HCA’s roots date back to 1968 when Dr. Thomas Frist, Sr. led a group of physicians to build a hospital in Nashville, Tennessee. Over four decades of hard work and dedication have made HCA – Hospital Corporation of America – the nation’s leading provider of quality healthcare services, with locally managed facilities that include approximately 163 hospitals and 109 outpatient surgery centers in 20 states and throughout Great Britain. In Florida, there are 41 hospitals and 31 surgery centers, three division offices and several other service facilities affiliated with HCA.

HCA East Florida Division

Located in Ft. Lauderdale, the HCA East Florida Division encompasses 14 hospitals in South Florida and the Treasure Coast, in addition to multiple ambulatory surgery centers and imaging centers, a supply chain center and an integrated regional laboratory. Together, the network employs more than 13,900 individuals and has nearly 5,000 physicians on staff.

The Palm Beach region of HCA East Florida consists of Columbia Hospital, JFK Medical Center and Palms West Hospital. Quality and Patient Safety

hCare Electronic Health Record (EHR)

EHR simplifies access to patient health information by immediately providing clinicians with accurate information for optimum patient care. We believe that EHR will be a fundamental support system for improving the quality of healthcare in our facilities.

hCare Hub/Mobility Physician Portal

This portal integrates the electronic medical

records of our physician practices with our hospitals to provide physicians with access to their patients’ clinical records. It puts critical information at their fingertips, both on their office computers and via smartphone technology.

hCare Access

This service provides physicians and their staff with a faster and more convenient remote login solution. Whether using PC, Mac or iPad, physicians have access to the same clinical applications they use at the hospital from virtually anywhere. Connection time is 75% faster than prior remote access solutions and single sign-on password management is available.

Computerized Provider Order Entry (CPOE)

CPOE is patient safety technology that allows providers to place orders directly into the clinical information system and check each order against the patient’s medical information. Providers are alerted to any potential contraindications or interactions at the time of order. This streamlines the process and eliminates opportunities for medical error. In addition to the safety checks, orders go immediately to the department(s) that will implement/initiate the order, leading to more efficient patient care. This system is used in all of our emergency departments and hospital-wide.

Picture Archiving and Communications

Systems (PACS) & MUSE

PACS stores digital versions of patients’ clinical images, such as CT and PET scans, MRIs and X-rays to make them readily accessible at the hospital or remote locations. The MUSE system digitizes EKG tracings, allowing for easy access to this information.

Electronic Bar-Coded Medication

Administration

All HCA East Florida hospitals have implemented eMAR software to prevent medication errors and improve patient safety. Each patient receives an armband with a unique bar code. Before a medication is administered, the bar codes on both the armband and medication are scanned to verify that the patient is receiving the right drug and dosage at the right time. The software checks each medication against the patient’s drug history and lab results. If conflicts or potential interactions are identified, warnings alert the nurse to verify or call the doctor before administering the medication. HCA’s results with eMAR have been instrumental in encouraging the FDA to develop a standardized bar code system for the pharmaceutical industry and at all hospitals to improve patient safety.

Bar code-enabled transfusion administration allows clinicians to scan the patient ID band and the bar code on the blood product to ensure that the correct product is being issued and administered to the patient. This capability is similar to the functionality introduced by the eMAR & Bar Coding project, now called Bar Code-Enabled Medication Administration.

Voice Recognition System

This technology allows physicians to report their clinical findings directly on a system that recognizes their voice. It creates efficiency in our radiology departments by eliminating the delays often caused by transcriptions. The report then gets reviewed on screen by the radiologist where it can be corrected, signed and sent for utilization by other clinicians within a 30-minute time frame.

Emergency Department Management

Module (EDM)

EDM is used to manage the flow of patients through the emergency department from arrival through departure. Both CPOE and eMAR fully integrate into EDM, allowing for streamlined communication of orders and medication administration between physicians and emergency department staff. EDM is also used to measure and track emergency room wait times quickly and accurately.

Telehealth

At JFK Medical Center, HCA East Florida has the ability to monitor patients from a distance using electronic devices to record and send measurements of patient vital signs. Telehealth technologies are evolving to provide real-time interactive health systems that can engage patients and their care teams more fully in their treatment. Upon discharge, patients are monitored and managed from home.

Emergency Department

For the past few years, HCA East Florida has been working diligently to improve the effectiveness and efficiency of its emergency rooms. Our focus on enhancing care and improving emergency processes has dramatically reduced wait times to below national averages. We have been so successful that average emergency room wait times are now available to the public. This vital information provides patients with the resources to make informed decisions about their urgent medical needs. Wait times for all of our hospitals are updated every 30 minutes and are available via text of ER to 23000, iPhone app on our website, iPhone iTriage app from the App Store and roadside digital billboards throughout the region. We also offer emergency room wait times for our hospitals with pediatrics. Patients can text ERKids to 23000 for access to immediate average wait times for children’s emergency care.

