• Transition is helping students with disabilities and their families think about their life after high school and identify long-range goals designing the high school experience to ensure that

students gain the skills and connections they need to achieve these goals the provision of funds and services to local school districts to assist in the transition process.

• It is a federal mandate requiring youth age 16 and older to have an IEP with a postsecondary transition plan.

• A postsecondary transition plan must include

measurable postsecondary goals for students based on age-appropriate transition assessments related to training/education, employment and, independent living skills (if appropriate) and a description of

transition services including courses of study needed to assist the students in reaching their goals.

• The following provides information on agencies and schools that provide services for students with disabilities that may assist them after high school. In addition, many of these agencies have resources to assist the student financially.

Disability Services

• OVR provides vocational rehabilitation services to help persons with disabilities prepare for, obtain, or maintain employment. OVR provides services to

eligible individuals with disabilities, both directly and through a network of approved vendors.

Services are provided on an individualized basis. The OVR counselor, during face-to-face interviews, assists customers in selecting their choice of

vocational goals, services and service providers. An Individualized Plan for Employment (IPE) is

developed, outlining a vocational objective, services, providers and responsibilities.

Intellectual Disability. The program builds essential job specific and employability skills needed to gain and maintain meaningful

employment, lead productive lives and become integrated in an adult work environment.

Education, has created the Child and Adolescent Brain Injury School Reentry

Program called BrainSTEPS (Strategies Teaching Educators, Parents, and Students). BrainSTEPS is working to make sure that those who must provide educational

support to children with brain injury have a good understanding of brain injury, the resulting challenges, and supports and interventions that will help these students achieve educational success through graduation.

• The BrainSTEPS Committee is comprised of education professionals, medical rehabilitation professionals, and family members; Receives ongoing training from local and nationally recognized leaders in the field of pediatric brain injury; Provides BrainSTEPS 101 presentations, helping schools compensate for the lack of brain injury training in college teacher training programs; Provides training and

consultation regarding identification, school re-entry planning, IEP development, intervention selection and implementation, long-term monitoring of students, and other issues professionals face in supporting students with brain injury; Consults with teams through the BrainSTEPS Program on a case by case basis.
