Associate Degree in

Nursing Track

Program

Curriculum Code: AAS.ADN.TRK

Complete ADN Application Includes:

1. Complete Lake Land College Intent to Enroll form online or submit to the Admissions Office: choose the Nursing Track Major (AAS.ADN.TRK).

2. Submit completed ADN Intent form to the Nursing Department. 3. Submit transcripts to the Admissions Office by February 1.

a. Up-to-date high school transcript and final transcript upon graduation. b. All college transcripts, if applicable.

4. Complete Lake Land Compass Test. Students may submit their ACT scores in lieu of taking the placement exam. ACT scores should be reported directly to the college from ACT. Outcome of testing determines whether students are eligible to take the Pre-RN exam.

5. Take NLN Pre-RN exam. Sign up through the Nursing Department.

6. One year of high school Biology or BIO 100 at Lake Land College and one year of high school chemistry or CHM 101, CHM 111 or CHM 120 at Lake Land College with a “C” or above.

7. Points for Healthcare experience.

DEADLINE FOR COMPLETED NURSING DEPARTMENT APPLICATION FILE:

FEBRUARY 1

*NLN Pre-RN test is offered the first Friday of April, September and December.

Payment for NLN test is due three weeks prior. Dates are subject to change.

Cosmetology

Track Program

Curriculum Code: CRT.COS.TRK

Complete Cosmetology Application Includes:

1. Complete the Lake Land College Intent to Enroll form and submit to the Admissions Office: choose the Cosmetology Track Major (CRT.COS.TRK).

2. Complete Lake Land Compass Test - call (217) 234-5301 to schedule.

3. Students may submit their ACT scores in lieu of taking the Compass Test. ACT scores should be reported directly to the college from ACT.

4. Complete all Cosmetology intent packet forms and submit to Peggy Strange, Director of Cosmetology.

5. Submit up-to-date high school transcript to the Cosmetology Department and the Admissions Office.

6. Submit final high school transcript to the Admissions Office and Cosmetology Department upon graduation.

7. Beneficial high school classes include: Art, Business, Chemistry, Computer, English, Psychology and Speech.

Dental Hygiene

Track Program

Curriculum Code: AAS.DH.TRK

Complete Dental Hygiene Application Includes:

1. Submit Lake Land College Intent to Enroll form online or submit to the Admissions Office: choose the Dental Hygiene Track Major (AAS.DH.TRK).

2. Complete Dental Hygiene intent packet by January 31 for the year you are applying. Available from Dental Hygiene office - call (217) 234-5201 to request.

3. Submit transcripts to the Admissions Office by January 31.

a. Up-to-date high school transcript and final transcript upon graduation. b. All college transcripts.

4. Complete Lake Land Compass Test. Students may submit their ACT scores in lieu of taking the placement exam. ACT scores should be reported directly to the college from ACT.

5. College chemistry must be completed with a C or above before entrance into the program.

JANUARY 31 BY 4 P.M. FOR THE YEAR YOU ARE APPLYING.

Esthetics Program

Curriculum Code: CRT.ESTH.TRK

Complete Esthetics Application Includes:

1. Complete the Lake Land College Intent to Enroll form and submit it to the Admissions Office: choose the Esthetics Major (CRT.ESTH.TRK).

2. Complete Lake Land Compass Test - call (217) 234-5301 to schedule. Students may sub-mit their ACT scores in lieu of taking the Compass Test. ACT scores should be reported directly to the college from ACT.

3. Complete all Esthetics intent packet forms and submit to Peggy Strange, Director of Cosmetology.

4. Enrollment into the program occurs in the fall semester only.

Tech Program

Admissions Requirements

1. Complete Lake Land College Intent to Enroll form online or submit to the Admissions Office: choose the John Deere Tech major (AAS.JDAT).

2. Contact Allen Drake, Russell Neu, or Brad Osborne, or Heather Nohren at the contact information below.

3. Complete Lake Land Compass Test and Mechanical Reasoning Test. Students may sub-mit their ACT scores in lieu of taking the Compass test. ACT scores should be reported directly to the college from ACT.

4. Secure participating John Deere Dealer Sponsor and submit dealer agreement to Allen Drake.

Massage Therapy

Track Program

Curriculum Code: CRT.MT.TRK

Complete Massage Therapy Application Includes:

1. Submit Lake Land College Intent to Enroll form online or submit to the Admissions Office: choose the Massage Therapy Track Major (CRT.MT.TRK).

2. Submit the online Massage Therapy Application.

3. Submit official transcripts to the Admissions Office by March 1/July 1 for fall registration or October 1/December 1 for spring registration:

a. Up-to-date high school transcript and final transcript upon graduation. b. All college transcripts.

4. Complete Lake Land Compass Test - call (217) 234-5301 to schedule.

Five semester part-time night program begins each fall semester, three semester full-time day program begins each spring semester. Deadline for five semester part-full-time program is March 1/July 1, while the deadline for three semester full-time program is October 1/December 1.

Admission is determined by the above information.

Students are informed of their status by April/July for Fall and November /December for Spring.

Nail Technology

Program

Curriculum Code: NDP.NAIL.TRK

Application File Includes:

1. Complete the Lake Land College Intent to Enroll form and submit to the Admissions Office: choose the Nail Technology Major (NDP.NAIL.TRK).

2. Complete all Nail Technology intent packet forms and submit to Peggy Strange, Director of Cosmetology.

3. Enrollment into the program occurs in the fall semester only.

Completed PTA Application File Includes:

1. Submit Lake Land College Intent to Enroll form online or submit to the Admissions Office: choose the Physical Therapist Assistant Track major- (AAS.PTA.TRK).

2. Submit completed PTA Program Application online.

3. Complete Lake Land Compass Test - call (217) 234-5301 to schedule. 4. Submit transcripts to the Admissions Office by March 1.

a. Up-to-date high school transcript and final transcript upon graduation. b. All college transcripts.

5. Complete pre-requisite biology class with a grade of “C” or better within the past five years (If a high school senior must complete the class by graduation). One year of high school biology, anatomy, or zoology, or Lake Land College Bio-Science I (BIO100), or equivalent.

Students are informed of their status by April.

ONCE SELECTED, FINAL ADMISSION IS CONTINGENT UPON SUBMISSION OF THE FOLLOWING BY JULY 1:

1. Complete health examination and immunization form. 2. Proof of Hepatitis B vaccination series begun.

3. Confirm twenty (20) hours of observation in a Physical Therapy setting.

Paramedical Services

Curriculum Code: AAS.PS.

Complete Paramedical* Application File Includes:

1. Submit Intent to Enroll form, indicating your Academic Program Major as “AAS.PS”. 2. Take Lake Land’s placement test or submit ACT scores (www.act.org). Call 217-234-5301

to schedule. Students who test into reading will be required to take it their first semester. 3. Complete EMS/Paramedical Services paperwork in the EMS office, located in NW26,

217-234-5362 to schedule.

4. If you have taken college courses from another institution:

a. Request the official transcript to be sent to LLC Admissions Office. b. Complete the Transcript Evaluation form.

(in person or from the Admissions website).

5. Complete (or be currently registered for) EMS050, EMT Basic. Prerequisite: CPR certification for the American Red Cross Professional Rescuer OR American Heart Association Healthcare Provider OR take EMS016 through Lake Land College).

6. Complete (or currently registered for) BIO050, Basic A and P, OR BIO0225, and P I.
