Associate Degree Nursing Program Application Requirements & Instructions

This letter contains vital information and instructions that the applicant must implement completely in order to be eligible for consideration for admission to a Nursing Program.

Pre-Application Requirements:

 Complete a Florence-Darlington College Admission Application

 Attend a Nursing Career Talk

 Take the TEAS Test and make a score of Proficient or above.

Application for the Nursing Program (may only be submitted on “Business Days” in September 2015):Limit the Admission application to only one nursing program during a given application period.  First, please print the Application for Admission to the Nursing Program on standard white printing paper.

 Completely fill out the Application once all of the instructions have been read and all of the information requested has been provided.

 Be sure to keep a copy for your records.

 The Application then must be returned in person by the applicant to the HSC Administrative Assistant (Ms. Evans), Room 362 in the Health Science Campus (HSC) at 320 West Cheves Street. Applications are accepted

beginning on the first business day in September, between the hours of 8:00 a.m.-12:45 p.m and 2:15 p.m-4:00 p.m (M-Th) and 8:00 a.m.-11:00 a.m. (F). The Application deadline is the last business day in March, no later than 4:00 p.m.

High School Diploma/GED/Transcripts:

An official copy of the applicant’s high school transcript/GED and official transcripts from all colleges or universities previously attended must be received in the Registrar’s Office by the last business day of the application period deadline.

High school credentials are required unless the applicant has earned 12 college credits or received an Associate in Arts/Science or Bachelor’s degree or higher from a regionally accredited institution. DO NOT BRING OFFICIAL

TRANSCRIPT(S) TO THE NURSING DEPARTMENT.

If you have been enrolled previously at any nursing school taking NUR/PHM courses, you must disclose this during the application process AND submit official transcripts to FDTC from the college or university. If you were

unsuccessful, there is a 2 year waiting period beginning with the last NUR/PHM course at the school taken.

*Proof of residence: Electric, water, gas, or telephone bill, bank statement, car registration, real estate tax property receipt, and rental agreements on business letterhead may be used as proof of residence. Bills must include the home address of the applicant, parent, or legal guardian. If the bill is in the same name of the legal guardian, it must be accompanied by guardianship papers signed by a judge. A driver’s license or check will not be accepted as proof of residence.

The following criteria must be met prior to submitting weighted admission form. Please check all that apply.

Must complete the application process to FDTC and have received an acceptance letter from admission. Must have official transcripts from other colleges evaluated by FDTC Registrar office.

Do not attach official transcripts to this form.

Current major coded by college as AS.AS.NUR and verify that course/grades can be viewed in Web Advisor. Must have completed TEAS exam with a score of “Proficient” or higher.

Applicants are to be advised that due to specific contract requirements by the clinical agencies used for clinical by the Nursing Program, background checks and drug screenings are part of the admission and retention process. The background check/drug screening will be completed once the applicant has been accepted to the nursing program. Applicants with specific conviction histories or positive drug screenings may ultimately not be accepted into the Associated Degree Nursing Program. Positive background checks will be sent to the clinical agencies, without identifying information, for their decision whether or not to allow the applicant to perform patient care in their institution. One agency denial is considered a denial from all agencies; therefore, the applicant cannot be admitted to the nursing program.

A student with a positive drug screen is not eligible for admission to the nursing program. Specific convictions or positive drug screens occurring after full admission and matriculation in the program will be addressed per Nursing Department policy, including dismissal.
