients.
▪ Regularly monitor and report on client outcomes and ensure outcomes are delivered in line with contractual requirements on behalf of consortia sub teams.
▪ Manage the program within agreed budget parameters. ▪ Perform call responsibilities as part of the larger SHM
on-call services. (Approx. every 10-12 weeks).
▪ Maintain an awareness and understanding of SHM’s risk strategy and its impact on the delivery of the program.

program for relevant stakeholders including the governance groups and DHHS.

Participate in Client Services and SHM management meetings. Monitor and meet required risk, quality, and compliance
requirements within set timeframes
▪ Contribute to a workplace environment which supports staff and volunteers, develops teamwork and ensures the provision of quality services.
Networking and relationship management
▪ Maintain and manage relationships with key GreenLight consortia management and teams
▪ Participate in local, state-wide and national forums as appropriate.
▪ Create and maintain relationships with housing and referral partners
▪ Foster and maintain positive relationships with SHM Managers
Health & Safety Exercise a duty of care to work safely, taking reasonable care to protect your own health and safety and that of your fellow workers, volunteers and clients including following safe working procedures and instructions.
RISK All SHM staff are responsible for considering, identifying and addressing risk (the effect of uncertainty on objectives) whether positive (opportunities) and/or negative (threats).
CQI
(Continuous Quality Improvement)
All SHM staff are responsible for identifying areas of the strategic plan that ‘add value’, and for implementing and monitoring CQI initiatives. Staff are open to new ways of doing things, respond to challenges with innovative ideas and solutions and promote a continuous quality improvement culture.
TIC
(Trauma Informed Care)
All SHM staff are responsible for engaging in learning and development to integrate their understanding of and responsiveness to the impact of trauma within their work including active participation in the supervision and performance development process.
MANDATORY REQUIREMENTS

QUALIFICATIONS
▪ Appropriate tertiary qualifications relevant to the delivery of community services e.g.: Social Sciences, Social Work, Psychology or equivalent.
KEY SELECTION CRITERIA Essential
▪ Demonstrated ability to lead, manage, and supervise the functional activities of a program team in alignment with higher-level program outcomes- and successfully deliver on contracted outcomes/ targets.
▪ Demonstrated ability to work effectively in a fast-changing environment, to work productively with limited supervision and to work to deadlines.
▪ Highly competent in supporting staff to adopt and use a sustaining tenancies practice framework and other supportive housing principles
▪ Demonstrated ability to manage multiple stakeholders including housing and other specialist agencies in order to effectively deliver on partnership aims.
▪ Demonstrated ability to organise and prioritise work both individually and across team and meet strict deadlines without compromising the quality of outputs/ outcomes
▪ Demonstrated ability to provide leadership to a team during periods of organisational and community change.
▪ Demonstrated ability to work within a quality framework and enhance systems and procedures to create organisational and program improvements, with a focus on delivering funded program/ client outcomes.
▪ Demonstrated ability to provide trauma informed staff supervision. ▪ Strong alignment with the values of Sacred Heart Mission.