Technology

Our East Florida Division hospitals are upgrading technology at an unprecedented rate to ensure that we provide clinical excellence and optimal patient outcomes. In addition to using the most advanced medical systems, our facilities have integrated wireless and digital technology through all modalities. We also offer nine robotic programs that enable surgeons to perform precision work with very small incisions and faster recovery times.

Remote Patient Monitoring

Airstrip Cardiology and Airstrip OB are two applications that are improving the speed and quality of communication within our hospitals. AirStrip Cardiology is the revolutionary new technology that delivers EKGs from Emergency Medical Services directly to doctors’ and nurses’ smartphones or tablets. It allows patients to be assessed while they’re being transported to the hospital. The crucial time saved can help save lives. Airstrip OB delivers vital patient waveform data, including fetal heartbeat and maternal contraction patterns, in real time directly from the hospital labor and delivery unit to a doctor’s smartphone. This application gives physicians the ability to closely monitor patients 24/7 when the demands of their day necessitate their periodic absence from the labor and delivery unit.

iTriage

iTriage provides access to medical information on thousands of symptoms, diseases and medical procedures. It also allows patients to find emergency room wait times, get healthcare cost information and view quality reports. iTriage was developed to put better medical information into patients’ hands. These medical pioneers have now empowered healthcare consumers with this revolutionary acute care decision tool. iTriage is available as a free app for iPhone,®

Android,TM Blackberry® & Palm® smartphones or

Social Media

Social media allows the community to communicate with us directly. All of our hospitals can be accessed through Facebook and Twitter, and there is a video library on YouTube. Our social media channels make it simple to stay up to date on our latest technologies, services and community involvement. Volunteering

In 2011, HCA East Florida hospital employees and volunteers dedicated 377,444 hours of patient and family support. Events included building homes with Habitat for Humanity, Making Strides for the American Cancer Society, and promoting wellness and education through complimentary health screenings and lectures. We sponsor the Team Storm Riders cycling team, which ranked 49th in the country for raising funds for the National Multiple Sclerosis Society, with riding members from our staff.

Step Up for Students

Since 2005, HCA has redirected corporate income tax to the Step Up for Students program. Through this gift, low-income students receive a K-12 scholarship enabling them to receive a quality education. Qualifying parents can use these grants to send their child to the public or private school that best meets their child’s needs.

Consult-A-Nurse® is a free service that provides

physician referrals and health information by phone, 24 hours a day. The service supplies information about physicians affiliated with HCA hospitals, including their medical specialty, skills, experience, and office information. Registered nurses are available to provide consultations and answer health questions.

The Center for Pain Care

Our Center for Pain Care provides caring treatment for patients with acute and chronic pain problems. Our physicians are dedicated to helping patients find relief through medication management, procedural intervention, biofeedback, psychiatric services and physical therapy. Specialists include neurologists, anesthesiologists, orthopedic surgeons,

physiatrists and nurses. They treat numerous chronic disorders, such as cancer pain, lower

back pain, sciatica, cervical (neck) strain, reflex sympathetic dystrophy, myofascial syndrome pain caused by peripheral nerve damage, pancreatic pain and neuropathic pain.

Cardiac Catheterization

We are proud to announce the grand opening of Palm Beach County’s newest Cardiac

Catheterization Lab. Our state-of-the-art cardiac catheterization (cath) lab will play a key role in the diagnosis and treatment of cardiovascular disease as well as the prevention of heart attack and stroke. Our lab is equipped with the most advanced technologies, including new digital imaging systems with flat screen technology that enable the medical team to see the workings of the heart and blood vessels with unprecedented clarity.

If your physician suspects that you have coronary artery disease, or if you have symptoms of the disease, several tests are used to make a diagnosis. If these tests indicate that your heart is not

getting enough of the oxygen that it needs, cardiac catheterization, also called angiography, may be suggested.

Behavioral Health Services

The Pavilion, an 88-bed inpatient unit adjacent to the hospital, offers comprehensive psychiatric and behavioral health services. Psychiatric needs of children, adolescents, adults and seniors are assessed and treated through a full range of programs, including individual therapy, group therapy, family therapy, medication management and aftercare. An intensive outpatient program and

partial hospitalization program are also offered for adults. The Pavilion has 27 licensed beds focusing on children and adolescents from the ages of 5 to 17. It is the largest licensed Baker Act receiving facility accepting children in Palm Beach County. Columbia Hospital recently opened a new after-school adolescent evening intensive outpatient program for teens ages 13 to 17 years old. Through individual, group and family sessions, we will strive to meet the academic, social and emotional needs of adolescents with behavioral health issues. Our team of caring professionals will be there to assist every step of the way.

