Biographical SummaryChris Smith joined Regence in 2006 to develop a strategic focus that has helped expand and enhance the company’s group and individual dental lines of business. Regence and its subsidiaries currently provide dental benefits for nearly one million members in several markets throughout its four-state service area of Washington, Oregon, Utah and Idaho. Chris now leads the Specialty Business Division which is responsible for developing and growing the company’s dental, vision, pharmacy, and member discount program and employee assistance program (EAP) products. His work on innovative individual dental products was shared at the ADA National Dental Benefits Conference in 2007 where he was a key note speaker and a member of an expert panel of dental benefits managers in 2008.
Chris is an active participant in several organizations that serve professionals in the health insurance and dental industry. He currently serves as Immediate Past President of the national Dental Benefits
Resource Association (DBRA) Board of Directors to advance the adoption of consumerism in the dental market. He also is an active member of both the National Association of Health Underwriters (NAHU) as well as his local chapter, Washington Association of Health Underwriters (WAHU), in Washington State. Prior to joining Regence, Chris worked with Premera BlueCross and Washington Dental Service (a Delta Dental licensee in Washington State with over two million members) as a market and competitive intelligence analyst.
Chris graduated from Western Washington University with a degree in Marketing and Business
Administration. He applies his experience in market research and product development to his role as a guest lecturer, virtual mentor and College of Business and Economics Advisory Board member at Western Washington University in Bellingham, WA.

Laura Smith- Delta Dental Washington Dental Services- President and CEO
Laura Smith is the President and CEO of Washington Dental Service Foundation. As community advocates for oral health, the WDS
Foundation is working to ensure that prevention and early treatment of oral disease -- with an emphasis on very young children and seniors -- are priorities. The Foundation works with partners to develop and execute
innovative solutions that provide lasting improvements to systems of care. The Foundations’ work includes training providers in health care, early learning, and in the senior community about including oral health in their work.
She brought to this work a commitment to improving the health of children and their families and a solid back ground in planning, execution, evaluation and financial management. From 1992 to 2001, she served as a Major Gift Director at Children’s Hospital Foundation. At Children’s, she worked with members of the Hospital and Foundation Boards to lead campaigns to reach fundraising goals and with department directors, researchers and others to design successful projects. During her career she has also held finance and management positions with the Seattle Parks Department, the City of Seattle’s Department of Management and Budget, and the Department of Social and Health Services. She started her career as a case worker and worked directly with families and disabled adults.
Ms. Smith has an undergraduate degree in Sociology from Washington State University and a Master’s Degree in Public Affairs (MPA), from the Evan’s School of Public Affairs at the University of Washington.

Thomas Gates- Delta Dental Washington Dental Services - VP of Planning and Development
Tom Gates is responsible for leading annual and on-going strategic planning efforts, as well as supporting WDS growth strategies through developing new business ventures and external business alliances. Tom joined WDS in 2001, following a ten-year career in management consulting, where he led a wide variety of strategy, finance and operations consulting projects for over twenty major health systems and payers across the US. Tom holds an MBA from the University of Washington, as well as an economics BA (with honors) from Pomona College.

Analytics Department
David DiGiuseppe is the Director of Community Health Plan of Washington’s Strategy and Analytics Department. He participated in the design and implementation of CHPW’s dental benefits offered under its Medicare Advantage product line. David represents CHPW in many settings, including the Office of the Insurance Commissioner’s RRR Workgroup on risk mitigation. David has been with CHPW for 8 years.

Casper Yu- Kaiser Permanente NW Region- Director of Dental Marketing and Plan Strategy
Casper Yu is the Director of Dental Marketing and Plan Strategy for the Kaiser Permanente Northwest Region. He is the business lead of the NW Region’s Dental Growth Pillar, which includes sales,marketing, pricing, product development, and product management for the dental line of business. Casper represents the Dental Care Program in new sales and retention efforts. He is also accountable for the Health Care Reform and State Exchange work for the Dental Program. Additionally, Casper is the communications lead, resulting in accountability for all internal and external customer communications, marketing collateral, and the ownership of the Dental Program’s intranet and internet websites. Casper joined Kaiser Permanente in 2008, and has launched three insurance products in his tenure contributing to significant membership growth within the Dental Program. Casper holds a BS in Marketing from Wheeling Jesuit University (Wheeling, WV) and an MBA from the University of Oregon's Executive MBA Program.
About The Kaiser Permanente Dental Care Program:
Since beginning in 1974 with one office, the Kaiser Permanente Dental Care Program has grown to 17 dental offices that serve more than 198,000 members in the Portland-Vancouver metro area, Longview, and Salem. It is now one of the largest group-practice dental programs in the country. In early 2012, the Dental Program became the first in the Northwest – and third in the nation – to attain dental home accreditation for all 17 of our offices. The Dental Program also offers a dental PPO offering with a very large and extensive network of community dentists in the state of Washington.

