People services provided by 44 different organisations.

The Supporting People programme aims to improve the

quality and life for a diverse range of vulnerable adults and

young people in Westminster who at times may need to

access Primary Care, Adult and Children Services in

Westminster.

A conference was held on 8

accessing the range of statutory and healthcare services by

providers of housing related support and this contact list

has been designed to assist housing related support service

providers to access social care services and the range of

primary health-care services in Westminster. Also included

are details of Primary Care Trust services discussed at the

conference, in particular the Westminster Rehabilitation

and Rapid Response Service.

There are a range of services across Westminster and are

generally divided into North and South with access to these

services covered by geographical areas.

This information should be made accessible for front-line

workers to ensure referrals can be made to services where

a tenant’s health deteriorates or where advice is sought to

meet the needs of vulnerable people in Westminster.

WESTMINSTER RAPID RESPONSE TEAM

The new Westminster Rehabilitation and Rapid Response Service is a multi-disciplinary team providing therapy and rapid response community nursing to residents of Westminster

What do we do?

The aim of the service is to keep Westminster residents healthy at home. Nurses therapists and support workers will see clients in their homes or residents or residential homes and other community settings.

Rapid Response Nursing Team (RRNT)

The team provides intensive community nursing assessment and care for prevention of admission to hospital and A&E.

Who makes up the Westminster Rehabilitation and Rapid Response Service?

Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Dieticians, Podiatrists, Psychologists, Rehabilitation Assistants and Support Workers

Who can refer?

Any health or social care professional can refer. Members of the public can self refer into the rehabilitation service.

How do you refer?

There are different ways to refer into Westminster Rehabilitation Service and the Rapid Response Nursing Team. The key difference is that you can only make referrals by phone into the Rapid Response service, whereas you can phone and fax into the rehabilitation service.

Who should be referred to the service?

Westminster residents 16 years and over requiring rehabilitation or intensive community nursing support that will avoid a preventable hospital admission.

What is unsuitable for referral?

Undiagnosed medical emergency

When care can be provided through existing services to patients/clients

When patients, carers or health/professionals are not able to participate in a treatment plan Patients of the Westminster Rehabilitation Service must have the potential to improve their

functional status.

When will the patient be seen?

WESTMINSTER REHABILITATION SERVICE MULTI-PROFESSIONAL TEAM

The Westminster Rehabilitation Service is a multi-professional team (with bases in the north and south). The Team consists of:

♦ Administrators ♦ Care Managers ♦ Clinical Psychologist ♦ Dietician ♦ Occupational Therapists ♦ Physiotherapists ♦ Podiatrist ♦ Rehabilitation Assistants ♦ Speech and Language Therapists

The team sees clients in their own homes or other community settings. The team promote safety and independence in daily activities (e.g. personal care, domestic tasks and functional mobility) by using a multidisciplinary goal setting approach. The team provides an urgent intervention service (to avoid unnecessary admission to hospital and / or minimise significant and urgent risks of injury or ill-health) as well as non-urgent intervention. Most clients require intervention for a period of several weeks, although this may be shorter or longer, depending on clients’ needs.

Acceptance Criteria:

The team sees people who: ♦ are 16 years or over

♦ live in the borough of Westminster

♦ require rehabilitative intervention at home or residential home (i.e. not living in a nursing home) ♦ require input from one of the following professional groups:

- Occupational Therapy - Physiotherapy

- Speech and Language Therapy

(with or without need for input from the other professionals in the service) ♦ are medically stable

♦ are in agreement with a referral being made and are willing to actively participate in a rehabilitation programme

♦ have a home that is safe and conducive for rehabilitation treatment to be provided

Exclusions:

♦ Need for non-mobility equipment or minor adaptation assessment only (should be referred to Dependability)

♦ Need for input from a Dietician only (should be referred to the Community Dietetics Service) ♦ Need for input from a Podiatrist only (should be referred to the Community Podiatry Service) ♦ Need for input from Nursing only (should be referred to the District Nursing Service)

♦ Clients living in nursing home accommodation (Therapy needs should be referred to the locum Occupational Therapist covering nursing and continuing care accommodation. She can be contacted via Garside House Nursing Home, Tel: 020 8237 2468).

FALLS PREVENTION SERVICE

The Falls Prevention Service aims to reduce the incidence of falls, improve clients’ physical ability and confidence, promote healthy living, raise awareness of falls and introduce clients to community life. The service provides a group intervention programme within local community centres, including:

♦ Health promotion - presentations to raise awareness of falls and ways in which to prevent them ♦ Education on preventing falls – provision of information on how to reduce the incidence of a fall at

home and outdoors, including guest speakers providing information on diet, exercise, safety in the home, foot care, anxiety management and relaxation, and road safety.

