facilities, and services in our UPMC Health Plan provider

network. The listings are designed to make it easy for you to find

doctors, health care facilities, and services near your home.

Service Area

The UPMC Health Plan provider listings are set up to make it easy to

find what you need. If you are looking for the name of a hospital,

service, or facility, simply go to the “Participating Hospitals” or

“Services and Facilities” sections in this book. All of the other listings

in this directory are divided by county and are in alphabetical order.

Go to the table of contents to locate the type of listing you want in the

county where you live.

This information is provided in the listings.

finding doctors, hospitals, and other providers in the UPMC Health Plan

network. The following pages give you information that will help you

use the listings. You will find the following information in this book:

Doctors.

Listings of primary care physicians (PCPs), pediatricians,

obstetricians and gynecologists (ob-gyns), and various types

of specialists.

Facilities.

Listings of hospitals, non-hospital facilities, behavioral

health facilities, and pharmacy locations.

Discounts.

Information on exclusive member discounts on LASIK eye

surgery and other refractive eye surgery. There is also information on

our Healthy Living Rewards program, which offers discounts on local

products and services that promote healthy lifestyles. See pages 421

through 434.

Alphabetical Index.

The index in the back of the book lists all

network providers. If you know the name of your provider, you can

use the index. The index will tell you which page includes the listing

you are looking for.

When choosing a provider, it is important that you have helpful

information. This book gives you some basic information about each

listed provider. Our Member Services representatives can direct you to

more information about many family practice doctors and specialists.

This information includes where the doctor went to medical school,

where he or she completed his or her residency, or other professional

PCPs and specialists may have one or more of the following codes

in their listing. The codes mean the following:

• SP

means specialty. Specialty means that the provider

may treat patients with a particular type of disease, illness,

or condition.

• BC

means board certification. To get a board certification,

a provider must pass a test on that specialty.

• HA

means hospital affiliation. These are hospitals where

the provider can send and treat his or her patients if they

need to be in a hospital.

Office Location. The provider’s address and telephone number are also

included in the listing. If the provider has more than one office, each

office is listed by ZIP code.

Language.

All providers speak English. To determine whether

providers speak another language or whether a provider is able to

communicate through sign language, contact our Member Services

Department at 1-888-876-2756 Monday through Friday from 8 a.m.

to 8 p.m. and Saturday from 8 a.m. to 3 p.m. TTY users should call

Gender.

Call the Member Services Department for more information

on whether a doctor is male or female.

Availability.

Almost all providers listed in this book accept new

patients. When you see the words “existing patients only,” that means

the provider is not accepting new patients.

Changing Your Provider.

If you have a primary care physician (PCP),

you can change to a new PCP at any time. The most convenient way is

to go to

and log in to

MyHealth OnLine. Select

MyBenefits and then the “Change Primary Care Physician” link. Or you

can call Member Services using the telephone number on the back of

your member ID card. If your ID card is not available, call Member

. We will help you pick a new provider who will

meet your needs. We will send you a new UPMC Health Plan member

ID card with the name and phone number of your new PCP.

Throughout this book you will see references to medical terms. The list

below will help you understand some of these terms.

Certified Nurse Midwife (CNM).

A registered, professional nurse

licensed to practice as a midwife. A midwife cares for pregnant

members and newborns while a woman is pregnant and after the

delivery of the baby.

Certified Registered Nurse Practitioner (CRNP).

A registered

nurse with more training in a specialty area. A CRNP is certified by a

board for that specialty area. A CRNP may diagnose a condition or

prescribe medicine under the direction of a doctor.

Family Practice Doctor.

A doctor who treats all family members.

These doctors see patients of all ages.

Internal Medicine Doctor.

A doctor who treats adult and older teen

illnesses. Most internal medicine doctors see patients who are age 16

and older.

Obstetrician.

A doctor who takes care of pregnant members and

delivers babies.

Ob-Gyn (Obstetrician-Gynecologist).

A doctor who cares for

pregnant members, delivers babies, and provides health services for

women. Services can include Pap smears, annual checkups, and

family planning services.

Ophthalmologist.

A doctor who treats eye problems. An ophthalmologist

may perform surgery and write prescriptions. Also, an ophthalmologist

may perform eye exams and fit patients for eyeglasses and contacts.

Optometrist.

A health care provider who performs eye examinations

and fits patients for eyeglasses or contact lenses.

Pediatrician.

A doctor who treats children. Most pediatricians see

patients age 21 and younger.

Physician Assistant (PA).

A health professional who is trained to

practice medicine with physician supervision. A PA may perform

physical exams, order and read lab tests, diagnose and treat illnesses,

prescribe medicine, and counsel patients.

Primary Care Practitioner (PCP).

PCPs provide or arrange for all

health care needs. Different types of PCPs include family and general

practice doctors, pediatricians, internal medicine doctors, and CRNPs.

Specialist.

A health care provider who treats a particular type of illness,

disease, or condition. For example, a cardiologist is a specialist who

cares for conditions and diseases of the heart, and a dermatologist is a

specialist for skin conditions and diseases. Throughout this book, you

will see terms used within each listing.

The list below will help you understand some of these terms.

• Accepting New Patients

. This term is used when a provider

or practice is willing and able to accept new patients.

• Accreditation.

Accreditation helps people to know whether

a certain facility was reviewed and met high standards. The

standards are set by several groups that closely review the

facilities. Two common hospital accreditation boards include

the Joint Commission on Accreditation of Healthcare Organizations

(JCAHO) and the American Osteopathic Association (AOA).

• Board Certification.

Board certification shows that a doctor

has earned a certificate from a certain medical group. The doctor

has to meet certain requirements of the board to get a certificate.

The groups require high standards and performance. Doctors

must also pass tests before being certified. Certification tells you

that the doctor went through extra studies and testing. UPMC

Health Plan recognizes board certification by either the American

Board of Medical Specialties (ABMS) or the American

Osteopathic Association (AOA).

How to Check Certification.

Doctors who have certification have taken

an extra career step. They have volunteered to have their qualifications

judged by a group of other physicians. You can check on a doctor’s

certification with certain groups that review medical providers. Check the

links below if you have a computer and Internet service.

American Osteopathic Information Association and the American

Osteopathic Association (AOA). It shows which doctors are certified by AOA.

A referral from a PCP is not needed to see a participating specialist.

If you have a PCP, the PCP should know that you are going to the

specialist so he or she can coordinate the care you may need.

If you have any questions about these services or need help in

choosing a provider, you can use the telephone number on the back

of your member ID card.

Behavioral Health Services.

If you need services for mental health,

drug abuse, and alcohol abuse, you may choose a participating

behavioral health provider. “Participating” means the provider has been

accepted into the provider network established for Health Plan members.

Emergency Care

If you have an emergency, go to the nearest emergency room, call

911, or call your local ambulance. After a visit to the emergency room,

you should call your doctor within 24 to 48 hours for any follow-up

care needed after a visit to the emergency room.

What if you need medical care when your

PCP’s office is closed?

PCPs, obstetrician-gynecologists, and behavioral health providers are

required to provide coverage 24 hours a day. If a provider has someone

else cover for him or her, that provider must also be participating in the

provider network affiliated with UPMC Health Plan.