Community Minded Enterprises history:In 1995, seventy community leaders participated in a community health assessment process that examined the “broader determinants of community health.” These leaders recognized that community health is more than physical health or access to healthcare; it includes a variety of factors such as economic vitality, healthy environments, access to work and recreational activities, civic engagement, and building assets in youth and families. It was fully launched in 1997 as a “catalyst organization” dedicated to facilitating breakthroughs in community health. Community-Minded Enterprises is dedicated to empowering organizations and individuals to create and expand opportunities for community wellness. We strive to continue to meet the needs of the community and state through innovative social service programs such as Health Care Access, Child Care Resource and Referral, Inland Northwest Early Learning Alliance, Spokane Connection, Community-Minded Television, and other key programs. Kathy continually strives improve our community through continued partnerships,
sustainable programs and innovative initiatives. Kathy Thamm Bio:
I started with FCR when it started and I think that was 1989. The program had 3 employer contracts, Spokane County employees, Sacred Heart Medical Center and Community Mental Health. I stayed for one year and then left to work in our graphics/sign business. In 1994 I re-joined FCR and have been directing the Division since 1996. In October of 2011 I was offered the role of Executive Director of Community Minded Enterprises. I oversee 9 major programs and several initiatives. One key program is the Health for All program which currently has a CHIP grant from CMS.
Outside interests include:
Art, photography, gardening, and traveling. My husband and I maintain an art studio in Bisbee, Arizona and run down there to escape for part of the Spokane winters. Bisbee is an old copper mining town, one mile up in the Mule Mountains about 6 miles from the Mexican border....unchanged and very rural.

Dr. Moffett Burgess, PhD, DDS- Public Health- Seattle & King County- Chief of Dentistry
Dr. Moffett Burgess received her PhD from the University of Washington’s School of Public
Health and Community Medicine in 1998. She graduated from the University of Washington’s
Dental School in 2003. She worked as a public health dentist and then took on the role of Chief
of Dentistry in 2007 at Public Health-Seattle & King County. She oversees five community
dental clinics serving low-income adults and children, two jail health sites, a school based
prevention team, and a mobile dental program for homeless people. Dr. Burgess is active in
numerous local and statewide forums related to oral health advocacy and best practices. She is
an active participant in the dental network of the Washington Association of Community &
Migrant Health Centers and with the Seattle-King County Dental Association. She brings a
public health perspective to issues of dental care access and quality, and in recent years has
been active in such areas as water fluoridation advocacy; analysis of state Medicaid dental
utilization data to better understand the population dental needs (with Washington Dental
Service Foundation); and production of an annual epidemiologic survey of the dental health of
King County’s school-age children. With her thorough understanding of the dental health
needs of the underserved population and broader dental public health issues, Dr. Burgess
would bring a unique perspective to the Technical Advisory Committee addressing dental plans
in the Exchange.

Dr. Benjamin Ruder received his B.A. from Cornell University in 2001, his D.D.S. from the University Of Washington School Of Dentistry in 2006, and his specialty certification in Pediatric Dentistry from Children's Hospital of Wisconsin in 2008. Growing up in Kennewick, WA I moved to Seattle to attend the University of
Washington for both my undergraduate degree and my DDS at the School of Dentistry. After graduation I returned to Eastern Washington and chose to pursue a job in public health, originally to qualify for loan repayment through the National Health Service Corps and concentrate on doing dentistry rather than being distracted by issues of owning a private business, but came to appreciate the importance of Public Health and the role I could have within it. I also began working one day a week as an instructor at Columbia Basin College shortly after they opened the School of Dental Hygiene, mostly in
restorative dentistry working directly with the students and patients in a clinical setting. I became the Dental Director for Tri-Cities Community Health in early 2009 and quickly tried to increase our reach within the Tri-Cities area. Since then we have begun a school based program, mostly in elementary schools, but now expanding to an alternative high school here in Pasco as well. We are also trying to develop a referral system with the Tri-Cities Cancer Center for patients with dental needs as well as work on hospital ED diversion. As a dentist for a Community Health Clinic and a member of the WACMHC Dental Committee I believe I can bring a valuable perspective to the Technical Advisory Committee on Dental Plans in the Exchange.

John L. Gibbons, DMD, PC- Pediatric Dentist
Dr. John Gibbons is a pediatric dentist who has been in private practice for 27 years, with an
office in both Tacoma and Silverdale, Washington. Dr. Gibbons received his Doctor of Dental
Medicine (DMD) from Washington University and completed a two year pediatric residency at
Oregon Health Sciences University.
Dr. Gibbons is an affiliate professor at the University of Washington School of Dentistry where
he has volunteered his time supervising the dental students during their rotation through the
pediatric dental clinic. He was chosen to participate on a panel of experts on the subject of
Behavior Management for the annual meeting of the California Society of Pediatric Dentistry.
He is a provider for the ABCD program and dedicates three days of his Tacoma practice for
Medicaid-enrolled children. He had the privilege of working two weeks in an orphanage in
Honduras, where not only was he able to work on the children but also teach some of the local
dentists about pediatric dentistry.
Dr. Gibbons is a member of the American Dental Association, American Academy of Pediatric
Dentistry, Western Society of Pediatric Dentistry, Washington State Dental Association,
Washington State Academy of Pediatric Dentistry, and the Kitsap Dental Society. He served as
the President of the Washington State Academy of Pediatric Dentistry (District IV of the AAPD)
and is the current Vice-President of the Western Society of Pediatric Dentistry.

