Abstract: Between 1985 and 1992 County K i l k e n n y was the site of a n evaluated programme of community intervention w h i c h aimed to reduce the population's h e a r t disease b u r d e n by promoting the adoption of a healthy lifestyle. T h i s paper analyses data on public awareness of the K i l k e n n y H e a l t h Project's activities collected in a 1990-91 evaluation survey. T h e implic­ ations of the findings for healthy lifestyle promotion programmes supporting the achievement of the targets set i n the recently unveiled national health strategy are discussed.

W h i l e s o m e c h a n g e s i n t h e s o c i a l e n v i r o n m e n t — s u c h a s i n c r e a s e d r e s t r i c t i o n of s m o k i n g i n t h e w o r k p l a c e — a r e e n v i s a g e d , t h e s u c c e s s f u l a c h i e v e m e n t of t h e n a t i o n a l h e a l t h strategy's t a r g e t s a p p e a r s to be p r i m a r i l y d e p e n d e n t o n t h e w i d e s p r e a d a d o p t i o n of a m o r e h e a l t h y l i f e s t y l e w i t h i n t h e p o p u l a t i o n i n r e s p o n s e to t h e s t i m u l u s of p u b l i c c o m m u n i c a t i o n c a m p a i g n s . T h e t h e o r y , p r a c t i c e a n d e v a l u a t i o n of s u c h c a m p a i g n s h a s g i v e n r i s e to a n e x t e n s i v e i n t e r n a t i o n a l l i t e r a t u r e . B u t , w h i l e t h i s i s u n d o u b t e d l y a v a l u a b l e s o u r c e of g e n e r a l i s e d g u i d a n c e for c a m p a i g n p l a n n i n g i n a n I r i s h c o n t e x t , p l a n n e r s a l s o n e e d to t a k e i n t o a c c o u n t t h e specificity of t h e n a t i o n a l - s t a t e s t r u c t u r e s t h r o u g h w h i c h c o m m u n i c a t i o n w i l l be c h a n n e l l e d to a n I r i s h p u b l i c . I I T H E K I L K E N N Y H E A L T H P R O J E C T : E V A L U A T I O N D E S I G N A N D C H A N G E P R O C E S S M O D E L

T h e K i l k e n n y H e a l t h P r o j e c t w a s d e s i g n e d a s a q u a s i - e x p e r i m e n t i n w h i c h a "treatment" c o m m u n i t y r e c e i v i n g a n i n t e r v e n t i o n p r o g r a m m e ( K i l k e n n y ) i s c o m p a r e d w i t h a s i m i l a r "control" c o m m u n i t y w h i c h w a s n o t e x p o s e d to t h e i n t e r v e n t i o n (Offaly). D a t a w e r e collected i n b o t h of t h e c o m m u n i t i e s , p r i n ­ c i p a l l y t h r o u g h d i s e a s e r e g i s t e r s a n d t h r o u g h b a s e l i n e ( p r e - p r o g r a m m e ) a n d t e r m i n a t i o n ( p o s t - p r o g r a m m e ) s u r v e y s . A p e r i o d of five y e a r s i n t e r v e n e d b e t w e e n t h e b a s e l i n e a n d t h e t e r m i n a t i o n s u r v e y s ( O ' M a h o n y , C o n r o y , S h e l l e y , et al, 1991; S h e l l e y , D a l y , G r a h a m , et al., 1991; M u r r a y , 1992).

W i t h i n t h i s d e s i g n n e t c h a n g e — t h a t i s , t h e c h a n g e i n t h e t r e a t m e n t c o m m u n i t y over t h e p e r i o d of t h e p r o g r a m m e n e t of t h e c h a n g e i n t h e c o n t r o l c o m m u n i t y o v e r t h e s a m e p e r i o d — i s t h e k e y o u t c o m e e v a l u a t i o n m e a s u r e . C h a n g e w i t h i n t h e c o n t r o l c o m m u n i t y r e p r e s e n t s t h e a m o u n t of c h a n g e o c c u r r i n g w i t h o u t a n y s p e c i a l i n t e r v e n t i o n b e i n g u n d e r t a k e n : t h e a d d i t i o n a l c h a n g e ( i f a n y ) t h a t r e m a i n s i n t h e t r e a t m e n t c o m m u n i t y after t h i s "secular" c o m p o n e n t i s n e t t e d out c o n s t i t u t e s w h a t c a n be s p e c i f i c a l l y a t t r i b u t e d to t h e i n t e r v e n t i o n p r o g r a m m e . E f f e c t i v e i n t e r v e n t i o n i s e x p e c t e d to s h o w a p a t t e r n of s t a t i s t i c a l l y s i g n i f i c a n t n e t c h a n g e a c r o s s i t s e v a l u a t e d outcomes.

