provide family FREE support services and health education to pregnant women,

fathers, and families with children ages zero to five. B&B uses the evidenced-based Nurturing Parenting Program (NPP) for Infant, Toddlers, and Preschoolers (ITP), and adaptations including Prenatal NPP, and Father’s NPP; other components include: Home Visitation: provides in-home parenting education, to improve bonding, parent-child relationships, and child behavior. Also connects families with community resources.

Parenting Workshops: workshops in multiple languages designed to improve parenting skills held at Family Resource Centers and other community locations. School Readiness: offers age appropriate learning and development skills and reading resources. Provide enrollment requirements and links families to their local school district.

Crisis Intervention: assist families with concrete support in times of need. Families receive advocacy and resources to address

Early Steps To School Success will provide early childhood education services to children birth to five years of age, education services to their parents, and ongoing training to the community early childhood educators involved with the program. The program is designed to assist children with early language development, social and emotional development, and equip parents with the skills and knowledge to successfully support their child’s growth.

DHHS WIC- Community Lactation Assistance (CLA) Program: provides home visits to lactating mothers high in need due to physical challenges and/or limited resources. Home visits usually include one-on-one lactation consults with a Lactation Consultant providing a feeding assessment and education. The WIC Program does not routinely offer home visits. Home visits are very limited and usually include delivery of a breast pump and/or basic “hands-off” breastfeeding triage and information.

Sacramento County Office of Education (SCOE) is implementing Project Screening, Outreach, and Referral Services (SOARS) to provide FREE screening and referral services for children from infancy through age five who are at risk for developmental delays and/or disabilities. Screening and referral services will be prioritized for children and families served by transitional housing programs, WEAVE and the Sacramento Food Bank. Family Advocates will conduct voluntary developmental screenings for children using the Ages and Stage Questionnaires (ASQ) and ASQ-Social Emotional (SE) through home visits to maximize access to these services. Based on screening results, families will be referred for further

assessments. Family Advocates will conduct home visits to develop a Service Plan for families to establish agreed upon goals for the families to access services for children with developmental delays and/or

disabilities and other appropriate resources. Project SOARS will also develop a

comprehensive approach to offer education, outreach, and training to parents/caregivers and providers that addresses the importance of screening services, early intervention, and

Sunburst Projects provides FREE support services for youth and families living with HIV/AIDS in Sacramento, Placer, and El Dorado counties. Our medical case management and mental health services, home visitation, school advocacy, and legal support are some of the services provided. We also offer groups that provide

opportunities to give and receive support from peers and stay current about

HIV/AIDS.

The maternal and childhood health program provides mothers living with HIV with valuable knowledge and support throughout pregnancy, labor and delivery. Follows mothers and children closely to foster healthy child development and post-partum

Sunburst Projects provides residential

camps for infected and affected youth ages 6 to 12 and youth leadership camps for ages 13 to 18. In addition a ‘One Love

Conference’ is offered yearly to young adults living with HIV/AIDS ages 17 to 25.