Bracken Killpack, Washington State Dental Association
Bracken Killpack is the Director of Government Affairs at the Washington State Dental Association which represents approximately 4,000 dentists across the state. Mr. Killpack leads WSDA’s efforts around health care reform, dental workforce issues, reducing barriers to dental care for underserved populations, and strengthening the state’s dental safety net. He is also responsible for educating member dentists and their dental office staffs about all rules and laws that relate to the practice of dentistry in Washington State.
Mr. Killpack has worked on dental policy for five years and has an extensive knowledge about the state’s dental workforce and the major differences between the state’s medical and dental delivery models. He currently serves on the Washington State Health Care Personnel Shortages Task Force and the advisory board of the Seattle Central Community College Dental Hygiene program and previously served on the Washington State Oral Health Coalition.
Mr. Killpack received his BA in Political Science from Willamette University and served as a Community Economic Development Consultant with the U.S. Peace Corps in Panama for two years.

Lisa A. Block, DMD, MS, PC
Dr. Lisa Block is a board certified pediatric dentist who has practiced in Gig Harbor, WA for 19 years. Dr. Block received her Doctor of Dental Medicine (DMD) from Washington University and her Masters in Oral Biology from the University of Missouri. Dr. Block also completed a general practice residency at University Hospital in Jacksonville, Florida and a pediatric dental residency at Children’s Mercy Hospital in Kansas City, Missouri.
Dr. Block has been a longtime advocate for the oral health of children. She has been a state and regional leader for the Access to Baby & Child Dentistry program, a public/private partnership that trains dentists and primary care medical providers to provide preventive care and treatment to Medicaid-enrolled children up to six years old. Dr. Block has been an ABCD trainer in Washington and Montana.
Dr. Block is a member of the American Dental Association, American Academy of Pediatric Dentistry, Washington State Dental Association, Pierce County Dental Society, and currently serves as the Treasurer of the Washington State Academy of Pediatric Dentists. She also serves on the dental committee for Mary Bridge Children’s Hospital and Tacoma General Hospital and is a former board member of the Washington Oral Health Foundation.

Dr. Christopher Delecki DDS, MBA, MPH
Dr. Chris Delecki is a dentist who has dedicated his career to public health dentistry. Dr. Delecki received his Doctor of Dental Surgery (DDS) from the University of Michigan, his Master of Business
Administration from City University, and his Master of Public Health from the University of Michigan. Dr. Delecki has also completed a dental public health residency with the Indian Health Service.
Dr. Delecki has been the Director of the Dental Program at the Odessa Brown Children’s Clinic in Seattle since 1998. The Odessa Brown dental program is affiliated with Children’s Hospital in Seattle and handled over 12,000 dental visits in the last fiscal year. From 1977-1998, Dr. Delecki served as a
Commissioned Officer with the U.S. Public Health Service where he managed and consulted for over 40 Indian Health Service dental clinics. Additionally, Dr. Delecki has worked on several dental research projects, developed curriculums for training physicians and patients on oral health, and serves as an Affiliate Assistant Professor at the University of Washington School of Dentistry.
Dr. Delecki is a member of the American Dental Association, American Academy of Pediatric Dentistry, Washington State Dental Association, and is Past President of the Seattle King County Dental Society. He is also a member of the advisory boards of the Seattle Central Community College Dental Hygiene program and the Seattle Vocational Institute Dental Assisting programs and served as an oral health consultant for the American Indian and Alaska Native Head Start programs.

Joel serves as the Chair of the Department of Pediatric Dentistry at the University of
Washington and as Dental Director at Seattle Children’s Hospital. He is also the Associate Dean
for Hospital Affairs at the University of Washington School of Dentistry, and is presently
President-elect for the American Academy of Pediatric Dentistry. Dr. Berg previously held
executive positions at Philips Oral Healthcare and ESPE Dental. He is the author of a multitude
of published manuscripts and abstracts, and is a co-editor of a textbook on early childhood oral
health.

Dr. Alejandro Narváez - Sea Mar CHC- Deputy Director
Dr. Alejandro Narváez has been Dental Director of Sea Mar’s dental services since 1982. He
oversees the clinical practice and operations of Sea Mar’s 14 dental clinics in western
Washington.
In 1977, Dr. Narváez earned a Bachelor of Arts degree from Stanford University and completed
an independent internship at Moses Lake Community Health Center.
In 1982, he earned a Doctor of Dental Surgery degree from the University of Washington and
completed a course for general practitioners in orthodontics in 1998. He presently sees patients
at the White Center and South Park clinics, with a focus on children and limited orthodontics.
Dr. Narváez brings a vital perspective to the discussions around dental coverage in the
exchange. He would be uniquely suited to provide expertise to the ad-hoc dental
subcommittee.
