You receive a complaint from Dow Corning via e-mail.

This e-mail provides you the following information: 1) References of the complaint 2) Link to the VQN Web Page

where you’ll be able to visualize the description of the complaint and take further action

3) Response deadline 4)(Optional) Purchase order

number and batch number. 5)Link to view attachments.

Click now on the link to access the VQN content. (Slide 3)

NOTE: Informational Notifications do not require any action from you. (Slide 12)

VENDOR NOTIFICATION –

DC PREMIER FIRST TIME USERS

WITH A DC PREMIER ACCOUNT

For first time users:

1. Enter Email address

2. The first time you do this, you need to click on the “Forgot Password” link to reset your password.

You will be prompted to re-enter your email address.

If you have an account you will

receive an email from Dow Corning with your temporary password. At this time a new password will need to be assigned by the user via the email link to DC Premier.

• Log off DC Premier after resetting your password.

Then open VQN email to access the VQN link application.

You will now be prompted to enter your email address and your newly assigned password.

WITHOUT A DC PREMIER ACCOUNT /

REGISTRATION

For first time users w/o a DC Premier Acct:

•Enter Email address •Click Register Now

•If you do not have an account you will be asked to register and complete the necessary form.

Register:

•Step #1 – Enter email address •Step #2 - Must select YES (to being a customer)

•Step #3 - Select Apply Now •Step #4 - Complete all required (*) fields

• Step #5 – Check both check boxes • Step #6 - Click Finished

Once form is received by Dow Corning it will be reviewed and access will be granted, if approved. Vendor will receive letter, along with training material.

Vendors will only be able to view information related to their account profile.

Registered Users:

Log into VQN Application (DC Premier) with your e-mail address and password.

PLEASE NOTE:

If you forgot your password, click on “Forgot Your Password” and you will receive an email from Dow Corning with a temporary password.

VENDOR NOTIFICATION – Complaint Content -

Provide Investigation / Corrective Action Plan

Read the Description of Problem.

Make note of the priority and response time:

• Standard processing • Immediate response • Health & Safety

Please provide the following:

1) Investigation (1500 Character Limit) 2) Corrective action plan (1500

Character Limit)

3) Cause Code (Use the dropdown button) – not mandatory

4) Send these elements by clicking on the ‘Submit to Dow Corning’ button. 5) If you want to send any document

supporting your investigation, please e-mail the Dow Corning coordinator shown below the screen.

6) If there are attachments, you will see a link.

VENDOR NOTIFICATION – Investigation & Corrective action plan Accepted by Dow Corning

Receiving this notification means Dow Corning is reviewing your Investigation & proposed Corrective Action.

Effectiveness of your Corrective action plan will be validated by Dow Corning.

If your action plan is effective, the complaint will be closed and you’ll be assessed on the quality of your response.

If your action plan is not effective, a new complaint will be created to further investigate the issue.

VENDOR NOTIFICATION – Investigation or Corrective action plan rejected by Dow Corning

Receiving this notification means Dow Corning might consider either your investigation or Corrective Action Plan (or both) is not relevant/effective and will ask you to propose and resubmit.

A new e-mail will be sent to your attention, advising that your response has been Rejected (and the reason why).

Click on the link to access VQN content, log in and submit a new version of your Investigation AND Corrective action plan taking into account our comments.

VENDOR NOTIFICATION – Vendor Quality Notification (VQN) Completed

Once all tasks related to a complaint are closed (Validation of

effectiveness of Corrective Action Plan incl.), the entire complaint will be completed.

The quality of your response and the response time to the complaint will be evaluated by Dow Corning and your score will be

communicated by e-mail. Here’s an example of that

VENDOR NOTIFICATION – Deadline not respected.

If you don’t provide us Investigation & Corrective Action Plan within requested deadlines, reminders will be sent to you on a regular basis.

VENDOR NOTIFICATION – Compensation not received

If the issue requires financial compensation and if this compensation has not been received, reminders will be sent to you.

INFORMATIONAL

If you receive a complaint from Dow

Corning via e-mail and the Priority for this complaint is

“INFORMATIONAL”

1) Email specifies that VQN is for informational purposes only & no response is needed 2) You may link on web to view

VQN description

3) ‘Cause Code’, ‘Investigation’, ‘Corrective Action Plan’ fields are deactivated, as you don’t need to reply to this VQN

If the VQN response has already been submitted then, all the fields would be in read-only mode. Once a VQN has been submitted, it cannot be resubmitted.

If the VQN response has not been submitted yet, then refer to Slide 6.
