Eligibility Minimum Hourly Requirement
You are eligible if you are an exempt full-time faculty or staff member working a minimum of 35 hours per week
Waiting Period for Benefits 6 months from date of hire
Elimination period There is no elimination period. Benefits will commence on the 1st day for injury or sickness.
Benefit Percentage Week 1-14: 100% of your Base Weekly Earnings Week 15-26: 60% of your Base Weekly Earnings
Maximum Duration 26 weeks

Successive Periods of Disability
If you become disabled for the same condition within 90 days following your prior disability, your benefits will continue under the same claim.

Partial Disability Partial disability benefits can be payable if you are totally or partially disabled for at least 10 work days and suffer a 20% or greater loss of earnings.

Family and Medical Leave Act (FMLA)
Eligibility and Service Time Requirements
You are eligible if you have been employed by DePaul for at least 12 months and have at least 1,250 hours of active employment during the 12 month period immediately preceding the start of the leave.
Entitlement/Benefit Duration 12 weeks in 12 month period (26 weeks if caring for a family member in the Armed Forces)
Benefit 12 weeks of unpaid job-protected leave unless coordinated with Short Term Disability or sick pay or if vacation time applied.

Basic Life Insurance: Part-time employees
Eligibility Class 1: Active part-time faculty Class 2: Active part-time staff
Employees who are covered by a collective bargaining agreement and independent contractors are excluded. You must be deemed health and welfare benefits eligible by meeting the minimum requirements outlined below.
Minimum Hours Class 1: Taught at least six 4-credit hour courses (or the equivalent) in the previous academic year (or seven courses including the summer session) and are anticipated to teach the same number of courses for the upcoming academic year.
Class 2: Worked at least 1,000 hours in the previous calendar year based on payroll records and who are expected to work at least 1,000 hours in the upcoming calendar year.
Waiting Period Class 1: 1 year of continuous service (determined by academic year, September to June).
Class 2: 1 year of continuous service (determined by calendar year, January to December).

An amount equal to 1, 2, 3, 4, or 5 times your annual earnings not to exceed $500,000.
The non-medical maximum is 1, 2, or 3 times your annual
earnings. If you elect for more than 3 times your annual earnings, you will be required to submit a medical questionnaire and be approved by Liberty.
Late Applicant: If you declined coverage before and wish to enroll now, you will need to submit a medical questionnaire and be approved for any amount you elect.
Waiver of Premium: In the event you become totally disabled prior to age 70, you may be eligible to continue your group life insurance. You may apply for the waiver after 6 months of being totally disabled and waiver will term at age 70.