General and Vascular Surgery

Columbia Hospital performs highly advanced inpatient and outpatient general and vascular surgical procedures. Our surgical team utilizes minimally invasive surgical methods, including endoscopic and laparoscopic procedures, to minimize recovery time and allow a quick return to a normal routine.

The Orthopedic & Spine Institute

of the Palm Beaches

The Orthopedic & Spine Institute of the Palm Beaches offers comprehensive, innovative technological solutions for disorders of the joints, neck pain, fractures, arthritis, joint discomfort and back pain. Using the latest advances, our dedicated professionals can attend to all of your surgical needs. Our team of experts includes board-certified orthopedic surgeons and neurosurgeons with extensive training, orthopedic-certified/

trained professional nurses, physical therapists and technicians. The Institute offers support from pre-admission to post-discharge and the resources needed to ensure a seamless transition from hospital to home.

Inpatient Hospice Unit

Columbia Hospital’s inpatient hospice unit, located on the second floor of the main hospital, offers a homelike setting with an interdisciplinary team of nurses, physicians, social workers, a chaplain and volunteers. The unit can accommodate 11 patients, each with a private room, and visiting hours are always available for any age or number of guests.

The Breast Care Center at

Columbia Hospital

The Breast Care Center is designed to provide a comfortable, compassionate environment for our patients, staff and physicians. The Center offers a full range of services including digital mammography, computer-aided detection (CAD) systems, ultrasound, ultrasound-guided biopsy/ cyst aspiration, stereotactic biopsy, weekly breast-focused multidisciplinary conferences, second- opinion and surgical consultations, and bone density tests. We are one of the first hospitals in West Palm Beach to utilize the HydroMARK, a device used to mark areas where breast tissue has been removed during a non-invasive breast biopsy. The HydroMARK is made of a bioabsorbable material that can be seen under ultrasound for an extended period of time. We are proud to offer this innovative device for the benefit of our patients.

The hospitals earning this designation were recognized for evidence-based clinical processes that are shown to improve care for various conditions, including heart attack, heart failure, pneumonia and surgical care. Columbia was also recognized for its success in performance achievement using evidence-based guidelines to provide the best possible care to patients through the American Heart Association/American Stroke Association’s Get With The Guidelines® Program. Get With

The Guidelines is a hospital-based, quality-improvement program designed to ensure that hospitals consistently care for heart and stroke patients utilizing the most up-to-date guidelines and recommendations. The Silver Performance Achievement Award is given to those hospitals that achieve 85% compliance with Get With The Guidelines measures for 12 consecutive months. More than 1,400 hospitals participate in this program.

Additionally, Columbia has been awarded a three-year term of accreditation in mammography as the result of a recent review by the American College of Radiology (ACR). The ACR gold seal of accreditation represents the highest level of image quality and patient safety.

Radiology and Diagnostic Imaging Services

The radiology department provides full-service diagnostic imaging specializing in interventional and women’s imaging. The department is staffed by registered licensed technologists of the American Registry of Radiologic Technologists and technologists with advanced certification in CT scan imaging. Columbia’s MRI with EXCITE technology delivers breakthrough results from an image-quality, application and enhancement perspective. The combination of faster and better scanning can significantly decrease patient time in the magnet. The VIBRANT MRI component enables simultaneous, contrast-enhanced acquisition with high spatial resolution of both breasts, which eliminates the need to return to obtain the highest-quality images. In addition, our PACS technology allows physicians to view and manipulate images simultaneously, even from separate locations.

Wound Management

Columbia Hospital specializes in the treatment of non-healing wounds. Our specially trained physicians and nurses develop effective, up-to-date treatment plans to assist in the healing process of specific types of wounds. Following a medical review, our team of caring professionals helps with decisions regarding wound care, including dressing instructions, home care, and wound prevention education. Weekly monitoring of progress enables the team to make adjustments to care to provide the fastest results.

