ADDENDUM NO. 2 July 27, 2021

Bid No.: 21-054 (CD1911)

Bid Name: Abate & Demolish: 1103 E. 8th Street and 1608 Wabash Avenue-CDBG

1. The Mandatory Submittals Page has been revised and is attached to this addendum. This revised page must be used for bidding purposes.

2. Article 2 – Special Provisions has been revised. Sections 2.18, 2.31, and 2.32 are hereby replaced with the following (revisions shown in red):

2.18 PRE-DEMOLITION CONFERENCE

The Contractor shall attend a Pre-Demolition Conference with the Project Manager, City employees, representatives of the utility companies, and/or other interested parties prior to any demolition. Pre- Demolition Conference will be scheduled 7-10 days after notice of award is issued. Contractor is required to submit the following submittals at or prior to the Pre-Demolition Conference:

• Contractor’s Site Management Organization (to include Site Supervisor, Quality Control Manager, and Site Safety Manager with 24-hour contact information)

• Activity Hazard Analysis (AHA)

• Demolition Plan

• Project Schedule

• Project Design for Abatement

2.31 ASBESTOS ABATEMENT

A copy of the Asbestos Abatement Specifications as prepared by the City’s Environmental Consultants and AMS of record, All Phase Environmental Consultants, Inc (APEC), are included with these specifications following Article 3. Prior to commencing the project, contractor shall obtain the following:

A. An abatement and demolition permit from the Colorado Department of Public Health and Environment.

B. A demolition permit issued by the Regional Building Department.

Prior to the commencement of any abatement work, the Contractor shall submit for City of Pueblo review, a Project Design detailing their plan for accomplishing abatement of premises. Both properties shall be abated using containment methods. Open-air abatement is not permitted, variances have been approved by CDPHE.

2.32 DEMOLITION SCOPE OF WORK

Demolish and remove all buildings, foundations, footings, floor slabs, fences, and concrete pathways and driveways from the project site unless otherwise identified by the City Engineer as an item that is to remain. Control dust resulting from demolition by sprinkling and wetting down; and install storm water control measures. Clean up the entire lot/parcel as identified by the City Engineer. This shall include all weeds, trash, abandoned items, etc. Contractor shall fill and compact all holes, pits, excavations, or depressions on the properties to bring the entire site back to grade.

3. A Sample and Material Location Map and Existing Site Conditions for 1608 Wabash Avenue are attached to this addendum.

4. Following are responses to Bidder’s questions:

Q1. Who will be responsible for utility disconnects?

Response: See Article 2-Special Provisions, Section 2.30 “Utility Disconnection”.

Q2. Will water and sanitary be capped within property lines or in the streets at the mains?

Response: See Article 2-Special Provisions, Section 2.30 “Utility Disconnection” for capping requirements.

Q3. Will City expedite termination of gas?

Response: The City has submitted the termination request to Xcel Energy for both properties as of July 20, 2021. It is anticipated that the termination request will be completed prior to start of work.

Q4. Is the ash considered ACM?

Response: See the Sample and Material Location Map for 1608 Wabash Avenue attached to this addendum. Due to the transite materials being mixed in with the ash, this is considered an ACM. CDPHE was informed of this spill on January 26, 2021, and discussed further on February 10 & 11, 2021 by APEC personnel.

Q5. Brandice mentioned discussions with CDPHE about Wabash: What engineering controls were agreed upon to keep the project from not becoming an open-air abatement?

Response: CDPHE was very against this structure being an open-air abatement. The sequence of work that was discussed with them was to treat the fire damaged section of the home (east side) as a major spill that would be cleaned prior to normal abatement. Then all transite that is lying on the soil is to be removed, along with all remaining transite on the home (east end exterior). The transite in the soil is non-friable even though this material is significantly damaged – friability was verified in the field on July 21, 2021. Once the spill is cleaned and cleared, then normal abatement of the interior can be performed. All-Phase Environmental staff walked the interior on July 21, 2021, and they were able to safely access all interior areas needing abatement.

Q6. Is the entire lot at Wabash considered contaminated?

Response: No, only the east side of the home has transite (non-friable) lying in the soil.

Q7. What backfill requirements and specifications must be met?

Response: See Article 2-Special Provisions, Section 2.23 “Site Restoration & Cleanup” and Section 2.33 “BackfillRequirements”.

Q8. Will BMP’s be required after backfill?

Response: BMPs shall be maintained for the duration of the project until the project has been accepted as complete by the City. See Article 2-Special Provisions, Section 2.36 “Erosion and Sediment Control” for requirements and procedures.

Q9. Wabash is currently free of debris in the yard. If debris has accumulated by start date, who will be responsible for the costs of removal?

Response: Contractors shall bid the project based upon the current condition of the site as documented on the Existing Site Conditions for 1608 Wabash Avenue attached to this addendum.

Any significant changes to the site conditions as of the start date of the project will be documented by the City’s Project Manager as a changed site condition.

Q10. Who will work with the neighboring property to the east of 8th Street to meet OSHA regulations of safe fall distances?

Response: Awarded Contractor shall provide a demolition plan to the City for approval as part of at or prior to the Pre-Demolition Conference. Based upon the Contractor’s submitted plan, the City’s Project Manager will work with neighboring properties for notification. The Contractor shall adhere to Article 3-General Provisions, Section 3.64 “Protection of the Public, Work, and Property”.

Q11. Are you asking for a notarized letter from Abatement contractors stating that they have no Notice of Violation (NOV) or any pending NOV?

Response: Yes, abatement contractors shall provide a notarized letter documenting that they do not have a Notice of Violation or any pending Notice of Violation. Refer to the revised Mandatory Submittals Page attached to this addendum.

Q12. There was no spill delineation in the inspection report for 1608 Wabash Avenue. It appears that there is transite siding scattered in the entire backyard, the alley, and possibly the neighboring yards. Will the City’s inspector be providing spill delineation for outside containments?

Response: Transite siding materials are non-friable even though they are damaged lying on the soil. The property does not require a spill delineation by CDPHE. This material does not need a containment on the exterior of the property. This phase of work will only require a notification to the state. All associated transite from the property will need to be cleaned.

Q13. Who will be providing the project designs and variances, if needed, for this project? Will this be the GAC or All Phase Environmental?

Response: See Article 2 – Special Provisions, Section 2.18 “Pre-Demolition Conference” and Section 2.31 “Asbestos Abatement” as amended in this Addendum. The awarded Contractor shall submit a Project Design for review prior to the start of demolition. Abatement shall be accomplished using a containment method. A variance to Open-Air abatement was previously submitted and approved by CDPHE.

Q14. Can the bidding contractor select whether to complete the work in a full containment or with open air work practices with a variance?

Response: No, contractor shall abate both properties using a containment method.

Q15. Can you provide clarification with regard to who is providing water and power?

Response: Contractor shall coordinate water requirements through the Board of Water Works (BOWW). Contractors may check-out a water meter for connecting to existing hydrants or domestic water lines. A backflow preventer will be required to prevent cross-contamination.

This Addendum No. 2 to the above project shall become a part of the Contract Documents and shall be binding in all respects. Information contained in this Addendum No. 2 shall supersede any information presented in the specifications. This Addendum No. 2 must be included in Bidder’s submittal. Walls

This area is an addition and during the site inspection, Transite was found mixed in with the Ash & Debris.

Damage in this area was due to the fire & fire fighting activities. This area will be abated as a spill separate from the normal abatement activities on the interior of the home. All Transite (non- friable) remaining on the exterior walls and lying on the soil to the east of the home will be removed prior to any demolition activities.
