Dear Colleagues and Friends,

It is our great pleasure to invite you to attend the 22

nd

Annual International

Congress of the European Society of Gynaecological Endoscopy in Berlin, one

of the world‘s most up-and-coming cities, on 16

th

–19

th

October 2013.

The slogan of our conference is also our common mission:

“together we design the future of women’s health”.

It is a great pleasure and honour to organise this European initiative in collaboration

with the German Academy of Gynaecologists and Obstetricians (DAGG) and the

Forum Operative Gynaecology (FOG), the association of the German working groups

on gynaecological surgery.

This partnership in synergy will allow us to face the universal challenge of providing

global women’s health of the future with a state-of-the-art medicine, including all

innovative and minimal invasive methods of diagnosis and treatment.

The DAGG, joining forces with the ESGE and the FOG, aims to tackle one of the

greatest obstacles for modern women’s health, namely the deficiency of optimal

training and continuous education for the young generation of gynaecologists.

The ESGE and its German peer, the Workgroup Gynaecologic Endoscopy (AGE)

of the DGGG, who found each other in the field of scientific validated training

modalities, converted this part of their common mission into internationally

certified education and training programmes for gynaecological endoscopy.

This brings together a scientific mission and national validated training programmes

in gynaecological surgery. State-of-the-art teaching and learning programmes

in the entire field of women’s health will complete the programme of this first

joint congress. This event, rooted in inter national collegiality, can become an

unforgettable milestone and a beacon for our future-oriented vision.

We greatly look forward to seeing you in Berlin.

Dear Colleagues,

We would like to extend you a warm invitation to Berlin on the occasion of the

6

th

Congress of the FOG.

This time, we are holding the congress in conjunction with the ESGE. The

European Urogynaecology Association will also be in attendance, represented

by its most important members.

This will enable all participants to have the entire spectrum of operative

gynaecology from both the German and the European perspective presented to

them. As a result of these framework conditions, the language of the congress

will primarily be English.

Although breast cancer therapy is not something that is covered by gynaecologists

internationally, we still wish to provide central inclusion of the AWOgyn, in

accordance with the FOG intention, live surgery, lectures and workshops of the

AWOgyn and will be integrated into the overall programme in separate symposia

carried out in German.

This ensures that all FOG themes can also be represented comprehensively at

this international congress.

Please make a note of this date, as this congress will be even more exciting than

any of the previous FOG congresses.

During the course of planning for this congress we have become increasingly

aware that it is going to be an outstanding event. A concentration of operative

manual and theoretical competence of this nature will likely remain a unique

event even in the long term.

Septa, myomas and polyps can represent a hindrance to nidation. As these findings can be well

diagnosed by transvaginal ultrasound nowadays, they are also detected in young women who

may wish not to have children until several years later.

With symptomatic alterations such as a submucosal myoma with hypermenorrhoea or

dysme-norrhoea, hysteroscopic removal is indicated. With asymptomatic findings a diagnostic

hystero-scopy is recommended primarily to enable the finding to be directly assessed visually. Whether

resection is necessary or appropriate and what point in time seems best suited for it, must be

discussed in a detailed conversation between the patient and the surgeon. The decision-making

is based on the current data situation and the mind of the woman concerned.

Histological, immunhistochemical and genetic examinations lead one to assume that a localised

ovarian cancer entity has its origin in the tube.

It therefore seems reasonable to conduct prophylactic salpingectomy on women whose family

planning has been completed, to prevent the risk of ovarian cancer in an individual case.

However, in order to be able to properly assess the value of prophylactic salpingectomy it is

important to know exactly what histological subtype of ovarian cancer it is, how large the

expected benefit is for the patient in the individual case, and whether the salpingectomy can

reduce the incidence of ovarian cancer. The laparoscopic operation technique is also important,

the aim of which must also be to avoid the interruption of blood supply to the ovary in order to

safeguard the patient against premature castration.

The laparoscopic operation technique has gained increasingly in ground in gynaecological

oncology in recent years.

The patient should benefit from the advantages of minimally invasive surgery without the

requisite oncological radicality suffering as a result.

Results of recent studies on the SN procedure with cervical, endometrial, and vulval cancer

even lead one to assume that this can offer the patient the oncologically better treatment.

The laparoscopic surgical approach is also becoming increasingly accepted in the early stages

of ovarian cancer.

However, oncological surgery should only be carried out by sufficiently trained and experienced

surgeons. In no case is increasing the oncological risk acceptable. This must be considered in

each individual case.

The aim of the laparoscopic treatment of endometriosis, particularly in the case of completed

family planning, is the complete removal of all lesions.

However, if the endometriosis is located at prominent places such as the intestine, urinary bladder

or ureter, complete removal may result in considerable extension of the surgery, possibly

including the involvement of other specialist disciplines. That is why the surgeon responsible

has to weigh up to what extent increased radicality is really helpful to the patient in the long

term, or whether the same effect can be achieved by deliberately leaving the endometriotic foci.

The laparoscopic approach to deep infiltrating endometriosis is one of the most difficult

procedures. Due to the pathophysiology of the disease all pelvic organs may be involved.

Inflammatorial reaction, fibrosis, infiltration into the ligaments and pelvic sidewall structures

make surgery very difficult. Complete pre-operative assessment of the disease and planning

of the surgical approach is mandatory. Due to adhesion formation and alteration of anatomic

structures the approach needs a sophisticated strategy.

Course participants will increase their knowledge of the minimal invasive approach to deep

infiltrating endometriosis including the cul-de sac obliteration or frozen pelvis. Outstanding

specialists will demonstrate the surgical skills and techniques including the pre- and

post-operative management.

On completion of this course the clinician will

– be familiar with the pathophysiology of deep infiltrating endometriosis

– able to make the right pre-operative diagnosis and plan the surgical procedure

– able to evaluate potential surgical approaches to women with extragenital lesions

– know how to identify the deep lesions which are responsible for the clinical signs and symptoms

– know the various techniques of dissection, excision, coagulation and vaporization

– be familiar with surgical strategies in DIE (deep infiltrating Endometriosis) including

extragenital lesions

– know the risk of the surgical procedure and how to avoid them

An advanced course targeted at gynaecologists involved in reproductive surgery

There are still a lot of controversies concerning endometriosis when it is associated with

infertility. The aim of this advanced course is to provide participants « state of the art » training

on this hot topic with the help of well recognised experts.

Endometriosis diagnostic, surgical treatment, results of IVF in endometriotic patients will

be discussed, as well as the guidelines to preserve fertility during endoscopic surgery for

endometriosis.

At the end of the course the participants will have a clear opinion on what to do or not to do

in the case of infertile endometriotic patients.