Outpatient Rehabilitation Services

Outpatient rehabilitation services at Columbia Hospital offer individualized and comprehensive physical, occupational and speech therapy. Our team is committed to providing professional and compassionate care in a one-on-one setting. Therapeutic plans are developed to help patients achieve the highest level of functional independence. Physical therapy services are available for

orthopedic, neurologic and neuromuscular conditions including neck/back pain, arthritis, preoperative care, postoperative care, traumatic injuries, sports injuries, balance deficiencies, stroke rehabilitation and osteoporosis. Occupational therapy is available for those with a decline in the ability to perform activities of daily living, such as self-feeding, bathing, grooming and dressing. Services are also available for the treatment of acute or chronic injuries of shoulders or hands. Speech therapy is provided by a certified speech language pathologist to meet the individual needs of patients with deficits in speech, language, cognition and/ or swallowing. Video fluoroscopic swallow studies are available in conjunction with the radiology department. Individuals requiring any rehabilitation treatment must be referred by a physician and have a signed prescription from that professional.

Volunteer Program

Our volunteers are the lifeblood of Columbia Hospital. They provide support services that are vital to providing daily care and comfort to our patients. We are always seeking volunteers to give their time and have many ongoing and short-term options available.

JFK Medical Center is proud to provide the highest-quality healthcare to South Florida. In 2011, we made many major accomplishments. We opened our 31-bed behavioral health services unit to provide adult acute psychiatric inpatient care and become a designated Baker Act receiving facility. Together with Palms West, we were the first to offer single-site non-invasive robotic surgery with the da Vinci®

Surgical System to the residents of Palm Beach County. We also began a complete renovation of the Comprehensive Cancer Center and held the groundbreaking for our new 50,000 square-foot medical office building.

JFK Medical Center has been designated as one of the few sites in Florida to offer transcatheter aortic valve replacement (TAVR) therapy with the Edwards SAPIEN Valve. JFK is part of a select group of hospitals in the U.S. approved to offer this promising new procedure for those considered too high risk for traditional heart valve replacement, or those who were previously considered inoperable. Looking forward, we will continue to bring break-throughs in cardiac and cancer care, as well as the latest diagnostic tools and procedures to care for the community.

Conveniently located in the heart of Palm Beach County, JFK Medical Center is a 460-bed facility known for its comprehensive services, including general and orthopedic surgery, neurosurgery, stroke care, minimally invasive surgery, robotics, cancer care, sleep disorders, emergency

services and cardiovascular care. Our medical staff is comprised of more than 500 physicians representing virtually every medical specialty. Our staff also includes more than 2,000 allied healthcare professionals, complemented by over 300 adult and teen volunteers.

Bariatrics

The Bariatric Wellness and Surgical Institute at JFK Medical Center is dedicated to helping patients achieve their weight loss and health goals through a multidisciplinary approach to morbid obesity. With comprehensive programs ranging from wellness services for medical weight loss to surgical procedures, we provide thoughtful, patient-focused care. Accredited as a Center of Excellence by the American Society for Metabolic and Bariatric Surgery (ASMBS), our bariatric surgical program is staffed by top-notch certified medical professionals and offers surgical weight loss procedures

including Laparoscopic Roux-en-Y Gastric Bypass, Laparoscopic Adjustable Gastric Band, Laparoscopic Sleeve Gastrectomy and revisions of prior surgeries. Our team also includes nurses, nutritional and

psychological support, physical therapy, pre-operative education, insurance approvals and appeals coordination.

Behavioral Health Unit

JFK Medical Center’s new behavioral health services unit is helping to ease the critical shortage of behavioral health beds in our area. Our 31-bed unit offers a safe, soothing and comfortable environment. We will provide adult acute psychiatric inpatient care and become a designated Baker Act receiving facility that will treat a wide variety of behavioral health diagnoses. Our high-quality care is evident in all of our treatments and with our experienced team of mental health professionals, including psychiatrists, nurses, therapists, social workers, mental health technicians and occupational therapists. Our program includes individual therapy, group psychotherapy, occupational therapy, family therapy and medication management. Patients and families are encouraged to actively participate in both the planning and implementation of the treatment.

The Heart & Vascular Institute

Founded more than 20 years ago, the Heart & Vascular Institute at JFK Medical Center boasts high volumes and superb outcomes. The cardiac surgery team has performed more than 12,000 open heart surgeries and the electrophysiology lab (EP) is one of the busiest facilities of its kind in the southeastern United States. The JFK Cardiac Catheterization Laboratory interventional cardiologists spend all of their time performing cardiac catheterization procedures. JFK Medical

