Florida Emergency Nurses Association

2015 Annual Awards

Welcome to the Florida Emergency Nurses Association State Council Awards and Scholarship program. FENA annual awards are recognition awards given to individuals that exemplify

exceptional professional practice, innovation, leadership and advocacy in the specialty of emergency nursing. Specific awards may differ in eligibility requirements and evidence of achievement. You can locate a copy of the applications at www.floridaena.org click on the Scholarships/Awards tab and search under scholarships.

 Receipt of a formal letter from the FENA President  Commemorative Plaque

Deadline

The final deadline to submit nominations and required documentation for the following annual award categories is September 30 of current year

Submission Guidelines

 Complete the award nomination form. This form will include:

1. The name of the award for which you are nominating someone 2. Contact information for both you and the nominee

 Nomination form must be e-mailed. Faxed application is not acceptable.

 Nominee must be an ENA member for a minimum of twelve (12) months at time of application.

 Nomination form submitted electronically must include scanned copies of the required

documents: signed application, narrative statement (s), letters of recommendation, etc. Letters do not need to be submitted in a sealed envelope.

 All blanks must be filled in. Attaching a document in place of filling in blanks is not acceptable.

 All paperwork must be turned in together.

 Completed nomination forms must be received by the deadline: September 30 of current year  Applicants will receive an acknowledgement of receipt of application and documents within

one (1) week.

 Applicants that do not receive an e-mail of confirmation within one week (not including weekends and/ or holidays) must contact the chairperson to assure application has been received.

2015 Award Categories

Nursing Education Award - Honors an emergency nurse who has made significant contributions to the education of colleagues, nursing students, personnel, patients, and families through the education and/or publication of articles, formal or informal courses in institutions (schools, hospitals, etc.), nursing books or the development of a specific emergency nursing program or curriculum.

Requirements

 Emergency Department registered nurse or advanced practice nurse licensed to practice in the state of Florida.

 Educational expertise in emergency nursing or EMS in roles such as mentor and/or preceptor.  Performance beyond job expectations in developing educational programs/instructions for

nursing students, patients, families, and/or EMS personnel.

 Emergency nursing expertise through publications in nursing journals or books, educational meetings with excellent evaluations, or the development of a specific nursing curriculum.  Two (2) letters of recommendation.

 Nominee must be an ENA member and have been a member for at least twelve (12) months or longer.

Leadership Award – Recognizes an emergency nurse who consistently demonstrates leadership within the association, demonstrates high professional behavior, and has contributed to the professional status of emergency nursing.

Requirements

 Emergency Department registered nurse or advanced practice nurse licensed to practice in the state of Florida.

 Demonstrates leadership within the association that has contributed to the professional status of emergency nursing.

 Encourages professional development and provides motivation through role modeling.  Recognized by peers as a leader.

 Narrative statement (s)

 Two (2) letters of recommendation.

 Nominee must be an ENA member and have been a member for at least of twelve (12) months or longer.

Nursing Practice Award – Honors an emergency nurse who exemplifies outstanding nursing practice as demonstrated through clinical skills and compassion.

Requirements

 Emergency Department registered nurse or advanced practice nurse licensed to practice in the state of Florida.

 Demonstrates emergency nursing knowledge through performance of consistent high-level care and compassion to patients and families.

 Evidence of quality of care improvement through implementing the ENA standards of Emergency Nursing Practice.

 Performance beyond job requirements in rendering patient care.

 Participation in nursing programs, committees, or projects contributing to emergency nursing.  Narrative statement (s) as outlined above.

 Nominee must be an ENA member and have been a member for at least twelve (12) months or longer.

Distinguished Service Award – This award is presented to an emergency nurse who reflects a career of dedicated service, accomplishments, and contributions to emergency nursing.

Requirements

 Emergency Department registered nurse or advanced practice nurse licensed to practice in the state of Florida.

 Sustained accomplishments affecting emergency nursing.

 Demonstration of service or commitments beyond job expectations.

 Progressive leadership and personal ideals which have advanced the field of emergency nursing.

 Inspiration and vision to promote the evolution and ongoing dynamic of emergency nursing.  Contributions that have affected the profession within the community, region, or county.  Narrative statement (s) as outlined above.

 Two (2) letters of recommendation.

 Nominee must be an ENA member and have been a member for at least twelve (12) months or longer.

Chapter of the Year Award – Recognizes a chapter for significant contributions toward Florida ENA through a project or improvement of the chapter.

Requirements

 Development of a project or program that has a positive impact on the chapter or FENA.  Significant increase in FENA membership ( attach supportive documentation)

 Significant increase number of members with BCEN Certification (CEN, CPEN, CFRN, CTRN).

 Significant increase number of members with TNCC and/or ENPC Verification.  Contributions that have affected the profession within the community, region or state.  Narrative statement (s) as outlined above.

 Two (2) letters of recommendation.
