1. Complete the second page (TAB 5) first and then fill in the totals from each schedule on page one (TAB 4)

where it indicates.

2. Only list those assets and liabilities that are legally titled in your individual name (listed in section 1) or

jointly with the Other Party (listed in section 2).

3. If you own real estate or business assets or have debts that are owned by another entity (ex. limited

liability company, limited partnership, corporation etc.) list the equity you have in these entities under

"Business Ventures" in the Assets column.

4. Under "Cash Surrender Value-Life Insurance" in the asset column only include the net cash surrender

value of the policy (i.e. the amount the insurance company would pay you if you canceled the policy) rather

than the death benefit payable to your beneficiary. The death benefit would be listed in Schedule E under

"Face Amount".