Center has the highest volumes in Palm Beach County and the Treasure Coast in coronary bypass surgery, cardiac catheterization, defibrillator implants and pacemaker insertion. The JFK Medical Center Vascular Institute features GE Innova Bi-PlaneTM technology, which enables physicians to

view detailed 3-D images on two different planes at once and is imperative in delivering state-of-the-art stroke care and vascular treatment. JFK Medical Center is the first heart program in Palm Beach County and the Treasure Coast to offer Transcatheter Aortic Valve Replacement (TAVR) therapy with the Edwards SAPIEN valve. Due to its curative nature, the TAVR procedure represents a major advance for patients who are not surgical candidates. The TAVR procedure is performed by cardiologists and cardiac surgeons in a hybrid operating suite that combines cardiac catheterization equipment with the amenities of a full-service operating room. This less-invasive technique uses small incisions and allows surgeons to operate with greater precision and control. The robotically trained surgeons at JFK Medical Center are recognized as pioneers and often host visiting surgeons from all over the country to observe their robotic cases.

Comprehensive Cancer Institute

The oncology team at the Comprehensive Cancer Center at JFK Medical Center works closely with each patient, from diagnosis through post-treatment. Our dedicated oncologists and oncology staff use the most current treatments and technology to deliver the most-effective cancer therapies, including Intensity Modulated Radiation Therapy (IMRT), Image Guided Radiation Therapy (IGRT) and High Dose Radiation (HDR) Brachytherapy. The Neuroscience Center at JFK Medical Center is at the forefront of stroke diagnosis, treatment and prevention. Our stroke program has been certified by the Joint Commission and given its “Gold Seal of Approval” for providing the greatest breadth and depth of specialists and advanced services for stroke patients. We were the first in Palm Beach County to achieve the distinction of Comprehensive Stroke Center by AHCA in 2006 and we are proud to provide care to stroke patients throughout Palm Beach, Martin, and St. Lucie Counties. Our 36-bed nursing Neuroscience unit and our highly specialized 18-bed Neuro-ICU are dedicated to patients recovering from neurological or neurosurgical procedures. Both are staffed with nurses who are uniquely trained and focused on the specific needs of neuroscience patients.

The Breast Institute

The Breast Institute at JFK Medical Center supports patients from diagnosis through treatment in a comfortable and compassionate environment. Our highly trained staff works closely with each patient to plan and execute the most effective course of treatment. We consistently strive to meet and exceed accreditation standards and have proudly earned NQMBC accreditation, as well as NAPBC accreditation. Our services include screenings to diagnostic mammography, digital mammography, ultrasound, stereotactic biopsy, pre-operative education programs, second opinion consultations, bone density testing and genetic testing.

Orthopedic Institute

The Orthopedic Institute at JFK Medical Center is the first and most experienced hospital on Florida’s East Coast to be designated by The Joint Commission as a Center of Joint Distinction. We take a comprehensive approach to the evaluation, treatment and rehabilitation of musculoskeletal injuries. By integrating the primary care physician and surgeon with a host of clinical support staff, we are able to develop customized treatment plans that help patients return to maximum function as quickly and safely as possible. This collaborative approach enables our clinical team to properly evaluate and prepare patients by continuously monitoring and managing their progress. This unique continuum of care produces outstanding clinical results and high patient-satisfaction rates.

Research at JFK Medical Center

The JFK Medical Center Research Department conducts a variety of clinical studies to test new strategies for medical treatment. Our research team performs treatment trials, prevention trials, diagnostic trials, screening trials and quality-of-life trials. JFK Medical Center is a participant in CISRP, an independent organization founded to educate the public and promote a greater understanding of clinical research participation and its role in public health. We are also affiliated with the WESTERB IRB, which provides review and approval of our clinical studies.

This is an exciting year for Palms West Hospital. Construction is well underway for our 32-bed expansion project, which will bring us to 204 total beds. This expansion will increase capacity in our Pediatric

Our pediatric medical staff includes more than 50 pediatricians and more than 40 sub-specialists in pediatric cardiology, ENT, endocrinology, gastroenterology, neurosurgery, pulmonary and urology. Our Obstetrics unit, also known as “Your New Life Center,” believes in promoting positive family bonds by offering family-centered maternity care. This department is known as the happiest place in the hospital and is focused on providing the mother, baby and family with a safe and memorable birthing experience. From our experienced, caring doctors and nurses to the most advanced technology, we deliver a level of comfort that means so much. Robotics

We have recently expanded our Robotic Surgical program and are fortunate to now have two da Vinci®

Si Surgical Systems. We have broadened our program beyond gynecological and urological robotic-assisted procedures into general surgery. Major surgeries can be performed through the smallest of incisions with the da Vinci®

Surgical System. It is an effective, minimally invasive alternative to both open surgery and laparoscopy. Compared to more traditional procedures, robotic-assisted surgery results in reduced recovery times, less risk of infection, less blood loss and reduced