S u p p o r t e d b y f u n d i n g f r o m g o v e r n m e n t s o u r c e s a n d from t h e I r i s h H e a r t F o u n d a t i o n , t h e K i l k e n n y H e a l t h P r o j e c t w a s i n i t i a t e d w i t h t h e a i m of r e d u c i n g h e a r t d i s e a s e a m o n g t h e people of C o u n t y K i l k e n n y b y p r o m o t i n g c o m m u n i t y - w i d e a d o p t i o n of a h e a l t h y lifestyle. T h e choice of K i l k e n n y a s t h e i n t e r v e n t i o n p r o g r a m m e ' s h o s t w a s i n f l u e n c e d b y a n u m b e r of factors. T h e c o u n t y h a s a h i s t o r y of c o m m u n i t y d e v e l o p m e n t a n d i s w e l l k n o w n for i t s c i v i c s p i r i t a n d c o u n t y p r i d e . T h e r e i s a m i x t u r e o f u r b a n a n d r u r a l c o m m u n i t i e s i n t h e c o u n t y . I t w a s a c c e s s i b l e to t h e a c a d e m i c a n d o t h e r n a t i o n a l i n s t i t u t i o n s w h o s e s u p p o r t for t h e d e v e l o p m e n t a n d e v a l u a t i o n of p r o g r a m m e s w a s n e e d e d . T h e S o u t h E a s t e r n H e a l t h B o a r d , w i t h i n w h o s e r e g i o n K i l k e n n y i s located, w e r e w i l l i n g to collaborate w i t h t h e P r o j e c t .

T h e P r o j e c t also a d a p t e d a U S p r i m a r y s c h o o l p r o g r a m m e Race to Health a n d f a c i l i t a t e d i t s i n t r o d u c t i o n i n t o K i l ­ k e n n y schools. A t second l e v e l i t offered s u p p o r t for t h e p r o v i s i o n i n K i l k e n n y s c h o o l s of a n I r i s h - d e v i s e d , j u n i o r c y c l e l i f e s k i l l s p r o g r a m m e , Learning for Life. C o o k e r y d e m o n s t r a t i o n s o r g a n i s e d by P r o j e c t s t a f f w e r e i n i t i a l l y offered a s p a r t of t h e a d u l t e d u c a t i o n p r o g r a m m e of t h e K i l k e n n y V o c a t i o n a l E d u c a t i o n C o m m i t t e e ( V E C ) : l a t e r t h e P r o j e c t d e v e l o p e d Look Good, Feel Good c l a s s e s , c o m b i n i n g a n e x e r c i s e s e s s i o n w i t h d i s c u s s i o n of a h e a l t h -r e l a t e d topic, a n d i n t -r o d u c e d t h e s e a s a p -r o g -r a m m e option. D u -r i n g t h e c o u -r s e of t h e P r o j e c t s p e a k e r s g a v e l e c t u r e s a n d t a l k s to a w i d e r a n g e of K i l k e n n y c o m m u n i t y groups. I n c o n j u n c t i o n w i t h t h e I r i s h C o u n t r y w o m e n ' s A s s o c i a t i o n ( I C A ) m e e t i n g s w e r e h e l d for a l l t h e p a r i s h e s i n t h e c o u n t y d u r i n g t h e Project's f i r s t t w o y e a r s .

