DIRECTIONS FOR COMPLETING THIS APPLICATION:

1. Type or print clearly in ink. Completing this application does not guarantee scholarship funds.

Davenport University awards scholarships funded by private donations to the Davenport University Foundation. These scholarships are awarded based on which campus location you attend and/or special requirements established by the donors. Correct use of this application allows you to be considered for any Davenport University Foundation scholarship, provided that you meet the criteria. If you are eligible to receive a scholarship from the Davenport University Foundation, you will be notified of your award by the Financial Aid Office.

I understand and agree that if selected as a Foundation scholarship recipient, I will communicate with my donor, remain enrolled at least half-time for the academic year, attend the Foundation Scholarship Dinner (if requested to do so), maintain satisfactory status based on the Davenport University standards of progress, and have my photo taken for use in scholarship publications and the website. Withdrawal from a class may result in a decrease in my award, so I understand that I should check with Financial Aid before withdrawing. I certify that the above information is true and complete. I realize that if any of the above information is found to be false, or if I fail to meet the requirements, I forfeit all monies and claims for scholarship consideration. If you are a nursing major at the Midland campus, are enrolled in clinical courses (Level I or Level II), have satisfactory performance during the fall semester within the nursing sequence, have satisfactory standing in clinical rotation, and have an overall GPA of at least 2.5, you may be eligible to receive the Mary Walsh Kole Memorial Nursing Scholarship. Please attach the following essay to your application to be considered for this scholarship. Your essay must be typed and double-spaced. Remember to attach a reference letter from an extended care facility or nursing home if currently or previously employed. Note: BSN students and students on probation will not be considered.

If you are a nursing major at the Midland campus, are enrolled in clinical courses (Level I or Level II) in fall 2006, have satisfactory performance during the fall semester within the nursing sequence, have satisfactory standing in clinical rotation, have an overall GPA of at least 2.5, and commit to working in the Tri-City area for a period of two (2) years, you may be eligible to receive the Alfred and Eleanor Dinsmore Nursing Scholarship that requires an essay and list of references as part of the application process. Please complete the following essay on an additional sheet of paper and attach it to your application to be considered for this scholarship. Your essay must be typed and 100 words or less in length. Remember to complete your list of references below. Note: BSN students and students on probation will not be considered.
