A rehab manager contact the employer for a rehab evaluation and will thereafter be in charge of the rehab process.

The rehab manager performs an evaluation and produces a rehab plan, to be approved by Euro Accident.

Regular coordination meetings.

The rehab manager writes a final report, after comeback to work. The employer fills in a claim form and reports to Euro Accident no later than by the 20th day of sick leave.

THE EMPLOYER MUST REPORT SICK LEAVE/WORKING

DISABLEMENT

WHEN MUST THIS BE REPORTED?

Working disablement for at least 8 days and estimated to last for a longer period (>4 weeks): To be reported no later than by the 20th day of sick leave, and also if you hesitate on the length of the sick leave period. Repetitive short-term absence due to working disablement, on at least 6 occasions during a 12-month period: To be reported as soon as these criteria are fullfilled.

Consultation with a registered psychologist/psychotherapist on at most ten occasions of treatment. Medical services by a registered physiotherapist/naprapath/chiropractor on at most ten occasions of treatment.

Initiatives provided by multi-professional teams amounting to at most SEK 30,000.

One (1) consultation with a psychologist for husband/wife/cohabitee/registered partner or child. Vocational guidance, for at most five hours, with the aim of ensuring that the insured will be able to continue their employment with the employer.

Advice and analysis relating to workplace adaptation, exchange of work tasks and vocational training with the employer.

Advice to manager responsible when necessary when an employee shows signs of ill-health. Health and working environment screening (for companies with >20 employees), a web survey focusing on the lifestyle and working environment of employees. Identifies risk group and health group.

COMEBACK

Costs that arose after the employee finished their employment with the policyholder.

Pay or other compensation to the insured, or any substitutes, during the rehabilitation period. Costs for adaptation of the workplace that arose as a consequence of the employer having failed to perform the obligations that normally lie within the employer’s area of responsibility. The insurance does not compensate in any way, should the policy holder and/or the insured refuse to follow the recommended rehabilitation plan.

The insurance is valid without any deductible.

The insurance reimburses costs for measures listed in the rehabilitation plan, see earlier slide, up to SEK 75,000.

Liability period: 12 months from the day of notice.

Recurrent work disablement: after 12 months of full working capability.

0.0 – 2.5 % = SEK 768

2.6 – 4.5 % = SEK 996

4.6 – 6.0 % = SEK 1.488

COMEBACK

ComeBack can be applied for by a person who is aged 16-64 years.

Resident and registered as resident in the Nordic countries, and and also entitled to compensation from a Swedish social insurance agency or the corresponding in a Nordic country.

Health requirement: Fully capable of working when joining the scheme.

ComeBack can be applied for as a standalone cover, for a defined group of ten individuals or more. ComeBack is included in EuroAccident’s long term disability cover, ’PlanSjuk’, without additional cost.

Health and working environment screening is a web survey, focusing on the lifestyle and working environment of employees, consisting of 41 questions concerning health habits, health experiences, health data, the degree of motivation to change the lifestyle. work situation, workgroup, nearest

manager, physical environment. It results in a status report showing the distribution of staff divided in a Risk Group and an Health Group.

HEALTH AND WORKING ENVIRONMENT SCREENING

Employees in the Risk Group have in average 10 more sick days annually, than employees in the Health Group.

One sick day costs around 10 % of the monthly wages, production losses not accounted for. 10 sick days x 10 % of the monthly wages = 1 month wages
