CANCELLATION POLICY

No refunds will be made for cancellations after July 1, 2016. Refund of CVS registration fee(s) less $60 per person for administrative costs will be granted by request only if postmarked by July 1, 2016. Registrations are not transferable.

EXHIBIT HALL ENTRY

All CVS registrants and attendees (e.g., family members) may enter the exhibit hall at no charge. An adult must accompany children under the age of 12.

BADGE ADMISSION

An official CVS name badge is required for entrance to all events. The loan or transfer of this badge is strictly prohibited. Violation of the policy subjects the registrant to forfeiture of the right to attend and the loss of all fees paid.

LECTURE ROOM SEATING

Most rooms will be set with chairs only.

Feel free to bring a lapboard for use in taking notes.

LITERATURE DISTRIBUTION

Registrants are not permitted to distribute literature, samples or souvenirs.

MOBILE DEVICES

All cell phones and pagers must be silenced during the lectures. If you must talk on your cell phone, please leave the lecture immediately.

CE Information

COURSE OUTLINES FOR OPTOMETRISTS AND PARAOPTOMETRICS

Our Course Outlines can be found on our website prior to the conference and can be downloaded onto your smartphone, tablet or computer. There will be free WiFi available in all of the conference rooms. A Printing Station will not be provided this year. Please print all course outlines ahead of time if you would like copies for each lecture.

CONTINUING EDUCATION CREDIT HOURS

The OD program offers 56 hours of education–20 hours obtainable. The Paraoptometric program offers 31 hours of education–10-11 hours obtainable. CVS does not guarantee that any courses are accepted for credit by a licensing board. Attendees are responsible for being thoroughly familiar with the licensing requirements of their individual states.

CE credit is applied in the following manner:

One 50-minute lecture equals one (1) hour of CE credit (i.e., 8:00 to 8:50 = 1 credit hr).

OD VERIFICATION OF ATTENDANCE

A course verification form is given to all optometrists who enter the course within the first ten (10) minutes of the lecture. At the completion of the lecture, monitors are available to stamp the forms verifying attendance. QR codes will be available for scanning for automatic upload to OE Tracker after each course finishes. Note: A $75 service fee is charged per request for copies of CE credit slips.

CONTINUING EDUCATION RECORD KEEPING

CVS does not maintain individual attendance records.

It is the responsibility of the doctor to maintain all attendance records. Colorado doctors do not need to submit records to the Colorado Board of Optometry unless requested to do so.

This event is included for all COA members, guests must RSVP for this event when they register for CVS or by emailing [email protected] no later than July 1, 2016

Tickets may be purchased for guests at $25/person for adults or $15/person for children One drink ticket will be given to all guests, included in your registration. A cash bar will be available.

Short presentations will include COA’s annual awards and recognition of COA’s President. Dress for this event is casual.

“ENCHANTMENT UNDER THE SEA” BANQUET FRIDAY, JULY 15, 2016
