This directory is a list of Mid-Atlantic Permanente Medical Group physicians and other physicians affiliated with

Kaiser Permanente as of the date of publication. It is updated annually. The continuous availability of physicians,

medical centers, hospitals, and other providers cannot be guaranteed. To receive updated information about

our physicians, please refer to kp.org or call Member Services at 301-468-6000 or 1-800-777-7902. It is important

to know when you enroll in this plan, services are provided through the plan's delivery system, as described in

the plan's Federal brochure (RI 73-047).

Upon enrolling in Kaiser Permanente Signature, Kaiser Permanente for Individuals and Families, or Kaiser

Permanente Child Only, you must select a primary care physician from the physicians listed in this directory,

from those listed on our website, or by contacting Member Services. Choose Your Doctor at kp.org/doctor has

the most up-to-date listing of physicians with open panels. It is easy to select your personal physician there.

If you are a woman age 18–75, please select an obstetrician/gynecologist in addition to your primary care physician.

Federal members: It is important to know when you enroll in this plan that the services are provided through the

Kaiser Permanente delivery system, as described in the Kaiser Permanente federal brochure, but the continued

participation of any one doctor, hospital, or other provider cannot be guaranteed.

Note: Not all services are available at each medical center or site. Kaiser Permanente reserves the right to relocate

IT STARTS WITH YOUR PERSONAL PHYSICIAN

Your primary care physician (PCP) partners with you to help you live the

healthiest life possible. He or she coordinates your care, orders needed

tests, and consults with specialists, pharmacists, and others, as needed.

When you have a PCP, you have a physician who knows you, understands your health goals and

preferences, and advocates for you. If you’re a woman age 18 or older, you also have a personal

obstetrician/gynecologist who coordinates your special health care needs.

CONTINUING YOUR CARE WITH SPECIALISTS

Should you ever need a specialist (for example, a cardiologist,

neurologist, podiatrist, or dermatologist), you have access to doctors

who are able to address health care needs across more than 50 medical

specialties. Our specialists are experts in their fields and are ready to help diagnose and treat

you whether you have a broken bone or a chronic condition. Having a team that knows you and

is focused on you can lead to quicker, better diagnoses and treatments. At many locations, your

doctor, specialists, lab services, radiology, and pharmacy are all together in one place.

There are more than 1,000 physicians

in the multi-specialty Mid-Atlantic

Permanente Medical Group, P.C., who

treat Kaiser Permanente members like

you. They practice in Kaiser Permanente

medical centers. The group includes

physicians from major specialties and

subspecialties, behavioral health, and

high-risk prenatal care.

Because our

care model

reduces the

burden of

administrative hassles, insurance claim forms, and other paperwork for Permanente doctors,

nurses, and specialists, they’re free to focus on what they do best — delivering great care

that’s right for you.

Note: You may also choose from among affiliated physicians, who do not work for Kaiser

Permanente, for primary care. Affiliated physicians are in private practice in the community,

Your doctor’s office, plus so many added conveniences

You can choose your doctor from any of our medical centers in Maryland, Virginia, or the

District of Columbia. You can choose a physician close to home or close to work, whichever

is most convenient for you. If you have a family, you may choose one family practice

physician to care for all of your family, or you can have a different physician care for each

member of your family. While the choice is yours, what you’ll get is conveniences like onsite

X-ray, pharmacy, lab, and easier access to specialists. With our unique model, the hassles of

driving all over town for medical care often become a distant memory.

It’s all inside: The convenience of Kaiser Permanente medical centers

Because Kaiser Permanente staff and physicians respect your time, we

make sure that convenience extends beyond the walls of your medical

center. You can set telephone appointments with your doctor to save

trips to his or her office, and whenever you need medical advice, specially trained nurses

and doctors are on call 24 hours a day, seven days a week. We have developed technology

that lets your physician schedule a specialist appointment for you during your visit. Online,

you can email your doctor’s office, order most prescription refills, read most lab test results,

Your membership

Keep your membership ID card with you at all times . You will need it to make appointments,