I n t h e c o m m u n i c a t i o n s m e d i a field t h e P r o j e c t from t h e o u t s e t e s t a b l i s h e d l i n k s w i t h t h e l o c a l p r e s s . I n N o v e m b e r 1 9 8 5 a " H e a l t h y People" c o l u m n w a s i n i t i a t e d i n t h e w e e k l y Kilkenny People n e w s p a p e r . A r t i c l e s o n h e a l t h y e a t i n g w e r e also r e g u l a r l y p l a c e d i n t h e Kilkenny Standard after i t s a p p e a r a n c e on t h e s c e n e i n 1989. I n M a y 1986 t h e P r o j e c t l a u n c h e d i t s o w n free n e w s l e t t e r CATCH, of w h i c h t h r e e i s s u e s w e r e u s u a l l y p r o d u c e d e a c h y e a r . CATCH w a s d i s t r i b u t e d w i t h t h e Kilkenny People a n d w a s a l s o a v a i l a b l e t h r o u g h o u t l e t s s u c h a s d o c t o r s ' s u r g e r i e s a n d l o c a l h e a l t h c e n t r e s . W i t h t h e a d v e n t of l i c e n s e d l o c a l r a d i o b r o a d c a s t i n g , P r o j e c t s t a f f b e c a m e r e g u l a r c o n t r i b u t o r s to a l u n c h t i m e c h a t s h o w on Radio Kilkenny. T h e P r o j e c t a l s o d e v i s e d a s e r i e s of i n f o r m a t i o n l e a f l e t s o n c o r o n a r y h e a r t d i s e a s e r i s k f a c t o r s , p r o d u c e d videos o n h o w to m a k e h e a l t h y food choices a n d p u b l i s h e d i t s o w n cookbook. C o m p e t i t i o n s a n d f e a t u r e s i n CATCH d o v e t a i l e d w i t h t h e d i s t r i b u t i o n of m a t e r i a l s i n schools w h i l e video s h o w i n g s s t r u c t u r e d a second r o u n d of i n t e n ­ s i v e c o n t a c t s w i t h c o m m u n i t y groups d u r i n g t h e f i n a l t w o y e a r s of t h e e v a l u ­ a t i o n p e r i o d . C o m m u n i c a t i n g i n t e r v e n t i o n p r o g r a m m e m e s s a g e s t h r o u g h c h a n n e l s a c c e s s i b l e to both K i l k e n n y a n d O f f a l y r i s k e d " c o n t a m i n a t i n g " t h e q u a s i - e x p e r i m e n t b y e x p o s i n g t h e c o n t r o l c o m m u n i t y to t h e i n t e r v e n t i o n . A s e t of q u e s t i o n s a b o u t k n o w l e d g e of t h e P r o j e c t a n d t h e c h a n n e l s t h r o u g h w h i c h t h i s h a d b e e n a c q u i r e d w e r e i n c l u d e d i n t h e t e r m i n a t i o n s u r v e y c a r r i e d o u t i n C o u n t y K i l k e n n y i n 1 9 9 0 - 9 1 . T h e s a m p l i n g f r a m e , f r o m w h i c h a s a m p l e of n a m e s a n d a d d r e s s e s w a s d r a w n , w a s t h e R e g i s t e r of E l e c t o r s . T h i s i s a n a n n u a l l y r e v i s e d l i s t i n g of a l l t h o s e eligible to vote i n l o c a l , n a t i o n a l or E u r o p e a n e l e c t i o n s . T h e s a m p l i n g w a s p e r f o r m e d u s i n g t h e R A N S A M " c o m p u t e r p r o g r a m m e (see W h e l a n ( 1 9 7 9 ) for a d e t a i l e d d e s c r i p t i o n ) w h i c h i m p l e m e n t s a m u l t i - s t a g e r a n d o m s a m p l e i n c o r p o r a t i n g b o t h s t r a t i f i c a t i o n a n d c l u s t e r i n g , a n d g i v i n g e a c h i n d i v i d u a l o n t h e r e g i s t e r a n e q u a l p r o b a b i l i t y of b e i n g selected. A d e t a i l e d s t u d y of t h e q u a l i t y of t h e R e g i s t e r a s a s a m p l i n g f r a m e ( K e o g h a n d W h e l a n , 1986) f o u n d t h a t i t w a s i n r e a s o n a b l e c o n c o r d a n c e w i t h t h e p o p u l a t i o n a s m e a s u r e d b y t h e C e n s u s .

T h e m a i n d e f i c i e n c i e s i d e n t i f i e d i n t h a t s t u d y , u n d e r - r e p r e s e n t a t i o n of y o u n g s i n g l e p e r s o n s a n d n e w l y - f o r m e d h o u s e h o l d s h a v e l i t t l e r e l e v a n c e i n t h e p r e s e n t c o n t e x t a s i n c l u s i o n w a s r e s t r i c t e d to t h o s e a g e d b e t w e e n 3 5 a n d 6 4 .