♦ Exercise classes – to improve strength, balance, flexibility, endurance, walking, posture, self confidence and independence

♦ Continued support and follow-up – including referral to other community-based classes to continue with progress

The Fall Prevention Service Team includes: ♦ Exercise Physiologist

♦ Physiotherapist ♦ Occupational Therapist ♦ Rehabilitation Assistants

♦ Guest Speakers including a Dietician and Podiatrist

The team sees people who: ♦ Are 16 years or over

♦ Live in the borough of Westminster

♦ Are able to independently mobilise with or without use of a walking aid ♦ Have a history of falls or fear of falling

♦ Are medically fit to participate in an exercise programme

♦ Are willing and motivated to make a commitment to a weekly group for a period of 16 weeks.

Exclusions:

♦ Significant problems with cognition (memory, concentration or attention etc) which would affect their ability to participate in a group or to carry out a home exercise programme

♦ Clients requiring a rapid response or crisis intervention service

NORTON HOUSE REHABILITATION UNIT (INTERMEDIATE CARE)

Norton House Rehabilitation Unit is an 8-bedded unit housed within a residential home in Victoria. The Unit promotes independence and quality of life for older people living in Westminster by preventing unnecessary hospital admissions, long hospital stays, and by helping people to remain living at home. The service provides short periods of rehabilitation (approximately 6 weeks depending upon individual needs) in a safe, residential environment.

The Norton House rehabilitation team consists of: ♦ Rehabilitation Unit Manager

♦ Physiotherapist ♦ Occupational Therapist

♦ Rehabilitation Assistants/Care Assistants

Team members work closely with other professionals including Podiatry, Dietetics, Speech and Language Therapists, and clients have access to Care Managers, GPs and District Nursing services, as they would from their own home.

Acceptance Criteria

The team sees people who: ♦ Are 65 years or over

♦ Live in the Borough of Westminster ♦ Are medically stable

♦ Are motivated and have the potential to benefit from a period of rehabilitation ♦ Have no greater need than residential care

♦ Have an identified discharge destination

♦ Are able to transfer with the assistance of one carer ♦ Have an allocated Care Manager

♦ Any client who requires more than residential home level of support ♦ Any client who cannot comprehend and follow a treatment programme

RHEUMATOLOGY SPECIALIST OCCUPATIONAL THERAPIST

The provision of Rheumatology Specialist Occupational Therapy to Westminster residents is a new improvement within Westminster Rehabilitation Service. The service is still in an early stage of development and at present sees clients either in their own homes or in a weekly clinic session at the South Westminster Clinic, in Vincent Square. The Specialist OT carries out assessment and intervention including:

♦ Splinting

♦ Education and advice on rheumatological conditions, joint protection techniques, energy conservation and work simplification

♦ Provision of small aids and equipment ♦ Provision of home exercise programmes

The Rheumatology Specialist Occupational Therapist works closely with the north and south teams of Westminster Rehabilitation Service and also provides advice to, or may do joint home visits with, other team members working with clients with rheumatological conditions.

The types of client the OT may see include those who:

♦ are experiencing recent or current arthritic flare-up (i.e. an acute episode) OR ♦ have recently been diagnosed with rheumatoid arthritis OR

♦ have an upper limb condition as a result of arthropathy (e.g. generalised wrist or thumb/finger pain, inflammation, swelling, or experiencing difficulty with a specific task involving upper limb use such as hand writing, specific sports or hobbies or care giving duties) OR

♦ have a specific upper limb condition such as carpal tunnel syndrome, de Quervain’s tenosynovitis and occupational overuse syndrome.

The Rheumatology OT sees people who: ♦ are 16 years or over

♦ require input from an Occupational Therapist specialised in rheumatology

Exclusions:

♦ Clients who are experiencing general functional difficulties, not specifically related to a rheumatological condition

♦ Clients with rehabilitation issues which could be addressed by a general rehabilitation Occupational Therapist (i.e. do not require an OT with specialist knowledge and skills in rheumatology)

INJURY PREVENTION THERAPIST FOR INFORMAL CARERS

The Injury Prevention Specialist is a new initiative and is due to start in September 2004. The post-holder (an Occupational Therapist) will work with informal carers and those they are caring for, in order to provide intervention to minimise risks of injury due to inappropriate manual handling and to improve carers’ ability to manage the needs of the person for whom they are caring.

The post-holder will:

♦ Liaise with carers in the community on what their needs are and develop a service that is user led to help meet these needs

♦ Carry out moving and handling assessments and provide intervention as required ♦ Provide education on, and promotion of back care and general health

♦ Provide information on community support services for carers

The Injury Prevention Specialist will work closely with the north and south teams of Westminster Rehabilitation Service and will also provide advice to, or may do joint home visits with, other team members working with clients with complex needs and their carers.

(Provisional) Acceptance Criteria:

The Injury Prevention Specialist works with those who care for people that: ♦ are 16 years or over

and where there exists a significant risk of injury or difficulty managing care needs
